Genel pop\u00fclasyonda akut otitis eksternan\u0131n (AOE) tahmini insidans\u0131 %1\u2019dir. <i>Pseudomonas aeruginosa<\/i>\u00a0ve\u00a0<i>Staphylococcus aureus <\/i>en s\u0131k g\u00f6r\u00fclen etken patojenlerdir.\u00a0Patogenezde, genel olarak nemin varl\u0131\u011f\u0131 ve kulak yolunun kontamine su ile maruziyeti yer almaktad\u0131r.\u00a0Akut otitis eksternal\u0131 hastalar; kulak a\u011fr\u0131s\u0131, hassasiyeti ve ak\u0131nt\u0131s\u0131 ile ba\u015fvurur (2).<\/p>\n<p class=\"p3\">Otitis media (OM), orta kula\u011f\u0131 etkileyen inflamasyon ve infeksiy\u00f6z s\u00fcre\u00e7 olarak tan\u0131mlanabilir. Hastal\u0131\u011f\u0131n akut ve kronik formlar\u0131 bulunmaktad\u0131r. Akut OM, uygun antibiyoterapiye ra\u011fmen kulak zar\u0131nda perforasyon ve p\u00fcr\u00fclan kulak ak\u0131nt\u0131s\u0131 ile karakterize olup alt\u0131 haftadan uzun s\u00fcren kal\u0131c\u0131 bir inflamatuar s\u00fcre\u00e7 olan kronik s\u00fcp\u00fcratif OM (KSOM)\u2019a ilerleyebilir. KSOM\u2019un prevalans\u0131 geli\u015fmi\u015f \u00fclkelerde %1\u2019den az olup geli\u015fmekte olan \u00fclkelerde %4 olarak bildirilmi\u015ftir. Hastal\u0131k s\u00fcrecinde kolestatom e\u015flik edebilmekle birlikte i\u015fitme kayb\u0131 KSOM\u2019un en yayg\u0131n sekellerinden biridir (3,4).<\/p>\n<p class=\"p3\">Tiplendirilemeyen\u00a0<i>Haemophilus influenzae, Streptococcus pneumoniae, S. aureus, Streptococcus pyogenes, P. aeruginosa, Proteus mirabilis, Escherichia coli<\/i>\u00a0ve\u00a0<i>Moraxella catarrhalis <\/i>gibi patojenik bakteriler orta kulak iltihab\u0131n\u0131n ba\u015fl\u0131ca etiyolojik ajanlar\u0131d\u0131r (5-10); viruslar ve mantarlar da OM nedeni olabilir (9-11).<\/p>\n<p class=\"p3\"><span class=\"s1\">A\u015f\u0131r\u0131 ve yanl\u0131\u015f kullan\u0131m\u0131n sonucu olarak antibiyotiklere kar\u015f\u0131 artan bakteri direnci, OM\u2019a neden olan ana patojenlerde ve antibiyotik duyarl\u0131l\u0131klar\u0131nda de\u011fi\u015fikliklere yol a\u00e7m\u0131\u015ft\u0131r.\u00a0Ampirik antibiyotik tedavisi, etkeni antibiyoti\u011fe diren\u00e7li hastal\u0131klarda, tedavi ba\u015far\u0131s\u0131zl\u0131\u011f\u0131na neden olabilir veya komplikasyonlarla sonu\u00e7lanabilir (12). \u00c7al\u0131\u015fmam\u0131zda kulak infeksiyonu ile ili\u015fkili bakteriyel patojenlerin tan\u0131mlanmas\u0131 ve izole edilen organizmalar aras\u0131nda antibakteriyel direncin de\u011ferlendirilmesi ama\u00e7land\u0131.<\/span><\/p>\n<h2 class=\"p1\">Y\u00d6NTEMLER<\/h2>\n<p class=\"p3\">\u015eubat 2022-\u015eubat 2023 tarihleri aras\u0131nda hastanemizin Kulak Burun Bo\u011faz Poliklini\u011fi\u2019ne ba\u015fvuran hastalar\u0131n kulak infeksiyonu \u00f6n tan\u0131s\u0131yla laboratuvar\u0131m\u0131za g\u00f6nderilen kulak s\u00fcr\u00fcnt\u00fc \u00f6rnekleri incelendi. Kulak s\u00fcr\u00fcnt\u00fc \u00f6rne\u011finden yap\u0131lan Gram boyama ile k\u00fclt\u00fcr \u00fcreme sonucu uyumlu olan 144 hasta \u00e7al\u0131\u015fmaya dahil edildi. Klinik olarak mantar infeksiyonu \u015f\u00fcphesi olan hastalarda birden fazla \u00f6rnekte ayn\u0131 mantar\u0131n \u00fcremesi ya da birden fazla besiyerinde \u00fcreme saptanmas\u0131 durumunda izole edilen mantar etken patojen olarak kabul edildi (13).