Avrupa\u2019da 1990\u2019l\u0131 y\u0131llar\u0131n ikinci yar\u0131s\u0131ndan itibaren yay\u0131lmaya ba\u015flayan KRGN\u00c7\u2019ler, 2018 y\u0131l\u0131na gelindi\u011finde 37 Avrupa \u00fclkesinin tamam\u0131nda tan\u0131mlanm\u0131\u015ft\u0131r (1-3). \u00dclkemizde doksanl\u0131<span class=\"Apple-converted-space\">\u00a0 <\/span>y\u0131llarda tan\u0131mlanmaya ba\u015flayan KRGN\u00c7 infeksiyonlar\u0131, y\u0131llar i\u00e7inde \u00e7ok h\u0131zla yay\u0131larak bir \u00e7ok hastanede endemik hale gelmi\u015ftir.<span class=\"Apple-converted-space\">\u00a0 <\/span>Karbapenem direnci, 2021 y\u0131l\u0131 ulusal sa\u011fl\u0131k bak\u0131m\u0131yla ili\u015fkili infeksiyon s\u00fcrveyans verilerine g\u00f6re, <i>Acinetobacter<\/i> t\u00fcrlerinde %97.5, <i>Pseudomonas aeruginosa<\/i>\u2019da %80 ve <i>Klebsiella pneumoniae<\/i>\u2019da %75\u2019e ula\u015fm\u0131\u015ft\u0131r (4)<span class=\"s1\">.<\/span><\/p>\n<p class=\"p2\">KRGN\u00c7 infeksiyonlar\u0131nda karbapenem direncine yol a\u00e7an en \u00f6nemli mekanizma karbapenemaz \u00fcretimi olmakla birlikte, porin kayb\u0131, efluks pompalar\u0131 gibi di\u011fer mekanizmalar da dirence yol a\u00e7abilir. Bu infeksiyonlar\u0131n tedavisini planlarken, bakterideki karbapenem direncinin mekanizmas\u0131n\u0131n, karbapenemaz varl\u0131\u011f\u0131n\u0131n ve varsa karbapenemaz\u0131n tipinin tan\u0131mlanmas\u0131 olduk\u00e7a \u00f6nemli olup karbapenemaz varl\u0131\u011f\u0131n\u0131n g\u00f6sterilmesi infeksiyonun kontrol\u00fc i\u00e7in de son derece \u00f6nemlidir (2,5).<b><i> <\/i><\/b>S\u00f6z konusu infeksiyonlardaki karbapenemaz varl\u0131\u011f\u0131n\u0131n ara\u015ft\u0131r\u0131lmas\u0131 i\u00e7in fenotipik ve genotipik testlerden yararlan\u0131labilir. Genotipik testler olduk\u00e7a pahal\u0131 oldu\u011fu ve yap\u0131lmalar\u0131 teknik donan\u0131m ve tecr\u00fcbeli eleman gerektirdi\u011fi i\u00e7in \u00fclkemizdeki mikrobiyoloji laboratuvarlar\u0131n\u0131n b\u00fcy\u00fck \u00e7o\u011funlu\u011funda rutin olarak \u00e7al\u0131\u015f\u0131lamamaktad\u0131r. Bu nedenle, gerek karbapenemaz varl\u0131\u011f\u0131n\u0131n belirlenmesi, gerekse var olan karbapenemaz\u0131n t\u00fcr\u00fcn\u00fcn saptanmas\u0131 i\u00e7in<span class=\"Apple-converted-space\">\u00a0 <\/span>ucuz, h\u0131zl\u0131, kolayca yap\u0131labilen, g\u00fcvenilir test y\u00f6ntemlerine<span class=\"Apple-converted-space\">\u00a0 <\/span>gereksinim vard\u0131r. Bu \u00e7al\u0131\u015fmada, KRGN\u00c7\u2019lerdeki karbapenemaz varl\u0131\u011f\u0131 ve \u00fclkemizde karbapenem direncinin en \u00f6nemli nedenlerinden olan <i>bla<\/i><sub>OXA-48<\/sub> ve <i>bla<\/i><sub>NDM<\/sub> genlerini veya her iki geni birden ta\u015f\u0131yan su\u015flar\u0131 ay\u0131rt etmede kullan\u0131lan molek\u00fcler olmayan, ucuz ve kolay yap\u0131labilen y\u00f6ntemlerin etkinli\u011fi de\u011ferlendirilerek bir algoritma olu\u015fturulmaya \u00e7al\u0131\u015f\u0131ld\u0131.