{"id":31060,"date":"2025-12-29T12:35:27","date_gmt":"2025-12-29T09:35:27","guid":{"rendered":"https:\/\/www.klimikdergisi.org\/?p=31060"},"modified":"2026-01-15T11:54:39","modified_gmt":"2026-01-15T08:54:39","slug":"enterokoklarda-molekuler-analiz","status":"publish","type":"post","link":"https:\/\/www.klimikdergisi.org\/tr\/2025\/12\/29\/enterokoklarda-molekuler-analiz\/","title":{"rendered":"Enterokok \u0130zolatlar\u0131nda T\u00fcrlerin ve Vankomisin Direncinin Molek\u00fcler Tespiti"},"content":{"rendered":"<h2><b>G\u0130R\u0130\u015e<\/b><\/h2>\n<p>Gerek toplum gerekse hastane k\u00f6kenli infeksiyonlara neden olan patojen bakterilerin tedavisinde antibiyotik direnci, d\u00fcnyada ve \u00fclkemizde giderek artan bir sorundur (1). <i>Enterococcus<\/i> t\u00fcrleri insan ve hayvanlar\u0131n sindirim sisteminin normal floras\u0131nda bulunan ancak bakteriyemi, endokardit, \u00fcriner sistem infeksiyonlar\u0131, intraabdominal ve pelvik infeksiyonlar, yara ve doku infeksiyonlar\u0131, kolesistit, menenjit, neonatal sepsis, hastane kaynakl\u0131 pn\u00f6moni ve septisemi gibi infeksiyonlara da neden olabilen f\u0131rsat\u00e7\u0131 patojen bakterilerdir (2).<span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p>\u00c7oklu antibiyotik direnci g\u00f6steren <i>Enterococcus<\/i> izolatlar\u0131n\u0131n ortaya \u00e7\u0131kmas\u0131 ile mevcut tedavi y\u00f6ntemlerinin uygulanabilirli\u011fi k\u0131s\u0131tlanm\u0131\u015f; glikopeptid, penisilin ve aminoglikozit grubu antibiyotiklere diren\u00e7 genleri ta\u015f\u0131yan enterokok infeksiyonlar\u0131 halk sa\u011fl\u0131\u011f\u0131 a\u00e7\u0131s\u0131ndan \u00f6nemli bir tehdit haline gelmi\u015ftir (3). Vankomisine diren\u00e7li enterokok (Vancomycin-resistant <i>Enterococcus<\/i>, VRE) su\u015flar\u0131, infekte veya kolonize hastalar, hastane personeli ve \u00e7evre kontaminasyonu arac\u0131l\u0131\u011f\u0131yla yay\u0131lmakta; salg\u0131nlara ve tedavisi g\u00fc\u00e7 infeksiyonlara neden olabilmektedir. Bu nedenle, klinik \u00f6rnekler, rektal s\u00fcr\u00fcnt\u00fcler ve \u00e7evre \u00f6rneklerinde VRE su\u015flar\u0131n\u0131n ara\u015ft\u0131r\u0131lmas\u0131, yay\u0131l\u0131m\u0131n \u00f6nlenmesi a\u00e7\u0131s\u0131ndan \u00f6nem ta\u015f\u0131maktad\u0131r (4).<span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p>Glikopeptid grubu antibiyotikler (vankomisin, teikoplanin) enterokoklarda peptidoglikan sentezini, dolay\u0131s\u0131yla h\u00fccre duvar\u0131 sentezini engelleyerek etki g\u00f6sterir. H\u00fccre duvar\u0131n\u0131n ana bile\u015feni olan peptidoglikan sentezi s\u0131ras\u0131nda iki D-alanin molek\u00fcl\u00fcn\u00fcn birbirine