Sitomegalovirus (CMV) infeksiyonu, imm\u00fcn sistemi normal olan bireylerde genellikle asemptomatiktir. Viremi ile karakterize primer infeksiyondan sonra \u00e7o\u011fu zaman latent olarak kalmakta, ileri evre insan imm\u00fcn yetmezlik virusu (HIV) infeksiyonu gibi imm\u00fcn sistemin bask\u0131land\u0131\u011f\u0131 durumlarda, yeniden reaktive olabilmektedir (3).<\/p>\n<p class=\"p3\">\u0130nsan imm\u00fcn yetmezlik virusu, 1980\u2019lerin ba\u015flar\u0131nda tan\u0131mland\u0131\u011f\u0131ndan beri yakla\u015f\u0131k 40 milyondan fazla \u00f6l\u00fcme neden olmu\u015ftur (4,5). \u00dclkemizde 1985\u2019ten 31 Aral\u0131k 2021 tarihine kadar do\u011frulama testi pozitif tespit edilerek bildirimi yap\u0131lan 30 293 HIV ile ya\u015fayan ki\u015fi ve 2083 HIV\/AIDS hastas\u0131 bildirilmi\u015ftir (6).<span class=\"Apple-converted-space\">\u00a0 <\/span>HIV\/AIDS tan\u0131s\u0131 alan hasta say\u0131s\u0131 bir\u00e7ok \u00fclkede azalma e\u011filiminde iken \u00fclkemizde art\u0131\u015f g\u00f6stermektedir (7,8). 1996 y\u0131l\u0131ndan itibaren, virusu bask\u0131lamak i\u00e7in antiretroviral tedavi (ART)\u2019nin kullan\u0131lmas\u0131, hastal\u0131\u011f\u0131n ilerlemesini yava\u015flatarak CMV hastal\u0131\u011f\u0131n\u0131n insidans\u0131n\u0131n azalmas\u0131na katk\u0131da bulunmu\u015ftur (9-11). Ancak ART kullan\u0131m\u0131na ra\u011fmen, CMV infeksiyonu, hedef organ hastal\u0131\u011f\u0131 ve HIV\/AIDS ile ili\u015fkili mortalite i\u00e7in \u00f6nemli bir risk fakt\u00f6r\u00fc olmaya devam etmektedir. HIV\/AIDS hastalar\u0131nda, CD4<sup>+<\/sup> T lenfosit say\u0131s\u0131 50\/\u00b5l\u2019nin alt\u0131na d\u00fc\u015ft\u00fc\u011f\u00fcnde CMV\u2019ye ba\u011fl\u0131 g\u00f6z, gastrointestinal sistem, sinir sistemi, karaci\u011fer, b\u00f6brekler, adrenaller ve akci\u011ferlerde tutulum ortaya \u00e7\u0131kabilir (12-14).<\/p>\n<p class=\"p3\">Etkenin tekrarlayabilmesi ve latent kalabilmesi nedeniyle CMV hastal\u0131\u011f\u0131n\u0131n tan\u0131s\u0131 g\u00fc\u00e7t\u00fcr. Virusun varl\u0131\u011f\u0131n\u0131 ara\u015ft\u0131ran testlerin pozitif olmas\u0131 her zaman aktif hastal\u0131\u011f\u0131 g\u00f6stermez. Hastal\u0131\u011f\u0131n tan\u0131s\u0131 i\u00e7in CMV hastal\u0131\u011f\u0131 ile uyumlu semptom ve bulgular\u0131n varl\u0131\u011f\u0131 ile beraber doku biyopsisinde CMV inkl\u00fczyon cisimciklerinin veya antijenin g\u00f6sterilmesi gerekmektedir (15). Doku biyopsisi m\u00fcmk\u00fcn olmad\u0131\u011f\u0131nda, daha az spesifik olmakla birlikte, ger\u00e7ek zamanl\u0131 kantitatif polimeraz zincir reaksiyonu (kPCR) CMV-DNA testi tan\u0131da kullan\u0131lmaktad\u0131r. Hastal\u0131\u011f\u0131 viral reaktivasyondan ay\u0131rt etmek i\u00e7in net bir e\u015fik plazma CMV viral y\u00fck de\u011feri yoktur.<\/p>\n<p class=\"p3\">Bu \u00e7al\u0131\u015fmada, HIV\/AIDS hastalar\u0131nda CMV ile ili\u015fkili s\u0131k g\u00f6r\u00fclen klinik sendromlar\u0131 tan\u0131mlamak, olas\u0131 sonu\u00e7lar\u0131 de\u011ferlendirmek, CMV infeksiyonu ve hastal\u0131\u011f\u0131n\u0131n ayr\u0131m\u0131n\u0131 yapmak i\u00e7in CMV-DNA kPCR e\u015fik de\u011ferinin belirlenmesi hedeflendi.