Mikrobiyal k\u00fclt\u00fcr, kan dola\u015f\u0131m\u0131 infeksiyonlar\u0131n\u0131n tan\u0131s\u0131nda kabul edilen bir y\u00f6ntemdir. Bununla birlikte, geleneksel mikrobiyolojik y\u00f6ntemlerin baz\u0131 k\u0131s\u0131tl\u0131l\u0131klar\u0131 vard\u0131r; uzun ink\u00fcbasyon s\u00fcreleri, kontaminasyon nedeniyle \u00f6zg\u00fcll\u00fc\u011f\u00fcn d\u00fc\u015f\u00fck olmas\u0131, konvansiyonel kan k\u00fclt\u00fcr\u00fc ortam ve sistemlerinde baz\u0131 mikroorganizmalar\u0131n yetersiz \u00fcremesi veya hi\u00e7 \u00fcrememesi bunlar\u0131n en \u00f6nemlileridir. Potansiyel kan dola\u015f\u0131m\u0131 patojenlerinin tamam\u0131n\u0131 saptamak i\u00e7in kullan\u0131lan ve en iyi olan tek bir k\u00fclt\u00fcr ortam\u0131 veya sistemi g\u00f6sterilmemi\u015ftir. B\u00fct\u00fcn bunlar tan\u0131da gecikmelere neden olabilmekte ve klinisyenler ampirik antibiyotik tedavisine ba\u015fvurmaktad\u0131r. Kan dola\u015f\u0131m\u0131 infeksiyonlar\u0131n\u0131n h\u0131zl\u0131 tan\u0131s\u0131nda kullan\u0131lan ana serolojik parametreler; C-reaktif protein (CRP) ve prokalsitonin (PCT)\u2019dir (1).<\/p>\n<p class=\"p3\">C-reaktif protein, infeksiy\u00f6z ve infeksiy\u00f6z olmayan inflamatuar s\u00fcre\u00e7lerde spesifik olmayan bir \u015fekilde y\u00fckselen bir akut faz proteinidir. Bakteriyel infeksiyonlarda viral infeksiyonlara g\u00f6re daha fazla artar (2). Ancak sistemik bakteriyel infeksiyonlara \u00f6zg\u00fc de\u011fildir (1).<\/p>\n<p class=\"p3\"><span class=\"s1\">Prokalsitonin, kalsitoninin \u00f6nc\u00fcs\u00fc olan ve kalsiyum hemostaz\u0131 sa\u011flayan bir prohormondur; normalde v\u00fccutta \u00e7ok d\u00fc\u015f\u00fck konsantrasyonda bulunur ve sadece tiroid bezi ve akci\u011ferdeki n\u00f6roendokrin h\u00fccrelerden sal\u0131n\u0131r (3). Bununla birlikte PCT, bakteriyel infeksiyonlarda sal\u0131nan endotoksin ve mediat\u00f6rlere yan\u0131t olarak v\u00fccutta daha y\u00fcksek konsantrasyonlarda salg\u0131lan\u0131r. Ayn\u0131 zamanda infeksiyonun ciddiyeti ve yayg\u0131nl\u0131\u011f\u0131 ile de olduk\u00e7a ili\u015fkilidir (4). Ayr\u0131ca yan\u0131k, travma ve cerrahi m\u00fcdahaleler gibi durumlarda PCT\u2019nin y\u00fckseldi\u011fi bilinmektedir. Prokalsitonin d\u00fczeylerinin cerrahi m\u00fcdahale sonras\u0131 herhangi bir infeksiyon olmaks\u0131z\u0131n be\u015finci g\u00fcne kadar y\u00fckselebilece\u011fi ancak normal s\u0131n\u0131rlar\u0131 a\u015fmad\u0131\u011f\u0131 \u00e7e\u015fitli \u00e7al\u0131\u015fmalarda g\u00f6sterilmi\u015ftir (5-7).