ba\u011flanmas\u0131, bir ligaz enzimi (<i>ddl<\/i>) taraf\u0131ndan katalize edilir. Olu\u015fan D-alanin-D-alanin dipeptidi, devam eden enzimatik reaksiyonlarla UDP-N-asetilmuramil-tripeptide eklenir ve UDP-N-asetilmuramil-pentapeptidi olu\u015fturulur. Bu pentapeptit molek\u00fcl\u00fc, transglikolizasyon yoluyla sitoplazmik membran\u0131n d\u0131\u015f y\u00fczeyine ta\u015f\u0131narak peptidoglikan sentezine kat\u0131l\u0131r. Pentapeptidin serbest karboksil ucunda bulunan terminal D-alanin-D-alanin b\u00f6lgesine y\u00fcksek afinite ile ba\u011flanan vankomisin, peptidoglikan sentezinin transglikolizasyon basama\u011f\u0131n\u0131 inhibe ederek h\u00fccre duvar\u0131 sentezini durdurur ve bakterisidal etki olu\u015fturur.<span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p>Enterokoklarda vankomisin direnci, d\u00fc\u015f\u00fck afiniteli pentapeptit \u00f6nc\u00fcllerinin sentezlenmesinden kaynaklan\u0131r. Vankomisine diren\u00e7li enterokok su\u015flar\u0131 farkl\u0131 bir ligaz enzimi sentezleyerek D-alanin-D-alanin\u2019deki alanin yerine laktat veya serin ba\u011flar; bu de\u011fi\u015fiklik u\u00e7 k\u0131sm\u0131n yap\u0131s\u0131n\u0131 de\u011fi\u015ftirerek vankomisinin hedef b\u00f6lgeye ba\u011flanma yetene\u011fini azalt\u0131r. B\u00f6ylece D-alanin-D-laktat veya D-alanin-D-serin i\u00e7eren modifiye pentapeptidler vankomisinin h\u00fccre duvar\u0131na ba\u011flanma yetene\u011finin azalmas\u0131na neden olur.<span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p>Enterokoklarda iki tip vankomisin direnci g\u00f6r\u00fcl\u00fcr. Birincisi, kromozom \u00fczerinde yerle\u015fik genlere ba\u011fl\u0131 do\u011fal (intrensek) diren\u00e7tir. Modifiye D-alanin-D-serin ta\u015f\u0131yan <i>Enterococcus gallinarum<\/i> ve <i>Enterococcus casseliflavus<\/i> izolatlar\u0131 vankomisine do\u011fal fakat d\u00fc\u015f\u00fck d\u00fczeyli bir diren\u00e7 g\u00f6sterir. \u0130kinci tip diren\u00e7 ise ba\u015fka bir organizmadan genetik transferle kazan\u0131lm\u0131\u015f (ekstrensek) diren\u00e7tir. Bu diren\u00e7 tipi en s\u0131k modifiye D-alanin-D-laktat ta\u015f\u0131yan <i>Enterococcus faecium<\/i> ve <i>Enterococcus faecalis<\/i> izolatlar\u0131nda g\u00f6r\u00fcl\u00fcr (5).<\/p>\n<p>Bug\u00fcne kadar enterokoklarda klinik a\u00e7\u0131dan farkl\u0131 \u00f6neme sahip dokuz vankomisin diren\u00e7 tipi (v<i>anA, vanB, vanC, vanD, vanE, vanG, vanL, vanM<\/i> ve <i>vanN<\/i>) tan\u0131mlanm\u0131\u015ft\u0131r. Klinik olgular\u0131n \u00e7o\u011funu olu\u015fturan <i>E. faecium<\/i> ve <i>E. faecalis<\/i> t\u00fcrlerinde s\u0131ras\u0131yla VanA ve VanB direncinin ve daha az g\u00f6r\u00fclen <i>E. gallinarum<\/i> t\u00fcr\u00fcnde ise VanC-1 direncinin yayg\u0131n oldu\u011fu bildirilmi\u015ftir (6).