Yatay gen transferi yoluyla diren\u00e7 genlerini kolayl\u0131kla kazanabilen bu bakteri, \u00e7oklu ila\u00e7 direnci geli\u015fti\u011finde tedavide s\u0131kl\u0131kla son \u00e7are antibiyotik olan kolistin kullan\u0131lmaktad\u0131r (2). Ancak son y\u0131llarda kolistin diren\u00e7li su\u015flar\u0131n varl\u0131\u011f\u0131 bildirilmi\u015f ve bu direncin kromozomal mutasyonlar, plazmid arac\u0131l\u0131 genler ve lipopolisakkarit (LPS) modifikasyonlar\u0131yla ili\u015fkili oldu\u011fu g\u00f6sterilmi\u015ftir (3). Plazmid arac\u0131l\u0131 diren\u00e7ten sorumlu olan <i>mcr<\/i> genlerinin g\u0131da \u00fcr\u00fcnleri, \u00e7evresel \u00f6rnekler ve klinik izolatlarda saptanmas\u0131, direncin h\u0131zla yay\u0131labilece\u011fini ortaya koymaktad\u0131r (4). Bu genler, Gram-negatif bakterilerin d\u0131\u015f membran\u0131ndaki lipid A\u2019ya fosfoetanolamin eklenmesini sa\u011flayan transferaz enzimlerini kodlayarak kolistin direncine yol a\u00e7maktad\u0131r (5). <i>mcr<\/i> genleri aras\u0131nda en yayg\u0131n olan <i>mcr<\/i>-1, yayg\u0131n olarak <i>Escherichia coli, K. pneumoniae, Salmonella enterica<\/i>, <i>Enterobacter cloacae<\/i> ve <i>Klebsiella aerogenes<\/i>\u2019te g\u00f6r\u00fclmekle birlikte, deneysel olarak <i>Pseudomonas aeruginosa<\/i>\u2019ya da aktar\u0131labilmektedir. Bu genler, geni\u015flemi\u015f spektrumlu beta-laktamaz (GSBL), New Delhi metallo-beta-laktamaz (NDM) ve metallo-beta-laktamaz (MBL) gibi diren\u00e7 genleriyle birlikte ta\u015f\u0131narak pan-ila\u00e7 direncine zemin haz\u0131rlayabilmektedir (5). 2016 y\u0131l\u0131 ba\u015flar\u0131nda yap\u0131lan \u00e7al\u0131\u015fmalarda <i>mcr<\/i>-1\u2019in 18\u2019den fazla \u00fclkede bulundu\u011fu bildirilmi\u015f, bu durum yay\u0131l\u0131m h\u0131z\u0131n\u0131n ciddiyetini ortaya koymu\u015ftur. \u00c7iftlik hayvanlar\u0131, k\u00fcmes hayvanlar\u0131 ve insanlar \u00f6nemli rezervuarlar olarak tan\u0131mlanm\u0131\u015ft\u0131r (6). Ayr\u0131ca, Asya ve Avrupa\u2019n\u0131n farkl\u0131 \u00fclkelerinde yap\u0131lan \u00e7al\u0131\u015fmalarda <i>mcr-1<\/i>\u2019in geni\u015f co\u011frafi da\u011f\u0131l\u0131m\u0131 ortaya konulmu\u015f ve hayvanlardan insanlara ge\u00e7i\u015fin bir\u00e7ok \u00fclkede g\u00f6zlendi\u011fi bildirilmi\u015ftir (7). 2019 y\u0131l\u0131 itibar\u0131yla <i>mcr-1<\/i> ile <i>mcr-9<\/i> aras\u0131nda dokuz farkl\u0131 mobilize kolistin diren\u00e7 geni tan\u0131mlanm\u0131\u015f, 2020\u2019de bu listeye <i>mcr<\/i>-10 geni eklenmi\u015ftir (8). Otomatize sistemle kolistin diren\u00e7li bulunan izolatlar, referans y\u00f6ntem olan s\u0131v\u0131 mikrodil\u00fcsyon (SMD) y\u00f6ntemi ile test edildi. Kontrol olarak kolistin duyarl\u0131 <i>E. coli<\/i> ATCC\u00ae 25922 ve kolistin diren\u00e7li <i>E. coli<\/i> NCTC 13846 su\u015flar\u0131 kullan\u0131ld\u0131. 