<\/span><\/p>\n<p class=\"p3\">Gram-pozitif bakteri (GPB)\u2019ler, Gram-negatif bakteri (GNB)\u2019ler veya mantarlar\u0131n farkl\u0131 Toll-benzeri resept\u00f6r (TLR) sinyal yollar\u0131n\u0131 aktive etti\u011fi ve sonu\u00e7ta PCT sal\u0131n\u0131m\u0131n\u0131 uyaran farkl\u0131 proinflamatuar sitokinlerin \u00fcretimine neden oldu\u011fu bilinmektedir (8). Bu da farkl\u0131 patojenlerin farkl\u0131 seviyelerde PCT sal\u0131n\u0131m\u0131na neden olabilece\u011fini g\u00f6stermektedir (9).<\/p>\n<p class=\"p3\">\u00c7al\u0131\u015fmam\u0131zda, kan dola\u015f\u0131m\u0131ndan izole edilen GNB, GPB ve mantarlar\u0131n CRP ve PCT de\u011ferlerini y\u00fckseltme seviyeleri kar\u015f\u0131la\u015ft\u0131r\u0131larak aralar\u0131nda anlaml\u0131 fark olup olmad\u0131\u011f\u0131n\u0131n ara\u015ft\u0131r\u0131lmas\u0131 ama\u00e7land\u0131.<\/p>\n<h2 class=\"p1\">Y\u00d6NTEMLER<\/h2>\n<p class=\"p3\">Ocak 2018-Temmuz 2021 tarihleri aras\u0131nda hastanemizin mikrobiyoloji laboratuvar\u0131na g\u00f6nderilen kan k\u00fclt\u00fcrleri retrospektif olarak tarand\u0131. Bir veya daha fazla sette saf k\u00fclt\u00fcr \u00fcremesi olan hastalar, e\u015f zamanl\u0131 ba\u015fka k\u00fclt\u00fcr \u00fcremesi olmayan hastalar, e\u015f zamanl\u0131 ba\u015fka k\u00fclt\u00fcr \u00fcremesi olan hastalardan kan k\u00fclt\u00fcr\u00fc ile ayn\u0131 patojen \u00fcreyen hastalar, kan k\u00fclt\u00fcr\u00fc al\u0131nd\u0131ktan \u00b124 saat i\u00e7erisinde CRP ve PCT bak\u0131lm\u0131\u015f olan hastalar, 18 ya\u015f\u0131ndan b\u00fcy\u00fck hastalar \u00e7al\u0131\u015fmaya dahil edildi. Polimikrobiyal \u00fcreme olan hastalar, <i>Corynebacterium <\/i>spp., koag\u00fclaz-negatif stafilokok<i>,<\/i> <i>Micrococcus <\/i>spp<i>.<\/i> gibi cilt flora bakteri \u00fcremesi olan hastalar, travma ve yan\u0131k hastalar\u0131 ve ameliyattan sonraki be\u015f g\u00fcn i\u00e7inde \u00fcreme olan hastalar ve e\u015f zamanl\u0131 ba\u015fka k\u00fclt\u00fcr \u00fcremesi olan hastalardan kan k\u00fclt\u00fcr\u00fc ile farkl\u0131 patojen \u00fcreyen hastalar \u00e7al\u0131\u015fma d\u0131\u015f\u0131 b\u0131rak\u0131ld\u0131. Ayn\u0131 hastaya ait ayn\u0131 patojen ile tekrarlayan kan k\u00fclt\u00fcr\u00fc \u00fcremelerinde hastan\u0131n ilk k\u00fclt\u00fcr \u00fcremesi \u00e7al\u0131\u015fmaya d\u00e2hil edilmi\u015f olup tekrarlayan \u00fcremeler \u00e7al\u0131\u015fma d\u0131\u015f\u0131 b\u0131rak\u0131ld\u0131.