<\/p>\n<p>Enterokok izolatlar\u0131n\u0131n t\u00fcr ve vankomisin diren\u00e7 tespitinde, konvansiyonel ve k\u00fclt\u00fcre dayal\u0131 fenotipik testlerin yan\u0131 s\u0131ra, t\u00fcr ve diren\u00e7 genlerinin h\u0131zl\u0131 ve do\u011fru bi\u00e7imde belirlenmesine olanak sa\u011flayan molek\u00fcler teknikler de geli\u015ftirilmi\u015ftir. Bu teknikler aras\u0131nda t\u00fcre \u00f6zg\u00fc de\u011fi\u015fkenlik g\u00f6steren D-alanin-D-alanin ligazlar\u0131n\u0131 kodlayan <i>ddl<\/i> genlerinin ve vankomisin diren\u00e7 genlerinin polimeraz zincir reaksiyonu (PCR) ile amplifikasyonu yer almaktad\u0131r (7-10).<\/p>\n<p>Bu \u00e7al\u0131\u015fmada, h\u0131zl\u0131 ve pratik olan multipleks-PCR (mPCR) y\u00f6ntemi kullan\u0131larak hastanenin servis ve yo\u011fun bak\u0131m \u00fcnitelerindeki hastalar\u0131n \u00e7e\u015fitli klinik \u00f6rneklerinden izole edilen 38 <i>Enterococcus<\/i> izolat\u0131n\u0131n t\u00fcr ve diren\u00e7 profillerinin genotipik olarak belirlenmesi ve bulgular\u0131n konvansiyonel k\u00fclt\u00fcr test sonu\u00e7lar\u0131yla kar\u015f\u0131la\u015ft\u0131r\u0131lmas\u0131 planland\u0131. Bu ama\u00e7la, klinik enterokok izolatlar\u0131nda en s\u0131k g\u00f6r\u00fclen <i>E. faecium<\/i>, <i>E. faecalis<\/i> ve <i>E. gallinarum<\/i> t\u00fcrlerinin da\u011f\u0131l\u0131m\u0131 ile <i>vanA, vanB ve vanC-1<\/i> diren\u00e7 genlerinin varl\u0131\u011f\u0131, tek reaksiyonda ve y\u00fcksek duyarl\u0131l\u0131kla tespit edebilen optimize bir mPCR protokol\u00fc ile ara\u015ft\u0131r\u0131ld\u0131 (11).<\/p>\n<h2><b>Y\u00d6NTEMLER<\/b><\/h2>\n<p><strong>Klinik \u0130zolatlar ve Referans Su\u015flar<\/strong><\/p>\n<p>2015\u20132019 y\u0131llar\u0131 aras\u0131nda hastanenin servis ve yo\u011fun bak\u0131m \u00fcnitelerinde yatan hastalardan al\u0131nan klinik \u00f6rneklerden T\u0131bbi Mikrobiyoloji Laboratuvar\u0131na\u2019nda izole edilip k\u00fclt\u00fcr koleksiyonunda saklanan 38 <i>Enterococcus<\/i> izolat\u0131 \u00e7al\u0131\u015fmaya dahil edildi.<span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p>Klinik \u00f6rnekler koyun kanl\u0131 agar ve \u201ceosin methylene blue\u201d (EMB) agara ekilerek 37\u02daC\u2019de 24\u201348 saat ink\u00fcbe edildi. \u0130nk\u00fcbasyon sonras\u0131 kanl\u0131 agarda uygun koloni morfolojisine sahip \u00f6rneklere Gram boyamas\u0131, katalaz testi, L-pirolidonil-\u03b2-naftilamid testi (PYR) testi gibi rutin laboratuvar y\u00f6ntemleri uyguland\u0131.