18\u201324 saat 37\u00b0C\u2019de ink\u00fcbasyon sonras\u0131nda, \u00fcremenin olmad\u0131\u011f\u0131 en d\u00fc\u015f\u00fck kuyucuktaki konsantrasyon de\u011feri minimum inhibit\u00f6r konsantrasyon (M\u0130K) de\u011feri olarak belirlendi. Kolistin duyarl\u0131l\u0131k sonu\u00e7lar\u0131, \u00e7al\u0131\u015fman\u0131n y\u00fcr\u00fct\u00fcld\u00fc\u011f\u00fc d\u00f6nemde ge\u00e7erli olan Avrupa Antimikrobiyal Duyarl\u0131l\u0131k Testleri Komitesi (European Committee on Antimicrobial Susceptibility Testing, EUCAST) 2022 kriterlerine g\u00f6re yorumland\u0131. Enterobacterales izolatlar\u0131nda kolistin i\u00e7in klinik s\u0131n\u0131r de\u011ferler M\u0130K \u22642 mg\/lt duyarl\u0131 ve M\u0130K &gt;2 mg\/lt diren\u00e7li olarak kabul edildi. Duyarl\u0131l\u0131k testleri, EUCAST 2022 \u00f6nerileri do\u011frultusunda yaln\u0131zca SMD y\u00f6ntemi kullan\u0131larak ger\u00e7ekle\u015ftirildi ve sonu\u00e7lar buna uygun olarak yorumland\u0131 (\u015eekil 1) (9).<\/p>\n<p>\u00c7al\u0131\u015fma, Zonguldak B\u00fclent Ecevit \u00dcniversitesi Giri\u015fimsel Olmayan Klinik Ara\u015ft\u0131rmalar Etik Kurulu taraf\u0131ndan 16 Eyl\u00fcl 2020 tarih ve 2020\/18 karar numaras\u0131yla onayland\u0131.<\/p>\n<h3><b>\u0130zolatlar\u0131n Epidemiyolojik Analizi<span class=\"Apple-converted-space\">\u00a0<\/span><\/b><\/h3>\n<p>\u0130zolatlar\u0131n epidemiyolojik \u00f6n analizi i\u00e7in rastgele ba\u015flat\u0131lm\u0131\u015f polimeraz zincir reaksiyonu (arbitrarily primed polymerase chain reaction, AP-PCR) y\u00f6ntemi uyguland\u0131 (10). Bakterilerden DNA elde edilmesi i\u00e7in haz\u0131r ticari kit kullan\u0131ld\u0131 (GeneAll Exgene\u2122 Clinic SV; GeneAll Biotechnology, Seul, G\u00fcney Kore).<\/p>\n<p>Amplifikasyon \u00fcr\u00fcnleri bromofenol mavisi i\u00e7inde seyreltildikten sonra, i\u00e7erisinde etidyum brom\u00fcr bulunan %2\u2019lik agaroz jele y\u00fcklendi. Marker olarak 100\u20132000 bp DNA ladder (GENESTA\u2122, GeneAll Biotechnology, Seoul, South Korea) kullan\u0131ld\u0131. Otomatize sistemle diren\u00e7li bulundu\u011fu halde SMD y\u00f6ntemiyle duyarl\u0131 oldu\u011fu tespit edilen 3 izolat\u0131n M\u0130K de\u011ferleri 0.5 \u03bcg\/ml, 1 \u03bcg\/ml ve 2 \u03bcg\/ml olarak belirlendi. \u0130zolatlardaki kolistin diren\u00e7 oran\u0131n\u0131n 2017 y\u0131l\u0131nda %1.38 oldu\u011fu, 2018 y\u0131l\u0131nda ise %0.18\u2019e d\u00fc\u015ft\u00fc\u011f\u00fc g\u00f6r\u00fcld\u00fc; ancak bu oran 2019 y\u0131l\u0131nda tekrar art\u0131\u015f g\u00f6stererek %1.61\u2019e ve 2020 y\u0131l\u0131nda daha da y\u00fckselerek %6.8\u2019e \u00e7\u0131kt\u0131\u011f\u0131 belirlendi (Tablo 3). \u00dc\u00e7 izolat karbapenemlerin t\u00fcm\u00fcne duyarl\u0131 iken 8 izolat karbapenem grubundan sadece imipeneme duyarl\u0131 olarak tespit edildi. Ayr\u0131ca izolatlar\u0131n tamam\u0131nda kinolon direnci saptand\u0131.