<\/p>\n<h2 class=\"p1\">Y\u00d6NTEMLER<\/h2>\n<p class=\"p2\"><span class=\"s2\">\u00c7al\u0131\u015fmaya, klinik \u00f6rneklerde \u00fcreyen ve karbapeneme diren\u00e7li oldu\u011fu belirlenmi\u015f 16 ard\u0131\u015f\u0131k Gram-negatif \u00e7omak su\u015fu (<i>K. pneumoniae,<\/i> n=9; <i>P. aeruginosa<\/i>, n=3; <i>Escherichia coli<\/i>, n=1; <i>Achromobacter<\/i> spp., n=1; <i>Acinetobacter baumannii<\/i>, n=1; <i>Stenotrophomonas maltophilia<\/i>, n=1) dahil edildi. Bakterilerin tan\u0131mlanmas\u0131nda konvansiyonel y\u00f6ntemler ve BD Phoenix<sup>TM <\/sup>(Becton Dickinson, ABD) tam otomatize sistemi kullan\u0131ld\u0131.<span class=\"Apple-converted-space\">\u00a0 <\/span>Duyarl\u0131l\u0131k testleri Avrupa Antimikrobiyal Duyarl\u0131l\u0131k Testi Komitesi (European Committee on Antimicrobial Susceptibility Testing \u2013 EUCAST) taraf\u0131ndan belirlenen zon \u00e7aplar\u0131na g\u00f6re disk dif\u00fczyon y\u00f6ntemiyle de\u011ferlendirildi ve 10 \u03bcg meropenem (Oxoid Ltd, \u0130ngiltere)<\/span> <span class=\"s2\">diski \u00e7evresindeki inhibisyon zonu &lt;16 mm olanlar karbapeneme diren\u00e7li kabul edildi (5). Karbapenemaz varl\u0131\u011f\u0131n\u0131 saptamak i\u00e7in fenotipik test olarak basitle\u015ftirilmi\u015f karbapenem inaktivasyon metodu (\u201csimplified carbapenem inactivation method\u201d \u2013 sCIM) ve modifiye Hodge testi (MHT); <i>bla<\/i><sub>OXA-48<\/sub>, <i>bla<\/i><sub>NDM<\/sub> ve <i>bla<\/i><sub>KPC<\/sub> genlerini tan\u0131mlamak i\u00e7inse molek\u00fcler y\u00f6ntem olarak ger\u00e7ek zamanl\u0131 polimerize zincir reaksiyon (RT-PCR) testi kullan\u0131ld\u0131. Genotipik testlerin yap\u0131labilmesi i\u00e7in ara\u015ft\u0131r\u0131lan su\u015flardan birer \u00f6ze dolusu mikroorganizma, 5 ml triptik soya buyyonuna (TSB) ekilip -20\u00b0C\u2019de sakland\u0131. \u00dczerine bakterilerin inok\u00fcle edildi\u011fi diskler en az 2 saat (2-4 saat) 35\u00b0C\u2019de ink\u00fcbe edildi. Mueller-Hinton agara karbapenemlere duyarl\u0131 oldu\u011fu bilinen kontrol <i>E. coli<\/i> su\u015fundan (ATCC 29522) s\u00fcspansiyon (0.5 MacFarland) haz\u0131rlan\u0131p ek\u00fcvyonla yay\u0131ld\u0131 ve agar\u0131n kapa\u011f\u0131 kapat\u0131l\u0131p