<span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p>Enterokok \u015f\u00fcphesi bulunan su\u015flar\u0131n t\u00fcr d\u00fczeyinde tan\u0131mlanmas\u0131 ve antibiyotik duyarl\u0131l\u0131k testleri VITEK\u00ae 2 (bioM\u00e9rieux, Marcy l\u2019Etoile, Fransa) otomatize sistemi ile yap\u0131ld\u0131. Otomatize sistemde vankomisin diren\u00e7li bulunan izolatlarda, Avrupa Antimikrobiyal Duyarl\u0131l\u0131k Testi Komitesi (European Committee on Antimicrobial Susceptibility Testing, EUCAST) \u00f6nerileri do\u011frultusunda Mueller-Hinton agar (MHA) besiyeri kullan\u0131larak vankomisin (5\u00b5g) diski ile Kirby-Bauer disk dif\u00fczyon testi uyguland\u0131 (12). Referans su\u015flar (VanA diren\u00e7li <i>E. faecium<\/i>, VanB diren\u00e7li <i>E. faecalis<\/i> ve VanC-1 diren\u00e7li <i>E. gallinarum<\/i>) T\u00fcrkiye Halk Sa\u011fl\u0131\u011f\u0131 Kurumu Molek\u00fcler Mikrobiyoloji Ara\u015ft\u0131rma Laboratuvar\u0131\u2019ndan temin edilerek, t\u00fcr ve vankomisin diren\u00e7 genleri i\u00e7in pozitif kontrol olarak kullan\u0131ld\u0131.<\/p>\n<p>Bu \u00e7al\u0131\u015fma i\u00e7in Ahi Evran \u00dcniversitesi Giri\u015fimsel Olmayan Klinik Ara\u015ft\u0131rmalar Etik Kurulu\u2019ndan 20 Ekim 2020 tarih ve 2020-15\/106 karar numaras\u0131yla onay al\u0131nd\u0131.<\/p>\n<p><strong>DNA \u0130zolasyonu<\/strong><\/p>\n<p>Molek\u00fcler analizlerde kullan\u0131lacak DNA\u2019n\u0131n elde edilmesi i\u00e7in -80\u00b0C\u2019de saklanan bakteri izolatlar\u0131 kat\u0131 besiyerinde \u00fcretilerek canland\u0131r\u0131ld\u0131. Referans ve klinik <i>Enterococcus<\/i> izolatlar\u0131n total DNA\u2019s\u0131 kaynatma y\u00f6ntemiyle elde edildi (13). T\u00fcpler vorteksle kar\u0131\u015ft\u0131r\u0131ld\u0131ktan sonraT100\u2122\uf87f\uf87f Thermal Cycler (Bio-Rad Laboratories, Hercules, CA, ABD) cihaz\u0131nda amplifikasyon i\u015flemi ger\u00e7ekle\u015ftirildi.<\/p>\n<p><strong>Agaroz Jel Elektroforezi<\/strong><\/p>\n<p>Referans su\u015flar ve klinik izolatlardan elde edilen mPCR \u00fcr\u00fcnleri %2\u2019lik agaroz jelde elektroforezle ayr\u0131\u015ft\u0131r\u0131ld\u0131. DNA bantlar\u0131, Gel Doc\u2122 XR g\u00f6r\u00fcnt\u00fcleme sistemiyle (Bio-Rad Laboratories, Hercules, CA, ABD) g\u00f6r\u00fcnt\u00fclendi. DNA i\u00e7ermeyen negatif kontrol \u00f6rne\u011finde beklendi\u011fi \u00fczere herhangi bir amplifikasyon \u00fcr\u00fcn\u00fc g\u00f6zlenmedi.<span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p>Ayn\u0131 jel \u00fczerinde analiz edilen 14 klinik izolat\u0131n t\u00fcr ve diren\u00e7 profilleri, amplifikasyon bantlar\u0131n\u0131n varl\u0131\u011f\u0131na g\u00f6re de\u011ferlendirildi (\u015eekil 1). G\u00fcn\u00fcm\u00fczde gerek hastane gerekse toplum k\u00f6kenli <i>Enterococcus<\/i> izolatlar\u0131nda vankomisin direncinin olduk\u00e7a y\u00fcksek d\u00fczeylere ula\u015ft\u0131\u011f\u0131 bildirilmektedir ve vankomisin direncinden sorumlu genlerin tespiti ve prevalans\u0131n\u0131n takibi hastanelerde diren\u00e7 yay\u0131l\u0131m\u0131n\u0131n \u00f6nlenmesi a\u00e7\u0131s\u0131ndan son derece \u00f6nemlidir (2).