<\/p>\n<h3><b>Hastalar\u0131n Demografik \u00d6zellikleri<span class=\"Apple-converted-space\">\u00a0<\/span><\/b><\/h3>\n<p>Belirtilen tarih aral\u0131\u011f\u0131nda 32 hastadan 41 kolistin diren\u00e7li izolat elde edildi. Bu izolatlar\u0131n elde edildi\u011fi hastalar\u0131n ya\u015f aral\u0131\u011f\u0131 20\u201392 y\u0131l (ortalama: 63 \u00b1 18 y\u0131l) ve erkek\/ kad\u0131n oran\u0131 20\/12 idi. Hastalar\u0131n %56\u2019s\u0131n\u0131n yo\u011fun bak\u0131m \u00fcnitesinde tedavi ald\u0131\u011f\u0131, %31\u2019inin de servis \u00f6ncesi yo\u011fun bak\u0131mda yat\u0131\u015f \u00f6yk\u00fcs\u00fcn\u00fcn mevcut oldu\u011fu belirlendi.<\/p>\n<h3><b>\u0130zolatlar\u0131n Molek\u00fcler Epidemiyolojisi<span class=\"Apple-converted-space\">\u00a0<\/span><\/b><\/h3>\n<p>\u00c7al\u0131\u015fmam\u0131zda yer alan 44 <i>K. pneumoniae<\/i> aras\u0131ndan, SMD y\u00f6ntemiyle kolistin direnci do\u011frulanan 41 izolat i\u00e7in AP-PCR analizi yap\u0131ld\u0131. \u0130zolatlarda iki farkl\u0131 AP-PCR profili g\u00f6zlendi ve k\u00fcmele\u015fme oran\u0131 %100 olarak bulundu. \u0130\u00e7inde 37 izolat\u0131n yer ald\u0131\u011f\u0131 genotip 1\u2019in en b\u00fcy\u00fck k\u00fcme oldu\u011fu belirlendi. Benzerlik s\u0131n\u0131r\u0131 %90 olan \u00e7al\u0131\u015fmam\u0131zda, genotip 1 ile genotip 2 aras\u0131nda %83,.6 gibi y\u00fcksek oranda benzerlik oldu\u011fu g\u00f6r\u00fcld\u00fc.<\/p>\n<h3><b>Plazmid Arac\u0131l\u0131 Diren\u00e7 Genlerinin Analizi<span class=\"Apple-converted-space\">\u00a0<\/span><\/b><\/h3>\n<p>Plazmid arac\u0131l\u0131 diren\u00e7 genleri olan <i>mcr-1, mcr-2, mcr-3, mcr-4<\/i> ve <i>mcr-5 <\/i>varl\u0131\u011f\u0131n\u0131n ara\u015ft\u0131r\u0131lmas\u0131 amac\u0131yla 41 kolistin diren\u00e7li <i>K. pneumoniae<\/i> izolat\u0131na multipleks PCR yap\u0131ld\u0131 ve 41 izolat\u0131n hi\u00e7birinde ara\u015ft\u0131r\u0131lan <i>mcr<\/i> genleri tespit edilmedi.<\/p>\n<h2><b>\u0130RDELEME<\/b><\/h2>\n<p>Karbapenem diren\u00e7li Enterobacterales\u2019in art\u0131\u015f\u0131, bir\u00e7ok \u00fclkede kolistin kullan\u0131m\u0131n\u0131 art\u0131rm\u0131\u015f ve ba\u015flang\u0131\u00e7ta yaln\u0131zca kromozomal mutasyonlara ba\u011fl\u0131 olan diren\u00e7, 2015\u2019ten itibaren diren\u00e7ten sorumlu <i>mcr<\/i> genlerinin plazmid arac\u0131l\u0131\u011f\u0131yla yay\u0131l\u0131m\u0131 ile k\u00fcresel bir sorun haline gelmi\u015ftir (12).<\/p>\n<p><i>K. pneumoniae, <\/i>hastane infeksiyonlar\u0131 ve antibiyotik direnci a\u00e7\u0131s\u0131ndan kritik \u00f6neme sahiptir. 2016 y\u0131l\u0131nda Avrupa, Kuzey Amerika ve G\u00fcneydo\u011fu Asya\u2019daki farkl\u0131 \u00fclkelerde <i>mcr-1 <\/i>geni ta\u015f\u0131yan su\u015flar rapor edilmi\u015f, ayn\u0131 y\u0131l \u0130talya\u2019da l\u00f6semili bir \u00e7ocuktan izole edilen KPC-3 enzimi \u00fcreten bir su\u015fta <i>mcr-1.2<\/i> varyant\u0131 bildirilmi\u015ftir (13,14). Fransa, L\u00fcbnan ve Laos\u2019ta da benzer bulgular rapor edilmi\u015ftir (15,16). 