<\/p>\n<h2 class=\"p4\">Y\u00d6NTEMLER<\/h2>\n<p class=\"p2\">Ocak 2016-Ocak 2022 tarihleri aras\u0131nda servisimize yat\u0131r\u0131larak izlenen, 18 ya\u015f \u00fcst\u00fc 115 HIV\/AIDS olgusu geriye d\u00f6n\u00fck olarak incelendi ve plazmada CMV-DNA kPCR de\u011feri herhangi bir titrede pozitif olan 36 olgu \u00e7al\u0131\u015fmaya dahil edildi. S\u00f6z konusu 36 olgunun yat\u0131r\u0131larak takip edildi\u011fi g\u00f6r\u00fcld\u00fc.<\/p>\n<p class=\"p3\">\u00c7al\u0131\u015fma verilerine, HIV\/AIDS hastalar\u0131 i\u00e7in kullan\u0131lan poliklinik izlem formlar\u0131ndan ve hastane veri taban\u0131ndan ula\u015f\u0131larak demografik, laboratuvar ve klinik veriler topland\u0131. Sitomegalovirus hastal\u0131\u011f\u0131 geli\u015fimi i\u00e7in risk fakt\u00f6rlerini belirlemek amac\u0131yla; HIV-RNA PCR de\u011ferleri, CD4<span class=\"s1\"><sup>+<\/sup><\/span> T lenfosit say\u0131lar\u0131, CMV-DNA kPCR de\u011ferleri ve CMV IgM ve IgG antikor d\u00fczeyleri kaydedildi. Doku biyopsilerinin patolojik inceleme sonu\u00e7lar\u0131 hastane veri taban\u0131ndan elde edildi; \u00e7al\u0131\u015fma i\u00e7in biyopsi \u00f6rnekleri tekrar de\u011ferlendirilmedi. Histopatolojik incelemeye ek olarak imm\u00fcnohistokimyasal boyaman\u0131n t\u00fcm biyopsi \u00f6rneklerine yap\u0131ld\u0131\u011f\u0131 tespit edildi.<\/p>\n<p class=\"p3\"><span class=\"s2\">Sitomegalovirus hastal\u0131\u011f\u0131 tan\u0131s\u0131, di\u011fer etiyolojiler ekarte edildikten sonra, tutulan organ ile ilgili klinik belirtiler varl\u0131\u011f\u0131nda herhangi bir v\u00fccut s\u0131v\u0131s\u0131nda viral n\u00fckleik asidin tespiti ile konulurken CMV infeksiyonu tan\u0131s\u0131 CMV ile ilgili semptom veya bulgular olmaks\u0131z\u0131n, plazmada CMV-DNA kPCR pozitifli\u011fi ile konuldu (16). Sitomegalovirus retiniti tan\u0131s\u0131n\u0131n g\u00f6z hastal\u0131klar\u0131 kons\u00fcltasyonu yap\u0131larak oftalmolojik muayene ile konuldu\u011fu g\u00f6r\u00fcld\u00fc. <\/span><\/p>\n<p class=\"p3\">Sitomegalovirus pn\u00f6monisi tan\u0131s\u0131 ise; hasta klini\u011fi, plazmada ve bronkoalveoler lavaj (BAL) s\u0131v\u0131s\u0131nda CMV DNA PCR pozitifli\u011fi, radyolojik incelemede akci\u011ferlerde CMV uyumlu tutulum saptanmas\u0131, g\u00f6\u011f\u00fcs hastal\u0131klar\u0131 ve radyoloji kons\u00fcltasyonlar\u0131, antiviral tedaviye yan\u0131t al\u0131nmas\u0131 ile konulmu\u015ftu. Tan\u0131y\u0131 do\u011frulamak i\u00e7in G\u00f6\u011f\u00fcs Hastal\u0131klar\u0131 Klini\u011fi taraf\u0131ndan hastalara bronkoskopi ve BAL\u2019dan aside diren\u00e7li boyama (ARB) yap\u0131lm\u0131\u015f, t\u00fcberk\u00fcloz ve viral infeksiyonlar i\u00e7in PCR \u00f6rnekleri al\u0131nm\u0131\u015f ve galaktomannan d\u00fczeyi g\u00f6nderilmi\u015fti; invazif bir i\u015flem olmas\u0131 nedeni ile patolojik inceleme i\u00e7in akci\u011fer biyopsisi yap\u0131lmam\u0131\u015ft\u0131.