<span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p class=\"p3\">K\u00fclt\u00fcr i\u00e7in BacT\/Alert<span class=\"s2\">\u00ae<\/span> 3D (bioM\u00e9rieux, Marcy l\u2019Etoile, Fransa) tam otomatik kan k\u00fclt\u00fcr\u00fc cihaz\u0131 kullan\u0131ld\u0131 ve bakteri identifikasyonlar\u0131 VITEK<span class=\"s2\">\u00ae<\/span> 2 (bioM\u00e9rieux, Marcy l\u2019Etoile, Fransa) otomatize sistemiyle ile yap\u0131ld\u0131. C-reaktif protein de\u011ferleri Cobas<span class=\"s2\">\u00ae<\/span>\u00a06000 (Roche Diagnostics, ABD) cihaz\u0131nda imm\u00fcnoturbidometrik y\u00f6ntemle \u00e7al\u0131\u015f\u0131ld\u0131 ve 0.3-5 mg\/lt normal s\u0131n\u0131r de\u011fer aral\u0131\u011f\u0131 olarak, 5 mg\/dl\u2019den b\u00fcy\u00fck ve e\u015fit de\u011ferler ise y\u00fcksek de\u011fer olarak kabul edildi. Prokalsitonin de\u011ferleri Cobas<span class=\"s2\">\u00ae<\/span>\u00a06000 (Roche Diagnostics, ABD) cihaz\u0131nda elektrokemol\u00fcminesans sandvi\u00e7 y\u00f6ntemi ile \u00e7al\u0131\u015f\u0131ld\u0131 ve 0.02-0.5 ng\/ml normal s\u0131n\u0131r de\u011fer aral\u0131\u011f\u0131 olarak, 0.5 ng\/ml\u2019ye e\u015fit ve daha b\u00fcy\u00fck de\u011ferler ise y\u00fcksek de\u011fer olarak kabul edildi. Hastalar kan k\u00fclt\u00fcr\u00fc \u00fcremesine g\u00f6re GNB, GPB, mantar, <i>Enterobacterales<\/i> ve nonfermenter bakteriler olarak grupland\u0131r\u0131ld\u0131.<\/p>\n<p class=\"p3\">\u00c7al\u0131\u015fma i\u00e7in Amasya \u00dcniversitesi T\u0131p Fak\u00fcltesi Giri\u015fimsel Olmayan Klinik Ara\u015ft\u0131rmalar Etik Kurulu\u2019ndan 12 May\u0131s 2022 tarih ve 52 karar numaras\u0131yla onay al\u0131nm\u0131\u015ft\u0131r.<\/p>\n<h3 class=\"p5\">\u0130statistiksel Analiz<\/h3>\n<p class=\"p3\">Verilerin analizi, SPSS (\u201cStatistical Package for the Social Sciences\u201d) versiyon 20.0 program\u0131 (IBM Corp., Armonk, NY, ABD) kullan\u0131larak yap\u0131ld\u0131. Say\u0131sal verilerin normal da\u011f\u0131l\u0131ma uygunlu\u011fu Shaphiro-Wilk testi ile de\u011ferlendirildi. Normal da\u011f\u0131lan say\u0131sal verilerin kar\u015f\u0131la\u015ft\u0131r\u0131lmas\u0131nda Student t testi, normal da\u011f\u0131lmayan say\u0131sal verilerin kar\u015f\u0131la\u015ft\u0131r\u0131lmas\u0131nda Mann-Whitney U testi kullan\u0131ld\u0131. Her gruptaki s\u00fcrekli de\u011fi\u015fkenler i\u00e7in medyan de\u011fer, birinci \u00e7eyrek (Q1) ve \u00fc\u00e7\u00fcnc\u00fc \u00e7eyrek (Q3) dilim de\u011ferleri verildi. Kategorik verilerin kar\u015f\u0131la\u015ft\u0131r\u0131lmas\u0131 \u03c72 testi ile, \u00fc\u00e7 