2017 y\u0131l\u0131nda G\u00fcney Kore\u2019de <i>mcr-1<\/i> ve <i>blaKPC-2<\/i> genlerini birlikte ta\u015f\u0131yan bir klinik su\u015f izole edilmi\u015ftir (17). Daha sonraki y\u0131llarda, L\u00fcbnan, Fas ve Katar\u2019da <i>mcr-8.1<\/i> geni; Bel\u00e7ika, Danimarka, Karada\u011f, Polonya, S\u0131rbistan, Slovenya, Romanya ve \u0130spanya\u2019daki hasta \u00f6rneklerinde ise <i>mcr-9<\/i> geni tan\u0131mlanm\u0131\u015ft\u0131r (18\u201320).<\/p>\n<p>2025 y\u0131l\u0131nda \u00c7in\u2019de kentsel kanalizasyon at\u0131k sular\u0131ndan elde edilen 366 izolat\u0131n 33\u2019\u00fc kolistine diren\u00e7li bulunmu\u015f, bu izolatlar\u0131n 3\u2019\u00fcnde (%0.83)<i>mcr<\/i> pozitifli\u011fi saptanm\u0131\u015f olup bunlar\u0131n biri <i>mcr-1 <\/i>ta\u015f\u0131yan <i>E. coli<\/i>, ikisi ise <i>mcr-10<\/i> ta\u015f\u0131yan <i>Klebsiella quasipneumoniae<\/i> subsp. <i>similipneumoniae<\/i> ve <i>Klebsiella variicola<\/i> subsp. <i>variicola<\/i> olarak belirlenmi\u015ftir. Daha \u00f6nce insan, hastane at\u0131ksuyu, evcil hayvan ve tavuk gibi di\u011fer hayvan \u00f6rnekleri de dahil olmak \u00fczere \u00e7e\u015fitli \u00e7evresel \u00f6rneklerde <i>mcr-10 <\/i>rapor edilmi\u015f olmakla birlikte \u00c7in\u2019de yap\u0131lan bir \u00e7al\u0131\u015fmada <i>mcr-10<\/i>\u2019un at\u0131ksu ar\u0131tma tesisinde tespit edilmesi bir ilktir. Bu durum kentsel at\u0131k sular\u0131n, antibiyotik diren\u00e7 genleri i\u00e7in \u00f6nemli bir rezervuar olabilece\u011fini ve halk sa\u011fl\u0131\u011f\u0131 i\u00e7in potansiyel bir risk olu\u015fturabilece\u011fini g\u00f6stermektedir (21).<span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p>\u00dclkemizde de <i>mcr<\/i> genleriyle ilgili ilk bildirim 2018 y\u0131l\u0131nda Hatay-Adana b\u00f6lgesinde tavuk etlerinden elde edilen <i>E. coli<\/i> izolatlar\u0131nda yap\u0131lm\u0131\u015ft\u0131r (22). <i>K. pneumoniae<\/i>\u2019n\u0131n insan kaynakl\u0131 su\u015flar\u0131nda ise <i>mcr<\/i>-1 2019 y\u0131l\u0131nda bildirilmi\u015ftir (23). Trakya \u00dcniversitesi\u2019nde 2024 y\u0131l\u0131nda yap\u0131lan bir \u00e7al\u0131\u015fmada, kolistin diren\u00e7li <i>K. pneumoniae<\/i> izolatlar\u0131n\u0131n tamam\u0131n\u0131n <i>mcr-1\u2013mcr-5<\/i> genleri a\u00e7\u0131s\u0131ndan negatif oldu\u011fu ve <i>E. coli<\/i> izolatlar\u0131nda kolistin direnci saptanmad\u0131\u011f\u0131 bildirilmi\u015ftir (24). Takip eden y\u0131llarda, kolistin diren\u00e7li<br \/>\n<i>K. pneumoniae<\/i> oranlar\u0131nda bir art\u0131\u015f g\u00f6zlemlenmi\u015f olmakla birlikte <i>mcr<\/i> bildirimlerinde dikkat \u00e7ekici bir art\u0131\u015f kaydedilmemi\u015ftir (25,26). \u00c7al\u0131\u015fmalar\u0131n bir b\u00f6l\u00fcm\u00fcnde izolatlar aras\u0131nda filogenetik inceleme yap\u0131lmad\u0131\u011f\u0131 