<\/p>\n<p class=\"p3\"><span class=\"s3\">Plazmada anti-CMV IgM ve anti-CMV IgG tespiti ELISA y\u00f6ntemi ile \u00e7al\u0131\u015f\u0131lm\u0131\u015f olup plazmada CMV viral y\u00fck d\u00fczeyinin tespiti Artus<\/span><span class=\"s4\"><sup>\u00ae <\/sup><\/span><span class=\"s3\">CMV QS-RGQ Kiti (QIAGEN, Almanya) kullan\u0131larak ger\u00e7ek zamanl\u0131 PCR y\u00f6ntemiyle \u00fcreticinin talimatlar\u0131na uygun olarak ger\u00e7ekle\u015ftirilmi\u015fti. <\/span><\/p>\n<p class=\"p3\">\u00c7al\u0131\u015fma i\u00e7in Bak\u0131rk\u00f6y Dr. Sadi Konuk E\u011fitim ve Ara\u015ft\u0131rma Hastanesi, Klinik Ara\u015ft\u0131rmalar Etik Kurulu\u2019ndan 26 Haziran 2022 tarih ve 2022-12-22 karar numaras\u0131yla onay al\u0131nd\u0131.<\/p>\n<h3 class=\"p5\">\u0130statistiksel Analiz<\/h3>\n<p class=\"p2\">Verilerin analizi, SPSS (Statistical Package for the Social Sciences) versiyon 23.0 program\u0131 (IBM Corp., Armonk, NY, ABD) ile yap\u0131ld\u0131. \u00c7al\u0131\u015fmada yer alan kesikli de\u011fi\u015fkenler \u03c7<sup>2<\/sup> ve Fisher kesin testiyle, s\u00fcrekli de\u011fi\u015fkenler ise Kolmogorov-Smirnov testiyle test edilerek Mann-Whitney U testiyle incelendi; <i>p<\/i>&lt;0.05 de\u011feri anlaml\u0131 d\u00fczey olarak kabul edildi. On sekiz hastada CMV hastal\u0131\u011f\u0131 ve yine 18 hastada CMV infeksiyonu belirlendi. Hastalar\u0131n 31 (% 86.1)\u2019i erkek olup ya\u015flar\u0131 20-66 (ortalama ya\u015f=46\u00b113) y\u0131l aras\u0131ndayd\u0131. Hem CMV hastal\u0131\u011f\u0131nda hem infeksiyonunda erkek cinsiyet bask\u0131nd\u0131 ve ya\u015f ortalamalar\u0131 benzer bulundu. Her iki grupta da CMV IgG, t\u00fcm hastalarda pozitif olarak belirlendi. Sitomegalovirus hastal\u0131\u011f\u0131 olan grupta CMV viral y\u00fck\u00fc, CMV infeksiyonu olan gruptan daha y\u00fcksek saptand\u0131. Hastal\u0131\u011fa sahip olan grupta en s\u0131k olarak s\u0131ras\u0131yla gastrointestinal, solunum, g\u00f6z ve cilt tutulumu saptand\u0131. Toplam sekiz gastrointestinal tutulum olan hastan\u0131n 6\u2019s\u0131nda (%75) kolon biyopsilerinde CMV PCR pozitifli\u011fi saptand\u0131. Gastrointestinal tutulum saptanan olgular\u0131n hepsinde ve cilt tutulumu olan bir olguda patolojik tan\u0131 mevcuttu. Antiretroviral tedavide en s\u0131k tenofovir disoproksil fumarat (TDF), emtrisitabin (FTC) ve dolutegravir (DTG) kombinasyonu kullan\u0131lm\u0131\u015ft\u0131 (Tablo 1). Hastalar\u0131n hi\u00e7biri sekonder profilaksi almam\u0131\u015ft\u0131. Sitomegalovirus hastal\u0131\u011f\u0131n\u0131n tedavisinde, gansiklovir, valgansiklovir veya gansiklovirin ard\u0131ndan valgansiklovir kullan\u0131lm\u0131\u015ft\u0131; antiviral tedavi s\u00fcresi ortalama 56.11\u00b139.765 (14-113) g\u00fcn olarak bulundu. Klinik ve laboratuvar parametrelerinin kar\u015f\u0131la\u015ft\u0131rmas\u0131 Tablo 2\u2019de verildi. \u00dc\u00e7 olguda mortalite geli\u015fmi\u015f olup hepsinde CMV hastal\u0131\u011f\u0131na ba\u011fl\u0131 olarak tutulan organ akci\u011ferdi. CMV hastal\u0131\u011f\u0131 olan olgular\u0131n klinik ve laboratuvar de\u011ferlendirmeleri Tablo 3\u2019te verildi.