veya daha fazla grubun kar\u015f\u0131la\u015ft\u0131r\u0131lmas\u0131 ise Kruskal-Wallis testi ile yap\u0131ld\u0131. GPB grubu-mantar grubu (<i>p<\/i>=0.101), GNB grubu-mantar grubu (<i>p<\/i>=0.051) ve <i>Enterobacterales<\/i>-nonfermenter bakteri grubu (<i>p<\/i>=0.589) aras\u0131nda PCT medyan de\u011feri a\u00e7\u0131s\u0131ndan anlaml\u0131 d\u00fczeyde bir fark bulunmad\u0131.<\/p>\n<p class=\"p3\">C-reaktif protein medyan de\u011feri; GNB grubunda 167.72 (94.37-265.81) mg\/lt, GPB grubunda 145.49 (81.31-235.23) mg\/lt, mantar grubunda 169.8 (79.89-220.47) mg\/lt, <i>Enterobactareles <\/i>grubunda 163.16 (91.69-271.26) mg\/lt ve nonfermenter bakteri grubunda 172.44 (104.37-261.59) mg\/lt olarak tespit edildi (Tablo 2). Gram-pozitif bakterilerin lipoteikoik asidi TLR2 taraf\u0131ndan tan\u0131n\u0131rken, GNB\u2019lerin lipopolisakaritleri TLR4 taraf\u0131ndan tan\u0131n\u0131r. Toll-benzeri resept\u00f6rlerin aktivasyonu ile inflamatuar sitokinlerin ve akut faz reaktanlar\u0131n\u0131n sal\u0131n\u0131m\u0131n\u0131 tetikleyen n\u00fckleer fakt\u00f6r kB ve di\u011fer interferon d\u00fczenleyici fakt\u00f6rler, l\u00f6kositlerde farkl\u0131 gen ekspresyonlar\u0131n\u0131 ind\u00fckler (10). Gram-negatif bakteriler, GPB\u2019lerden daha fazla proinflamatuar sitokin sal\u0131n\u0131m\u0131na neden olur (11). Bu farkl\u0131 sitokin yan\u0131t\u0131, farkl\u0131 PCT d\u00fczeylerinin g\u00f6r\u00fclmesine yol a\u00e7ar.<span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p class=\"p3\">\u00c7al\u0131\u015fmam\u0131z\u0131n en \u00f6nemli bulgusu, GNB\u2019lerin neden oldu\u011fu kan dola\u015f\u0131m\u0131 infeksiyonlar\u0131nda GPB\u2019lere g\u00f6re PCT\u2019nin anlaml\u0131 d\u00fczeyde daha y\u00fcksek konsantrasyonlarda bulunmas\u0131 olup GNB, GPB ve mantar gruplar\u0131n\u0131n PCT medyan de\u011ferleri s\u0131ras\u0131yla 6.97 (1.59-27.96) ng\/ml, 1.35 (0.4-7.26) ng\/ml ve 2.79 (0.8-12.68) ng\/ml olarak tespit edildi; PCT medyan de\u011feri GNB grubunda GPB grubuna g\u00f6re anlaml\u0131 d\u00fczeyde daha y\u00fcksek bulundu (<i>p<\/i>&lt;0.001). Ancak GPB-mantar grubu (<i>p<\/i>=0.101) ve GNB-mantar grubu (<i>p<\/i>=0.051), PCT medyan de\u011ferleri a\u00e7\u0131s\u0131ndan istatistiksel olarak benzer bulundu. Varela-Pati\u00f1o ve ark.