<span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p><i>Enterococcus<\/i> t\u00fcrlerinde vankomisine diren\u00e7 geli\u015fimine katk\u0131da bulunan dokuz farkl\u0131 tipte diren\u00e7 operonu (<i>vanA, vanB, vanC, vanD, vanE, vanG, vanL, vanM ve vanN<\/i>) tan\u0131mlanm\u0131\u015ft\u0131r. Bunlar aras\u0131nda VanA ve VanB kaynakl\u0131 diren\u00e7 klinik olarak en \u00f6nemlileridir. VanA tipi diren\u00e7 hem vankomisine hem de teikoplanine y\u00fcksek d\u00fczeyde diren\u00e7 kazand\u0131r\u0131rken, VanB tipi diren\u00e7 yaln\u0131zca vankomisine kar\u015f\u0131 etkilidir. Hem <i>vanA<\/i> hem de vanB gen k\u00fcmeleri s\u0131ras\u0131yla Tn1546 ve Tn1549 mobil elementleri (transpozonlar) \u00fczerinde yerle\u015fik oldu\u011fundan <i>vanA<\/i> ve <i>vanB<\/i> arac\u0131l\u0131 vankomisin direnci do\u011frudan bu mobil elementler ya da bunlar\u0131 ta\u015f\u0131yan plazmidlerin izolatlar aras\u0131nda transferi ile yay\u0131lmaktad\u0131r (15). Buna kar\u015f\u0131l\u0131k, kromozomal lokasyonu nedeniyle aktar\u0131lamayan bir operon olan <i>vanC<\/i> gen k\u00fcmesi <i>E. gallinarum<\/i> ve <i>E. casseliflavus<\/i> izolatlar\u0131nda vankomisine d\u00fc\u015f\u00fck d\u00fczeyde diren\u00e7 olu\u015fturur (16).<\/p>\n<p>\u0130nsan klinik \u00f6rneklerinden izole edilen VRE\u2019ler aras\u0131nda en s\u0131k rastlanan t\u00fcr VanA tipi diren\u00e7li <i>E. faecium<\/i>\u2019dur; bunu VanB diren\u00e7li <i>E. faecalis<\/i> izlemektedir. Klinik \u00f6rneklerde daha seyrek g\u00f6r\u00fclen ve bu nedenle \u00f6nemsiz oldu\u011fu d\u00fc\u015f\u00fcn\u00fclen VanC grubu diren\u00e7 ta\u015f\u0131yan <i>E. gallinarum<\/i> ve <i>E. casseliflavus<\/i> t\u00fcrlerinden \u00f6zellikle <i>E. gallinarum<\/i>\u2019un imm\u00fcn yetmezlik sorunu olan hastalarda bakteriyemi, endokardit ve menenjit gibi ciddi infeksiyonlara neden olabildi\u011fi bildirilmi\u015ftir (17). Bu t\u00fcrlerin do\u011fru tan\u0131mlanmas\u0131 yaln\u0131zca tedavi i\u00e7in de\u011fil, ayn\u0131 zamanda hastane i\u00e7i bula\u015fman\u0131n \u00f6nlenmesi a\u00e7\u0131s\u0131ndan da \u00f6nemlidir.