<\/p>\n<h2 class=\"p4\">\u0130RDELEME<\/h2>\n<p class=\"p2\"><span class=\"s2\">Sitomegalovirus, ba\u011f\u0131\u015f\u0131kl\u0131\u011f\u0131 bask\u0131lanm\u0131\u015f bireylerde dissemine ya da lokal hedef organ hastal\u0131\u011f\u0131 yapar; ge\u00e7irilmi\u015f f\u0131rsat\u00e7\u0131 infeksiyonlar, CMV viremisinin y\u00fcksekli\u011fi, y\u00fcksek plazma HIV RNA d\u00fczeyi, CD4<\/span><span class=\"s5\"><sup>+<\/sup><\/span><span class=\"s2\"> T lenfosit h\u00fccre say\u0131s\u0131n\u0131n &lt;50 h\u00fccre\/mm<sup>3<\/sup> olmas\u0131 ve ART al\u0131nmamas\u0131 risk fakt\u00f6rleridir (17,18). HIV\/AIDS olgular\u0131nda, CMV retiniti saptanan grupta CD4<\/span><span class=\"s5\"><sup>+<\/sup><\/span><span class=\"s2\"> T lenfosit say\u0131s\u0131n\u0131n, CMV retiniti saptanmayan gruba g\u00f6re daha d\u00fc\u015f\u00fck bulundu\u011fu \u00e7al\u0131\u015fmalar mevcuttur (19,20).<\/span><\/p>\n<p class=\"p3\">HIV\/AIDS hastalar\u0131nda CMV hastal\u0131\u011f\u0131n\u0131n erken te\u015fhisi \u00f6nemlidir. Sitomeg<span class=\"s2\">alovirus replikasyonunu saptayan tan\u0131 testleri yayg\u0131n olarak kullan\u0131lmakla birlikte sonu\u00e7lar\u0131n de\u011ferlendirilmesinde zorluklar ya\u015fanmaktad\u0131r (21). T\u00fcrkiye\u2019de yeti\u015fkinlerde CMV seroprevalans\u0131 %85-95 aras\u0131nda bildirilmektedir (22). Bu durum \u00fclkemizdeki HIV ile infekte ki\u015filerin de virusun reaktivasyonu i\u00e7in y\u00fcksek risk alt\u0131nda oldu\u011fu anlam\u0131na gelmektedir. Anti-CMV-IgM antikoru s\u0131kl\u0131kla aktif viral infeksiyonun bir belirteci olarak kullan\u0131lmaktad\u0131r. Zhao ve arkada\u015flar\u0131n\u0131n (23) ve Udeze ve arkada\u015flar\u0131n\u0131n (24) \u00e7al\u0131\u015fmalar\u0131nda anti-CMV-IgM pozitiflik oran\u0131 s\u0131ras\u0131 ile %11.7 ve %10.4 bulunmu\u015ftur. \u00c7al\u0131\u015fmam\u0131zda, anti-CMV-IgM pozitifli\u011fi oran\u0131n\u0131n %5.5 olarak daha d\u00fc\u015f\u00fck bulunmas\u0131n\u0131n nedeni \u00e7al\u0131\u015fmaya al\u0131nan HIV\/AIDS hastalar\u0131nda virusun reaktivasyon sonucu hastal\u0131k olu\u015fturmas\u0131d\u0131r.<\/span><\/p>\n<p class=\"p3\">Sitomegalovirus infeksiyonunun en s\u0131k kar\u015f\u0131la\u015f\u0131lan hedef organ tutulumlar\u0131 retinit, ard\u0131ndan \u00f6zofajit ve kolit olarak bildirilmektedir (25). Caplan ve arkada\u015flar\u0131n\u0131n (26) \u00e7al\u0131\u015fmas\u0131nda gastrointestinal tutulum oran\u0131 %68 olarak tespit edilirken bu oran Perello ve arkada\u015flar\u0131n\u0131n (27) \u00e7al\u0131\u015fmas\u0131nda %14\u2019t\u00fcr. \u00c7al\u0131\u015fmam\u0131zda CMV ile ili\u015fkili hastal\u0131\u011f\u0131 olan grupta gastrointestinal tutulum oran\u0131 %44.4 idi. Hedef organ tutulumunun kesin tan\u0131s\u0131 biyopsi ve dokuda CMV varl\u0131\u011f\u0131n\u0131n g\u00f6sterilmesidir ancak biyopsi i\u015flemi her zaman kolay yap\u0131lamamaktad\u0131r. Merkezimizde hastalar multidisipliner bir yakla\u015f\u0131mla de\u011ferlendirilerek invazif i\u015flemlere ve biyopsiye eri\u015fim kolayl\u0131kla sa\u011flanabildi\u011finden CMV hastal\u0131\u011f\u0131 tan\u0131s\u0131n\u0131n konulabilmesi de m\u00fcmk\u00fcnd\u00fcr.