\u2019n\u0131n (12) 653 hastay\u0131 inceledikleri \u00e7al\u0131\u015fmalar\u0131nda, PCT medyan de\u011feri GNB grubunda 6.02 (1.39-39.40) ng\/ml, GPB grubunda ise 1.74 (0.22-15.61) ng\/ml olarak bildirilmi\u015ftir. S\u00f6z konusu \u00e7al\u0131\u015fmada, iki grup PCT de\u011ferleri kar\u015f\u0131la\u015ft\u0131r\u0131ld\u0131\u011f\u0131nda, GNB grubunun anlaml\u0131 d\u00fczeyde daha y\u00fcksek PCT de\u011ferine sahip oldu\u011fu belirtilmi\u015ftir (<i>p<\/i>&lt;0.05). \u00dclkemizde yap\u0131lan ve 410 hastan\u0131n dahil edildi\u011fi ba\u015fka bir \u00e7al\u0131\u015fmada ise PCT medyan de\u011feri GNB grubunda 7.31 (2.12-20.72) ng\/ml, GPB grubunda 0.46 (0.25-2.30) ng\/ml olarak bildirilmi\u015f ve GNB grubunun PCT de\u011ferinin anlaml\u0131 d\u00fczeyde daha y\u00fcksek oldu\u011fu belirtilmi\u015ftir (<i>p<\/i>&lt;0.001) (13). <span class=\"s3\">Brodsk\u00e1<\/span> ve ark. (14), Leli ve ark. (9), Li ve ark. (15) \u00e7al\u0131\u015fmalar\u0131nda, GNB grubunun GPB grubundan anlaml\u0131 d\u00fczeyde daha y\u00fcksek PCT de\u011ferlerine sahip oldu\u011funu bildirmi\u015ftir. Ayr\u0131ca bizim ara\u015ft\u0131rmam\u0131zdan farkl\u0131 olarak GNB grubunun mantar grubundan anlaml\u0131 d\u00fczeyde daha y\u00fcksek PCT de\u011ferlerine sahip oldu\u011funu da bildirmi\u015flerdir. Ogasawara ve ark. (16) PCT de\u011ferinin GNB grubunda GPB grubuna g\u00f6re anlaml\u0131 d\u00fczeyde daha y\u00fcksek oldu\u011funu bildirmi\u015ftir. Bulgular\u0131m\u0131zla benzer \u015fekilde, Leng ve ark. (17), GNB grubunun GPB grubundan anlaml\u0131 d\u00fczeyde daha y\u00fcksek PCT de\u011ferlerine sahip oldu\u011funu tespit etmi\u015ftir; ancak GNB ve mantar grubu PCT de\u011ferlerini istatistiksel olarak benzer bulmu\u015flard\u0131r. Lin ve ark. (18) 119 yan\u0131k hastas\u0131n\u0131 inceledikleri \u00e7al\u0131\u015fmalar\u0131nda, PCT medyan de\u011ferinin GNB grubunda [2.67 (1.58-6.08) ng\/ml] GPB grubuna [1.04 (0.35-1.60) ng\/ml; <i>p<\/i>=0.01] ve mantar grubuna [1.09 (0.82-2.30) ng\/ml; <i>p<\/i>=0.05] g\u00f6re anlaml\u0131 d\u00fczeyde daha y\u00fcksek oldu\u011funu bildirmi\u015ftir. Bu sonu\u00e7lar bize PCT\u2019nin GNB-GPB etiyolojisinin ayr\u0131m\u0131nda kullan\u0131labilece\u011fini g\u00f6stermekle birlikte PCT\u2019nin GNB-mantar etiyolojisinin ayr\u0131m\u0131nda kullan\u0131lmas\u0131 konusunda tam bir fikir birli\u011fi yoktur. Mantarlar\u0131n \u00fcremesi ve k\u00fclt\u00fcrde saptanmas\u0131 zor oldu\u011fundan say\u0131lar\u0131 di\u011fer patojenlere g\u00f6re nispeten daha az bulundu. Bu nedenle mantar grubunun di\u011fer gruplarla kar\u015f\u0131la\u015ft\u0131r\u0131lmas\u0131nda anlaml\u0131 d\u00fczeyde bir fark bulunamad\u0131. Bu sorunu \u00e7\u00f6zmek i\u00e7in mantarlar\u0131n daha kolay tespit edilebildi\u011fi PCR tabanl\u0131 testler ile ileri \u00e7al\u0131\u015fmalara ihtiya\u00e7 vard\u0131r.