oda havas\u0131nda 10 dakika kurumaya b\u0131rak\u0131ld\u0131. \u0130ki saatlik ink\u00fcbasyon sonras\u0131nda steril besiyerinde bekleyen, \u00fczerine karbapenemlere diren\u00e7li bakteri inok\u00fcle edilmi\u015f olan diskler duyarl\u0131 <i>E. coli <\/i>su\u015fu (ATCC 29522)\u2019nun oldu\u011fu Mueller-Hinton agar\u0131na yerle\u015ftirildi. Besiyerinin merkezine kontrol amac\u0131yla yeni bir meropenem diski yerle\u015ftirildi. 35\u00b0C\u2019de 24 saat ink\u00fcbasyon sonras\u0131nda sCIM sonu\u00e7lar\u0131 de\u011ferlendirildi. Kontrol meropenem diski etraf\u0131nda duyarl\u0131 oldu\u011funu bildi\u011fimiz <i>E. coli <\/i>kontrol su\u015fu (ATCC 29522) taraf\u0131ndan bir inhibisyon zonu olu\u015ftu. On alt\u0131 su\u015fun 12\u2019sinde karbapenemaz geni saptand\u0131; 11\u2019inde sCIM, 10\u2019unda MHT pozitif bulundu (duyarl\u0131l\u0131k s\u0131ras\u0131yla %91.9 ve %83.3 idi) (Tablo 3). Karbapenemaz geni saptanmayan d\u00f6rt su\u015fun ise birinde sCIM pozitif bulunurken hi\u00e7birinde MHT pozitif de\u011fildi (Tablo 2).<\/p>\n<p class=\"p2\">On <i>Enterobacterales<\/i> \u00fcyesinin hepsinde en az bir karbapenemaz geni belirlendi; yedisinde <span class=\"s4\"><i>bla<\/i><\/span><sub>OXA-48<\/sub>, birinde <span class=\"s4\"><i>bla<\/i><\/span><sub>NDM<\/sub> ve ikisinde de <span class=\"s4\"><i>bla<\/i><\/span><sub>NDM<\/sub> ve <span class=\"s4\"><i>bla<\/i><\/span><sub>OXA-48 <\/sub>beraber bulunmaktayd\u0131. Genotipik olarak karbapenemaz geni saptanan 10 <i>Enterobacterales<\/i> \u00fcyesi su\u015fun hepsinde sCIM pozitif saptand\u0131. <i>Achromobacter<\/i> spp. su\u015funda PCR ile <span class=\"s4\"><i>bla<\/i><\/span><sub>OXA-48 <\/sub>saptand\u0131 ve sCIM pozitif bulundu. <i>Pseudomonas<\/i> spp. su\u015flar\u0131n\u0131n (n=5) birinde <span class=\"s4\"><i>bla<\/i><\/span><sub>OXA-48 <\/sub>saptand\u0131 ve bu su\u015fta sCIM negatifti (yanl\u0131\u015f negatiflik). Genotipik olarak karbapenamaz geni saptanmayan <i>Pseudomonas<\/i> spp. su\u015funda sCIM negatifti.<span class=\"Apple-converted-space\">\u00a0 <\/span><i>Acinetobacter<\/i> spp. su\u015funda<span class=\"Apple-converted-space\">\u00a0 <\/span><span class=\"s4\"><i>bla<\/i><\/span><sub>OXA-48<\/sub>, <span class=\"s4\"><i>bla<\/i><\/span><sub>NDM<\/sub> veya <span class=\"s4\"><i>bla<\/i><\/span><sub>KPC<\/sub><span class=\"Apple-converted-space\">\u00a0 <\/span>genotipik y\u00f6ntemlerle saptanamam\u0131\u015f olup ve sCIM pozitifti. <i>Stenotrophomonas<\/i> spp. su\u015funda genotipik y\u00f6ntemlerle karbapenemaz geni saptanmad\u0131 ve hem sCIM hem de MHT negatifti (Tablo 2).