<span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p>Son y\u0131llarda yurtd\u0131\u015f\u0131nda ve \u00fclkemizde yap\u0131lan baz\u0131 \u00e7al\u0131\u015fmalarda VanA tipi diren\u00e7li <i>E. faecium<\/i> etkenli VRE infeksiyonlar\u0131n\u0131n oranlar\u0131nda art\u0131\u015f bildirilmektedir (18-22). Benzer \u015fekilde, \u00e7al\u0131\u015fmam\u0131zda da 2015\u20132019 y\u0131llar\u0131 aras\u0131nda izole edilen 38 klinik <i>Enterococcus<\/i> izolat\u0131 aras\u0131nda, konvansiyonel k\u00fclt\u00fcr testleriyle %66, mPCR analizleriyle %61 oran\u0131nda VanA diren\u00e7li <i>E. faecium<\/i> saptanm\u0131\u015f ve bu t\u00fcr en y\u00fcksek oranda belirlenmi\u015ftir. <span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p>T\u00fcr ve diren\u00e7 belirlemede konvansiyonel testler ile mPCR y\u00f6ntemi aras\u0131nda 38 \u00f6rne\u011fin 31 (%82)\u2019inde tam uyum saptanm\u0131\u015ft\u0131r. Elde edilen sonu\u00e7lar, \u00e7al\u0131\u015fmada kullan\u0131lan optimize mPCR testinin ba\u015far\u0131l\u0131, h\u0131zl\u0131, pratik ve ekonomik bir y\u00f6ntem oldu\u011funu g\u00f6stermektedir. Konvansiyonel ve otomatize k\u00fclt\u00fcr sistemleri, VRE izolatlar\u0131n\u0131n yaln\u0131zca vankomisine diren\u00e7li veya duyarl\u0131 oldu\u011funu belirleyebilirken, mPCR testi diren\u00e7 geninin tipini (<i>vanA, vanB ve vanC-1<\/i>) de ortaya koyabilmektedir. Bu durum tedavi se\u00e7imini de\u011fi\u015ftirmese de olas\u0131 gen transferinin ve VRE yay\u0131l\u0131m\u0131n\u0131n izlenmesi a\u00e7\u0131s\u0131ndan klinik ve epidemiyolojik a\u00e7\u0131dan \u00f6nem ta\u015f\u0131maktad\u0131r.<span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p>\u00c7al\u0131\u015fmam\u0131zda iki y\u00f6ntem aras\u0131nda %18 oran\u0131nda tan\u0131sal farkl\u0131l\u0131k g\u00f6zlenmesi t\u00fcr ve diren\u00e7 tespitinde konvansiyonel testler ile birlikte molek\u00fcler tan\u0131n\u0131n \u00f6nemli ve gerekli oldu\u011funu g\u00f6stermektedir. T\u00fcr d\u00fczeyinde kar\u015f\u0131la\u015ft\u0131r\u0131ld\u0131\u011f\u0131nda \u00f6zellikle <i>E. gallinarum<\/i> tan\u0131mlamas\u0131nda farkl\u0131l\u0131klar saptand\u0131. K\u00fclt\u00fcr y\u00f6ntemiyle <i>E. faecium<\/i> olarak tan\u0131mlanan bir izolat, <i>E. casseliflavus<\/i> olarak tan\u0131mlanan 2 izolat ve tan\u0131mlanamayan 3 izolat, mPCR y\u00f6ntemiyle <i>E. gallinarum<\/i> olarak belirlendi.<span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p>Fang ve arkada\u015flar\u0131 (23) taraf\u0131ndan yap\u0131lan bir \u00e7al\u0131\u015fmada, Bruker MS, VITEK MS, VITEK 2 sistemleri ve mPCR y\u00f6ntemi kar\u015f\u0131la\u015ft\u0131r\u0131lm\u0131\u015f; VITEK 2\u2019nin 132 <i>Enterococcus<\/i> izolat\u0131n %91.7\u2019sini t\u00fcr d\u00fczeyinde do\u011fru tan\u0131mlayabildi\u011fi bildirilmi\u015ftir. Ancak, 21 <i>E. gallinarum<\/i> izolat\u0131ndan 17\u2019si do\u011fru tan\u0131mlanm\u0131\u015f, 