<\/p>\n<p class=\"p3\">\u00c7al\u0131\u015fmam\u0131zda GNB, GPB ve mantar gruplar\u0131n\u0131n CRP medyan de\u011ferleri s\u0131ras\u0131yla 167.72 (94.37-265.81) mg\/lt, 145.49 (81.31-235.23) mg\/lt ve 169.8 (79.89-220.47) mg\/lt olarak tespit edildi; GNB-GPB grubu (<i>p<\/i>=0.73) ile GNB-mantar grubunun (<i>p<\/i>=0.384) CRP medyan de\u011ferleri istatistiksel olarak benzer bulundu. Varela-Pati\u00f1o ve ark. (12) ve Li ve ark.\u2019n\u0131n (15) bulgular\u0131 \u00e7al\u0131\u015fmam\u0131zla benzer olmakla birlikte Bilgili ve ark.\u2019n\u0131n (13) yo\u011fun bak\u0131mda yatan 136 sepsis hastas\u0131n\u0131 inceledikleri \u00e7al\u0131\u015fmalar\u0131nda, GNB grubunun GPB grubuna g\u00f6re anlaml\u0131 d\u00fczeyde daha y\u00fcksek CRP de\u011ferlerine sahip oldu\u011fu bildirilmi\u015ftir. \u0130ncelenen hasta say\u0131s\u0131n\u0131n bu \u00e7al\u0131\u015fmadan daha az olmas\u0131 ve daha spesifik bir hasta grubunun incelenmi\u015f olmas\u0131 bu farkl\u0131 sonuca neden olmu\u015f olabilir. Bulgular\u0131m\u0131za benzer \u015fekilde, 636 hastay\u0131 kapsayan bir \u00e7al\u0131\u015fmada, GNB grubunun GPB grubuna g\u00f6re anlaml\u0131 d\u00fczeyde daha y\u00fcksek PCT de\u011ferlerine sahip oldu\u011fu ancak CRP de\u011ferlerinin istatistiksel olarak benzer oldu\u011fu bildirilmi\u015ftir (19). Koizumi ve ark. (20) 207 hastay\u0131 kapsayan ara\u015ft\u0131rmalar\u0131nda, GNB grubunun GPB grubuna g\u00f6re anlaml\u0131 d\u00fczeyde daha y\u00fcksek PCT ve CRP de\u011ferlerine sahip oldu\u011funu tespit etmi\u015ftir.<\/p>\n<p class=\"p3\">\u00c7al\u0131\u015fmam\u0131zda <i>Enterobacterales<\/i> ve nonfermenter grubun PCT (<i>p<\/i>=0.589) ve CRP (<i>p<\/i>=0.357) medyan de\u011ferleri istatiksel olarak kar\u015f\u0131la\u015ft\u0131r\u0131ld\u0131\u011f\u0131nda iki grup benzer bulundu. Leng ve ark. (17) <i>Enterobacterales<\/i> ve nonfermenter grubun PCT medyan de\u011ferlerini s\u0131ras\u0131yla 1.51 (0.28-10.48) ng\/ml ve 0.99 (0.24-4.14) ng\/ml olarak bildirmi\u015f ve \u00e7al\u0131\u015fmam\u0131zla benzer \u015fekilde, iki grubu PCT de\u011ferleri a\u00e7\u0131s\u0131ndan benzer bulmu\u015flard\u0131r.