i\u00e7in diren\u00e7 art\u0131\u015f\u0131 oranlar\u0131n\u0131n do\u011fru yorumlanmas\u0131 zordur. \u00d6zellikle hastane ortamlar\u0131nda kolistin diren\u00e7li su\u015flar\u0131n endemik hale gelmesi ve hastalar aras\u0131nda horizontal bula\u015fma ile infeksiyonlar\u0131n yay\u0131lmas\u0131, diren\u00e7 oranlar\u0131ndaki art\u0131\u015f nedeni olarak de\u011ferlendirilebilir. \u00c7al\u0131\u015fmam\u0131zda, yap\u0131lan epidemiyolojik \u00f6n analiz ile izolatlar aras\u0131nda iki farkl\u0131 genotip saptand\u0131 ve bu genotipler aras\u0131nda %83.6 oran\u0131nda benzerlik bulundu. Bu benzerlik, hastalar aras\u0131nda horizontal bula\u015fma olas\u0131l\u0131\u011f\u0131n\u0131 d\u00fc\u015f\u00fcnd\u00fcr\u00fcyor.<\/p>\n<p>T\u00fcrkiye\u2019de yap\u0131lan yak\u0131n tarihli bir \u00e7al\u0131\u015fmada, s\u0131\u011f\u0131r karkaslar\u0131ndan elde edilen <i>E. coli<\/i> O157 ve O157:H7 izolatlar\u0131nda <i>mcr-1<\/i> ve <i>mcr-5<\/i> genlerinin s\u0131ras\u0131yla %37.5 ve %25 oranlar\u0131nda saptand\u0131\u011f\u0131 bildirilmi\u015ftir (27). T\u00fcrkiye genelindeki marketlerden ve kasaplardan toplam 180 perakende et \u00f6rne\u011fi ile yap\u0131lan farkl\u0131 bir \u00e7al\u0131\u015fmada ise kuzu etlerinden elde edilen<br \/>\n<i>E. coli<\/i> izolatlar\u0131nda <i>mcr-1<\/i> ve <i>mcr-2 <\/i>genleri bulunmu\u015ftur (28). Ayr\u0131ca, Kas\u0131m 2020\u2013Eyl\u00fcl 2022 tarihleri aras\u0131nda farkl\u0131 su kaynaklar\u0131ndan al\u0131nan 600 \u00e7evresel su \u00f6rne\u011finden elde edilen <i>Aeromonas<\/i> spp. izolatlar\u0131nda <i>mcr-3 <\/i>ve <i>mcr-7<\/i> geni tespit edilmi\u015ftir (29). \u015eimdilik s\u0131n\u0131rl\u0131 say\u0131da olmakla birlikte, bu bulgular daha geni\u015f kapsaml\u0131 bir g\u00f6zetim ihtiyac\u0131n\u0131 ve g\u0131da ama\u00e7l\u0131 yeti\u015ftirilen hayvanlarda entegre izleme ve ihtiyatl\u0131 antimikrobiyal kullan\u0131m gereksiniminin \u00f6nemini ortaya koymaktad\u0131r.<\/p>\n<p>\u00c7al\u0131\u015fmam\u0131zda incelenen kolistin diren\u00e7li <i>K. pneumoniae<\/i> izolatlar\u0131nda diren\u00e7 oranlar\u0131n\u0131n art\u0131\u015f e\u011filiminde oldu\u011fu belirlendi. Y\u00fcksek M\u0130K de\u011ferleri ile birlikte karbapenem ve kinolon direncinin saptanmas\u0131, tedavi se\u00e7eneklerinin olduk\u00e7a s\u0131n\u0131rl\u0131 oldu\u011funu g\u00f6stermektedir. Epidemiyolojik \u00f6n analizlerde izolatlar\u0131n \u00e7o\u011funlukla tek bir genotip alt\u0131nda k\u00fcmelenmesi, izolatlar\u0131n klonal olarak ili\u015fkili olabilece\u011fini; plazmid arac\u0131l\u0131 <i>mcr<\/i> genlerinin tespit edilmemesinin ise kolistin direncinin b\u00fcy\u00fck olas\u0131l\u0131kla kromozomal mutasyonlara ba\u011fl\u0131 geli\u015fti\u011fini d\u00fc\u015f\u00fcnd\u00fcrd\u00fc. Bu bulgular, g\u00fcvenilir duyarl\u0131l\u0131k testlerinin uygulanmas\u0131, diren\u00e7li izolatlar\u0131n izlenmesi ve antimikrobiyal y\u00f6netim stratejilerinin g\u00fc\u00e7lendirilmesinin \u00f6nemini g\u00f6stermektedir.