<\/p>\n<p class=\"p2\"><i>Enterobacterales<\/i> su\u015flar\u0131n\u0131n hepsine y\u00fcksek d\u00fczey temosilin direnci bak\u0131l\u0131p, seftazidim-avibaktamla disk dif\u00fczyon testi yap\u0131ld\u0131; <span class=\"s4\"><i>bla<\/i><\/span><sub>OXA-48<\/sub> geni olan su\u015flar\u0131n hepsinde y\u00fcksek d\u00fczey temosilin direnci saptan\u0131rken (M\u0130K &gt;128 \u03bcg\/ml), tek ba\u015f\u0131na <span class=\"s4\"><i>bla<\/i><\/span><sub>NDM<\/sub> ta\u015f\u0131yan su\u015fta diren\u00e7 g\u00f6sterilmedi. Tek ba\u015f\u0131na <span class=\"s4\"><i>bla<\/i><\/span><sub>OXA-48 <\/sub>ta\u015f\u0131yan su\u015flar seftazidim-avibaktama duyarl\u0131 bulunurken, tek ba\u015f\u0131na <span class=\"s4\"><i>bla<\/i><\/span><sub>NDM<\/sub> ta\u015f\u0131yan su\u015f diren\u00e7liydi.<\/p>\n<h2 class=\"p1\">\u0130RDELEME<\/h2>\n<p class=\"p2\">\u00c7al\u0131\u015fma kapsam\u0131nda, molek\u00fcler y\u00f6ntemlerle karbapenemaz genleri tan\u0131mlanm\u0131\u015f olan <i>Enterobacterales<\/i> t\u00fcrlerinin hepsinde sCIM y\u00f6ntemiyle karbapenemaz \u00fcretimi oldu\u011fu g\u00f6sterildi. Bu veri CIM testinin karbapenemaz varl\u0131\u011f\u0131n\u0131 g\u00f6stermede rutin olarak kullan\u0131labilece\u011fini belirten Klinik ve Laboratuvar Standartlar\u0131 Enstit\u00fcs\u00fc (Clinical and Standards Institute \u2013 CLSI)\u2019n\u00fcn \u00f6nerisini desteklemektedir (11). Literat\u00fcrde de \u00e7al\u0131\u015fmam\u0131za benzer olarak karbapenemaz ta\u015f\u0131d\u0131\u011f\u0131 molek\u00fcler y\u00f6ntemlerle belirlenmi\u015f <i>Enterobacterales<\/i> su\u015flar\u0131nda sCIM\u2019in karbapenemaz varl\u0131\u011f\u0131n\u0131n g\u00f6sterilmesinde kullan\u0131\u015fl\u0131 oldu\u011fu g\u00f6sterilmi\u015ftir (6,12). Bununla birlikte sCIM\u2019in duyarl\u0131l\u0131\u011f\u0131n\u0131n modifiye CIM ile benzer ve laboratuvarda rutin kullan\u0131m i\u00e7in uygun oldu\u011fu belirtilmi\u015ftir (6).<\/p>\n<p class=\"p2\">Karbapenem direnci, karbapenemaz \u00fcretimine ek olarak porin kayb\u0131, efluks pompalar\u0131n\u0131n a\u015f\u0131r\u0131 \u00fcretimi gibi bir\u00e7ok ba\u015fka mekanizmayla da ili\u015fkili olabilir. \u00d6zellikle <i>P. aeruginosa\u2019<\/i>da karbapenem direncinden esas olarak bu mekanizmalar sorumludur. \u00c7al\u0131\u015fmam\u0131zda yer alan <i>P. aeruginos<\/i>a su\u015flar\u0131nda sCIM testi negatif bulundu. Bu sonu\u00e7lar e\u011fer bir yalanc\u0131 negatifli\u011fi g\u00f6steriyorsa nedeninin disk \u00fczerine yerle\u015ftirilen inokulumun yetersiz olmas\u0131 