9 izolat <i>E. casseliflavus<\/i> ile <i>E. gallinarum<\/i> aras\u0131nda ay\u0131rt edilememi\u015ftir. VITEK 2 sisteminin <i>E. faecalis<\/i> ve <i>E. faecium<\/i> d\u0131\u015f\u0131ndaki <i>Enterococcus<\/i> t\u00fcrlerini tan\u0131mlamada daha az verimli oldu\u011fu bildirilmi\u015ftir (23). Ba\u015fka bir \u00e7al\u0131\u015fmada, VITEK 2 ve MALDI-TOF MS sistemleri ile 16S rRNA gen dizi analizi y\u00f6ntemi kar\u015f\u0131la\u015ft\u0131r\u0131lm\u0131\u015f; <i>E. gallinarum<\/i> izolatlar\u0131n\u0131n VITEK 2 sistemi taraf\u0131ndan do\u011fru tan\u0131mland\u0131\u011f\u0131, ancak, sistemin genel olarak daha d\u00fc\u015f\u00fck ay\u0131rt edicili\u011fe sahip oldu\u011fu ve 39 izolat\u0131n 10\u2019unun yanl\u0131\u015f tan\u0131mlad\u0131\u011f\u0131 bildirilmi\u015ftir. \u00d6rne\u011fin, 6 <i>Enterococcus durans<\/i> izolat\u0131ndan 2\u2019si <i>E. gallinarum<\/i> olarak yanl\u0131\u015f tan\u0131mlanm\u0131\u015ft\u0131r. Bu bulgular sonucunda \u00e7al\u0131\u015fmada, otomatize tan\u0131mlama sistemlerinin benzer biyokimyasal \u00f6zelliklere sahip t\u00fcrlerde s\u0131n\u0131rl\u0131 ay\u0131rt edicili\u011fe sahip oldu\u011fu ve veritabanlar\u0131n\u0131n g\u00fcncellenmesi gerekti\u011fi ifade edilmi\u015ftir (24).<\/p>\n<p>\u00c7al\u0131\u015fmam\u0131zda, konvansiyonel k\u00fclt\u00fcr y\u00f6ntemleriyle vankomisin diren\u00e7li olarak saptanan 3 izolatta mPCR y\u00f6ntemiyle diren\u00e7 geni saptanamad\u0131. Benzer \u015fekilde, literat\u00fcrde de konvansiyonel ve molek\u00fcler y\u00f6ntemlerin de\u011ferlendirildi\u011fi \u00e7al\u0131\u015fmalarda diren\u00e7 saptanamayan veya hatal\u0131 saptanan izolatlar bildirilmi\u015ftir. Bilen ve arkada\u015flar\u0131 (22) taraf\u0131ndan yap\u0131lan bir \u00e7al\u0131\u015fmada, otomatize identifikasyon sistemi ile t\u00fcm <i>Enterococcus<\/i> su\u015flar\u0131n\u0131n vankomisin ve teikoplanine diren\u00e7li bulundu\u011fu; ancak gradyan test y\u00f6ntemine g\u00f6re \u00fc\u00e7 izolat\u0131n her iki antibiyoti\u011fe de duyarl\u0131 oldu\u011fu, bu su\u015flarda <i>vanA,<\/i> <i>vanB<\/i> ve <i>vanC<\/i> genlerinin hi\u00e7birinin saptanmad\u0131\u011f\u0131 bildirilmi\u015ftir (25). Ayr\u0131ca bu durum, kullan\u0131lan molek\u00fcler testin kapsay\u0131c\u0131l\u0131\u011f\u0131na (\u00f6rne\u011fin <i>vanC2-C3<\/i> gen b\u00f6lgelerini hedeflemesi) ba\u011fl\u0131 olarak da de\u011fi\u015febilir. Ancak y\u00f6ntemin rutin kullan\u0131ma ge\u00e7ebilmesi i\u00e7in kitin maliyet etkinli\u011fi a\u00e7\u0131s\u0131ndan geli\u015ftirilmesi ve kapsaml\u0131 validasyon \u00e7al\u0131\u015fmalar\u0131 gerekmektedir.