<\/p>\n<p>\u00c7al\u0131\u015fmam\u0131zda kolistin direnci de\u011ferlendirmesi EUCAST 2022 kriterlerine g\u00f6re yap\u0131lm\u0131\u015f olmakla birlikte, sonu\u00e7lar EUCAST 2026 g\u00fcncel \u00f6nerileri \u0131\u015f\u0131\u011f\u0131nda yeniden ele al\u0131nd\u0131\u011f\u0131nda yorumlar\u0131n b\u00fcy\u00fck \u00f6l\u00e7\u00fcde ge\u00e7erlili\u011fini korudu\u011fu g\u00f6r\u00fcld\u00fc. Her iki versiyonda da Enterobacterales i\u00e7in kolistin klinik s\u0131n\u0131r de\u011feri de\u011fi\u015fmedi (M\u0130K \u22642 mg\/lt duyarl\u0131, &gt;2 mg\/lt diren\u00e7li) ve referans y\u00f6ntem olarak SMD y\u00f6ntemi \u00f6nerisi korundu. Bununla birlikte EUCAST 2026\u2019ya g\u00f6re de\u011ferlendirildi\u011finde kolistin duyarl\u0131l\u0131k testlerinin klinik ve epidemiyolojik belirsizlikleri daha g\u00fc\u00e7l\u00fc bi\u00e7imde vurguland\u0131\u011f\u0131 ve kolistinin \u201crutin antibiyogram antibiyoti\u011fi\u201d olarak kullan\u0131lmamas\u0131 gerekti\u011finin a\u00e7\u0131k\u00e7a belirtildi\u011fi g\u00f6r\u00fcld\u00fc. Bu yakla\u015f\u0131m, EUCAST 2022\u2019de mevcut olan metodolojik uyar\u0131lar\u0131n 2026 versiyonunda daha belirgin ve k\u0131s\u0131tlay\u0131c\u0131 hale geldi\u011fini g\u00f6stermektedir. \u00d6zellikle EUCAST 2026\u2019da s\u0131n\u0131r M\u0130K de\u011ferlerine (2 mg\/lt) sahip izolatlar\u0131n klinik a\u00e7\u0131dan dikkatle de\u011ferlendirilmesi gerekti\u011fi vurgulanm\u0131\u015f; heterorezistans ve plazmid arac\u0131l\u0131 <i>mcr<\/i> genlerinin fenotipik testlerle her zaman saptanamayabilece\u011fi belirtilmi\u015ftir. \u00c7al\u0131\u015fmam\u0131zda EUCAST 2022\u2019ye g\u00f6re duyarl\u0131 olarak s\u0131n\u0131fland\u0131r\u0131lan baz\u0131 izolatlar\u0131n, EUCAST 2026 bak\u0131\u015f a\u00e7\u0131s\u0131yla klinik belirsizlik potansiyeli ta\u015f\u0131yabilece\u011fi de\u011ferlendirilmektedir. Ayr\u0131ca EUCAST 2026, kolistin kullan\u0131m\u0131na ili\u015fkin klinik \u00f6nerileri daha da daraltm\u0131\u015f; \u00f6zellikle <i>K. pneumoniae<\/i> gibi \u00e7ok ilaca diren\u00e7li etkenlerde kolistinin m\u00fcmk\u00fcn oldu\u011funca kombinasyon tedavilerinde ve uygun doz optimizasyonu ile kullan\u0131lmas\u0131n\u0131 \u00f6nermi\u015ftir (30).<\/p>\n<p>\u0130zolatlar\u0131m\u0131zda kolistin diren\u00e7 oranlar\u0131n\u0131n y\u0131llara g\u00f6re de\u011fi\u015fkenlik g\u00f6sterdi\u011fi, 2020 y\u0131l\u0131nda ise belirgin bir \u015fekilde y\u00fckseldi\u011fi saptand\u0131. Bu art\u0131\u015f\u0131n nedeninin tek bir etkene indirgenemeyecek bi\u00e7imde, hastane ortam\u0131ndaki \u00e7ok say\u0131da fakt\u00f6r\u00fcn e\u015f zamanl\u0131 etkisiyle ili\u015fkili olabilece\u011fi de\u011ferlendirilmektedir.