ya da daha uzun s\u00fcre ink\u00fcbasyon gerektirmesi olabilir (12). Bir di\u011fer a\u00e7\u0131klama ise fermenter olmayan Gram-negatif \u00e7omaklarda sCIM\u2019in duyarl\u0131l\u0131\u011f\u0131n\u0131n, kullan\u0131lan kontrol su\u015funun (<i>E.coli<\/i> ATCC 29522) farkl\u0131 konsantrasyonlar\u0131nda de\u011fi\u015fmesi olabilir (6). Ayr\u0131ca CIM\u2019in performans\u0131 kullan\u0131lan karbapenem disk i\u00e7eri\u011fine g\u00f6re de\u011fi\u015fiklik g\u00f6sterebilir (7,13). Farkl\u0131 karbapenem diskleriyle sCIM\u2019in performans\u0131n\u0131n de\u011ferlendirildi\u011fi bir \u00e7al\u0131\u015fmada, <i>P. aeruginosa<\/i> ve <i>A. baumannii <\/i>i\u00e7in imipenemle yap\u0131lan test sonu\u00e7lar\u0131n\u0131n daha iyi oldu\u011fu g\u00f6sterilmi\u015ftir (7). \u00c7al\u0131\u015fmam\u0131zda genotipik testler fermenter olmayan su\u015flar\u0131n hepsinde yap\u0131lamad\u0131\u011f\u0131ndan g\u00f6zlemimiz bir su\u015fla s\u0131n\u0131rl\u0131 kald\u0131. Seftazidim-avibaktam disk dif\u00fczyon ile inhibisyon ve y\u00fcksek d\u00fczey temosilin direncinin birlikte kullan\u0131m\u0131n\u0131n<span class=\"s4\"><i> bla<\/i><\/span><sub>OXA-48<\/sub>\u2019i \u00f6ng\u00f6rd\u00fcrebilece\u011fini destekleyen verilerimizin farkl\u0131 karbapenemaz t\u00fcrlerini (<span class=\"s4\"><i>bla<\/i><\/span><sub>OXA-48, <\/sub><i>bla<\/i><sub>KPC<\/sub>, <i>bla<\/i><sub>NDM<\/sub>) ta\u015f\u0131yan daha \u00e7ok say\u0131da su\u015fla de\u011ferlendirildi\u011fi \u00e7al\u0131\u015fmalara ihtiya\u00e7 vard\u0131r (8) (\u015eekil 1).<\/p>\n<p class=\"p2\">Karbapenemaz varl\u0131\u011f\u0131n\u0131n ara\u015ft\u0131r\u0131lmas\u0131nda kullan\u0131lan modifiye karbapenem inaktivasyon metodu son g\u00fcncel rehberlerde \u00f6nerilen fenotipik y\u00f6ntemler aras\u0131ndad\u0131r (11). \u00c7al\u0131\u015fmam\u0131zda sCIM\u2019in duyarl\u0131l\u0131\u011f\u0131 %91.6 olup <span class=\"s4\"><i>bla<\/i><\/span><sub>OXA-48<\/sub> ta\u015f\u0131yan su\u015flarda ise %100 olarak tespit edildi. \u00c7al\u0131\u015fmam\u0131z\u0131n verileri, \u00f6zellikle <i>Enterobacterales<\/i> su\u015flar\u0131nda sCIM\u2019in duyarl\u0131l\u0131\u011f\u0131n\u0131n olduk\u00e7a y\u00fcksek oldu\u011funu, sCIM\u2019in fenotipik testler aras\u0131nda \u00f6nemli bir alternatif olabilece\u011fini destekledi.<\/p>\n<p class=\"p2\">Sonu\u00e7 olarak; karbapenemaz \u00fcretiminin saptanmas\u0131, h\u0131zl\u0131ca izolasyon \u00f6nlemlerinin uygulanarak karbapenemaz \u00fcreten su\u015flar\u0131n yay\u0131l\u0131m\u0131na engel olmak i\u00e7in \u00f6nemlidir.