<span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p>Yap\u0131lan \u00e7al\u0131\u015fmalar, VRE\u2019lerde y\u00fcksek d\u00fczeyde glikopeptid direncine neden olan genlerin, genellikle y\u00fcksek kopya say\u0131s\u0131na ve y\u00fcksek stabiliteye sahip plazmidler ile bu plazmidlere entegre olmu\u015f transpozonlar \u00fczerinde bulundu\u011funu g\u00f6stermektedir. Ayr\u0131ca bu diren\u00e7 genleri, plazmid veya kromozomlara entegre olabilen konjugatif transpozonlar arac\u0131l\u0131\u011f\u0131yla da kazan\u0131labilir. Bu transpozonlar, i\u00e7erdikleri insersiyon dizileri arac\u0131l\u0131\u011f\u0131yla farkl\u0131 konak DNA\u2019lara entegre olma yetene\u011fine sahiptir (26). Literat\u00fcrde, insan klinik \u00f6rneklerinden hem <i>vanC-1<\/i> hem <i>vanA<\/i> diren\u00e7 genlerini birlikte ta\u015f\u0131yan <i>E. gallinarum<\/i> (27-29) ve VanA diren\u00e7li <i>E. faecalis<\/i> (30) izolatlar\u0131n\u0131n bildirildi\u011fi az say\u0131da \u00e7al\u0131\u015fma bulunmaktad\u0131r. \u00c7al\u0131\u015fmam\u0131zda da benzer \u015fekilde, \u00f6zel bir diren\u00e7 profili kapsam\u0131nda say\u0131lan VanC-1 ve van<i>A<\/i> diren\u00e7 birlikteli\u011fine sahip bir <i>E. gallinarum<\/i> izolat\u0131 ve ayr\u0131ca <i>vanB<\/i> direnci yerine <i>vanA<\/i> direnci ta\u015f\u0131yan bir <i>E. faecalis<\/i> izolat\u0131 saptand\u0131. Bu bulgu son derece \u00f6nemlidir; \u00e7\u00fcnk\u00fc normalde VanC-1 fenotipi ile d\u00fc\u015f\u00fck d\u00fczeyde intrensek diren\u00e7 g\u00f6stermesi beklenen <i>E. gallinarum<\/i> t\u00fcr\u00fcnde <i>vanA<\/i> tipi diren\u00e7 geni bulunmas\u0131 ve orta d\u00fczey diren\u00e7ten sorumlu <i>vanB <\/i>tipi diren\u00e7 geni yerine <i>vanA<\/i> direnci ta\u015f\u0131yan <i>E. faecalis<\/i> izolat\u0131 bulunmas\u0131 <i>vanA <\/i>diren\u00e7 geninin \u00fclkemizde de enterokok t\u00fcrleri aras\u0131nda aktar\u0131labildi\u011finin kan\u0131t\u0131 niteli\u011findedir. Bu t\u00fcr izolatlar\u0131n ayr\u0131nt\u0131l\u0131 olarak incelenmesi ve bu t\u00fcrlerin yay\u0131l\u0131m\u0131n\u0131 \u00f6nlemek amac\u0131yla hastanelerde d\u00fczenli tarama programlar\u0131n\u0131n s\u00fcrd\u00fcr\u00fclmesi gereklidir.<\/p>\n<p>Sonu\u00e7 olarak; bu \u00e7al\u0131\u015fma, hastane infeksiyon etkeni klinik <i>Enterococcus<\/i> izolatlar\u0131 aras\u0131nda en s\u0131k g\u00f6r\u00fclen <i>E. faecium, E. faecalis<\/i> ve <i>E. gallinarum<\/i> t\u00fcrlerinin ve bunlar\u0131n ta\u015f\u0131d\u0131\u011f\u0131 <i>vanA, vanB<\/i> ve <i>vanC-1<\/i> diren\u00e7 genlerinin, optimize edilmi\u015f bir mPCR protokol\u00fc ile k\u0131sa s\u00fcrede ve y\u00fcksek duyarl\u0131l\u0131kla saptanabilece\u011fini g\u00f6stermi\u015ftir.