<\/p>\n<p class=\"p3\">ROC e\u011frisine g\u00f6re GNB ve GPB gruplar\u0131 kar\u015f\u0131la\u015ft\u0131r\u0131ld\u0131\u011f\u0131nda, PCT optimal e\u015fik de\u011feri %75 duyarl\u0131l\u0131k ve %53 \u00f6zg\u00fcll\u00fck ile 1.45 ng\/ml olarak bulundu; ROC e\u011frisi alt\u0131nda kalan alan 0.675 (%95 GA=0.623-0.726; <i>p<\/i>&lt;0.001) olarak hesapland\u0131. Ayr\u0131ca, GNB grubunun PCT medyan de\u011feri anlaml\u0131 d\u00fczeyde daha y\u00fcksek bulundu. Li ve ark. (15) GNB ve GPB gruplar\u0131n\u0131n kar\u015f\u0131la\u015ft\u0131r\u0131lmas\u0131nda PCT i\u00e7in ROC e\u011frisi alt\u0131nda kalan alan\u0131 0.786 (%95 GA=0.731\u20130.841; <i>p<\/i>&lt;0.001) olarak bildirmi\u015ftir. Lai ve ark. (21) 25 \u00e7al\u0131\u015fman\u0131n incelendi\u011fi meta-analiz \u00e7al\u0131\u015fmas\u0131nda, GNB kaynakl\u0131 kan dola\u015f\u0131m\u0131 infeksiyonlar\u0131nda ROC e\u011frisi alt\u0131nda kalan alan PCT i\u00e7in 0.85 (%95 GA=0.81-0.87) ve CRP i\u00e7in 0.78 (%95 GA=0.74-0.81) olarak tespit edilmi\u015f ve kan dola\u015f\u0131m\u0131 infeksiyonlar\u0131nda etiyolojiyi tahmin etmede PCT\u2019nin CRP\u2019den daha iyi performans g\u00f6sterdi\u011fi<span class=\"Apple-converted-space\">\u00a0 <\/span>bildirilmi\u015ftir. Koizumi ve ark.\u2019n\u0131n (20) bulgular\u0131 da \u00e7al\u0131\u015fmam\u0131zla benzerdir.<\/p>\n<p class=\"p3\">Ayr\u0131ca <i>Enterobacterales <\/i>ve nonfermenter bakteri gruplar\u0131n\u0131n kar\u015f\u0131la\u015ft\u0131r\u0131lmas\u0131nda PCT optimal e\u015fik de\u011feri %17 duyarl\u0131l\u0131k ve %93 \u00f6zg\u00fcll\u00fck ile 56.09 ng\/ml olarak bulundu; PCT de\u011feri i\u00e7in ROC e\u011frisi alt\u0131nda kalan alan 0.518 (%95 GA=0.453-0.583; <i>p<\/i>=0.587) idi. Olmu\u015f\u00e7elik ve ark. (19) ise bu iki grubun kar\u015f\u0131la\u015ft\u0131r\u0131lmas\u0131nda ROC e\u011frisi alt\u0131nda kalan alan\u0131 0.609 (%95 GA=0.542-0.676; <i>p<\/i>=0.003) olarak bildirmi\u015ftir.<\/p>\n<p class=\"p3\">\u00c7al\u0131\u015fmam\u0131zda ROC e\u011frilerine g\u00f6re t\u00fcm grup kar\u015f\u0131la\u015ft\u0131rmalar\u0131nda CRP de\u011ferleri benzer bulundu. Li ve ark. (15) da \u00e7al\u0131\u015fmam\u0131zla benzer bir \u015fekilde ROC e\u011frilerine g\u00f6re GNB ve GPB gruplar\u0131n\u0131n CRP de\u011ferlerinin benzer oldu\u011funu bildirmi\u015ftir.<\/p>\n<p class=\"p3\">Hasta verilerinin hastane bilgi sisteminden al\u0131nd\u0131\u011f\u0131 retrospektif bir \u00e7al\u0131\u015fma olmas\u0131 ara\u015ft\u0131rmam\u0131z\u0131n en \u00f6nemli k\u0131s\u0131tl\u0131l\u0131\u011f\u0131d\u0131r. Bu nedenle hastalar\u0131n klinik durumlar\u0131 hakk\u0131nda \u00e7ok az bilgi edinebildik.