<\/p>\n<p class=\"p3\">HIV\/AIDS hastalar\u0131nda CMV retiniti bug\u00fcn hala bir sorundur ve erken d\u00f6nemde ART kullan\u0131m\u0131 ile \u00f6nlenebilir. Virus, hematojen yolla retinaya yay\u0131ld\u0131\u011f\u0131 i\u00e7in plazma CMV y\u00fck\u00fc y\u00fcksek olan hastalarda retinit geli\u015fme riski y\u00fcksektir. <span class=\"s6\">Hastalara ART ve CMV \u2018ye y\u00f6nelik antiviral tedavi ba\u015flanmazsa retinit lezyonlar\u0131 giderek geni\u015fler ve geri d\u00f6n\u00fc\u015f\u00fc olmayan g\u00f6rme kayb\u0131na yol a\u00e7abilir (28,29). <\/span>\u00c7al\u0131\u015fmam\u0131zda HIV\/AIDS hastalar\u0131nda \u00f6nceki \u00e7al\u0131\u015fmalara benzer \u015fekilde CMV retinit oran\u0131 %11.1 olarak belirlendi (30,31).<\/p>\n<p class=\"p3\"><span class=\"s7\">HIV\/AIDS hastalar\u0131nda sa\u011fkal\u0131m\u0131 art\u0131rmak ve antiviral tedavi karar\u0131 i\u00e7in CMV viral y\u00fck de\u011ferinin saptanmas\u0131 ve hedef organ tutulumunun ara\u015ft\u0131r\u0131lmas\u0131 gereklidir. McCoy ve arkada\u015flar\u0131 (32) \u00f6nceden biyopsi sonu\u00e7lar\u0131 histolojik olarak negatif olan kolitli hastalarda CMV PCR testinin artm\u0131\u015f duyarl\u0131l\u0131\u011f\u0131n\u0131 g\u00f6stermi\u015ftir. Bu nedenle, biyopsi sonu\u00e7lar\u0131 CMV i\u00e7in negatifse ve g\u00fc\u00e7l\u00fc bir klinik CMV infeksiyonu \u015f\u00fcphesi varsa PCR y\u00f6nteminin kullan\u0131m\u0131 \u00f6nerilmi\u015ftir. Sitomegalovirus pn\u00f6monisinin \u00e7al\u0131\u015fmam\u0131zda literat\u00fcr\u00fcn aksine y\u00fcksek oranda bulunmas\u0131, CMV pn\u00f6monisi tan\u0131s\u0131n\u0131n doku biyopsisi ile konulmay\u0131p, klinik ve g\u00f6r\u00fcnt\u00fcleme bulgular\u0131 uyumlu hastalarda di\u011fer etkenlerin d\u0131\u015flanm\u0131\u015f olmas\u0131 yoluyla konulmas\u0131 da di\u011fer \u00f6nemli s\u0131n\u0131rl\u0131l\u0131\u011f\u0131d\u0131r.<\/p>\n<p class=\"p3\">\u00c7al\u0131\u015fmam\u0131zda her iki grupta da plazmada CMV-DNA kPCR e\u015fik de\u011feri 3154 kopya\/ml bulundu. Sitomegalovirus tan\u0131s\u0131 i\u00e7in alt\u0131n standart y\u00f6ntem biyopsi \u00f6rneklerinde CMV ile ilgili histopatolojik bulgular\u0131n g\u00f6sterilmesi ve imm\u00fcnohistokimyasal inceleme ile tan\u0131n\u0131n do\u011frulanmas\u0131d\u0131r. Histopatoloji sonu\u00e7lanma s\u00fcresinin uzun olmas\u0131, hastan\u0131n giri\u015fimsel i\u015flemi reddetmesi veya altta yatan hastal\u0131klardan dolay\u0131 yap\u0131lamamas\u0131 gibi nedenlerle noninvazif, erken sonu\u00e7 veren molek\u00fcler testler tan\u0131da kullan\u0131lmaktad\u0131r.