<span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p class=\"p3\">Hastalardan al\u0131nan t\u00fcm s\u00fcr\u00fcnt\u00fc \u00f6rnekleri; %5-10\u00a0koyun kanl\u0131\u00a0agar (Orbak, Ankara, T\u00fcrkiye), \u00e7ikolatams\u0131 agar (Orbak, Ankara, T\u00fcrkiye), \u201ceosin methylen-blue\u201d (EMB) agar (Orbak, Ankara, T\u00fcrkiye) ve \u201cSabouraud\u00a0dextrose\u00a0agar\u201d (SDA) (Orbak, Ankara, T\u00fcrkiye) kullan\u0131larak 37\u02daC\u2019de 18-24 saat ink\u00fcbe edildi. K\u00fclt\u00fcrde \u00fcreme olan mikroorganizmalar\u0131n tan\u0131mlanmas\u0131 konvansiyonel y\u00f6ntemler (koloni morfolojisi, Gram boyama \u00f6zelli\u011fi, Gram-pozitif bakteriler i\u00e7in katalaz, koag\u00fclaz ve DNase enzim aktivitesi, Gram-negatif bakteriler i\u00e7in ise oksidaz testi, \u015feker fermentasyonu, indol \u00fcretimi, sitrat kullan\u0131m\u0131, H<span class=\"s2\"><sub>2<\/sub><\/span>S \u00fcretimi, \u00fcreaz enzim aktivitesi ve hareket \u00f6zelli\u011fi) ile tan\u0131mland\u0131. Maya su\u015flar\u0131n\u0131n tan\u0131mlanmas\u0131nda ise germ t\u00fcp y\u00f6ntemi kullan\u0131ld\u0131. <i>Aspergillus<\/i> t\u00fcr komplekslerinin ayr\u0131m\u0131 i\u00e7in SDA\u2019da \u00fcreyen k\u00fcf kolonileri makroskobik olarak de\u011ferlendirildi. \u00dcreyen k\u00fcf kolonilerini mikroskobik olarak de\u011ferlendirmek i\u00e7in laktofenoll\u00fc pamuk mavisi boyas\u0131 ile preparat haz\u0131rland\u0131. Haz\u0131rlanan preparatlar X40\u2019l\u0131k objektifte incelenerek tan\u0131mland\u0131. \u0130zole edilen su\u015flar\u0131n antimikrobiyal duyarl\u0131l\u0131klar\u0131 Kirby-Bauer disk dif\u00fczyon y\u00f6ntemi kullan\u0131larak belirlendi ve Avrupa Antimikrobiyal Duyarl\u0131l\u0131k Testi Komitesi (European Committee on Antimicrobial Susceptibility Testing &#8211; EUCAST) kriterlerine uygun olarak de\u011ferlendirildi (14).<\/p>\n<p class=\"p3\">\u00c7al\u0131\u015fma i\u00e7in Van E\u011fitim ve Ara\u015ft\u0131rma Hastanesi Klinik Ara\u015ft\u0131rmalar Etik Kurulu\u2019ndan 26 Nisan 2023 tarih ve 2023\/08-07 karar numaras\u0131yla onay al\u0131nm\u0131\u015ft\u0131r.<\/p>\n<h3 class=\"p6\">\u0130statistiksel Analiz<\/h3>\n<p class=\"p3\">Verilerin analizi, \u201cStatistical Package for the Social Sciences\u201d (SPSS) versiyon 22.0 (IBM Corp., Armonk, NY, ABD) program\u0131 kullan\u0131larak yap\u0131ld\u0131. Gruplar\u0131n normal da\u011f\u0131l\u0131ma uyumlulu\u011fu Shapiro-Wilk testi ile de\u011ferlendirildi. Normal da\u011f\u0131l\u0131m g\u00f6steren gruplar\u0131n verileri \u201cortalama\u00b1standart sapma\u201d \u015feklinde, normal da\u011f\u0131l\u0131ma uymayan gruplar\u0131n verileri \u201cmedyan [25. y\u00fczdelik \u2013 75. y\u00fczdelik]\u201d \u015feklinde g\u00f6sterildi. Normal da\u011f\u0131l\u0131ma uyan gruplar\u0131n kantitatif verilerinin kar\u015f\u0131la\u015ft\u0131rmas\u0131nda Student t-testi kullan\u0131ld\u0131. Normal da\u011f\u0131l\u0131ma uymayan iki grubun kantitatif verilerinin kar\u015f\u0131la\u015ft\u0131r\u0131lmas\u0131nda Mann-Whitney U testi kullan\u0131ld\u0131. Kalitatif verilerin kar\u015f\u0131la\u015ft\u0131r\u0131lmas\u0131nda gruplar\u0131n \u00f6rneklem say\u0131lar\u0131n\u0131n hepsinin be\u015ften b\u00fcy\u00fck olmas\u0131 durumunda \u03c7\u00b2 testi, herhangi bir verinin be\u015ften k\u00fc\u00e7\u00fck olmas\u0131 durumunda ise Fischer testi kullan\u0131ld\u0131. Testlerin <i>p<\/i> de\u011ferlerinin 0.05\u2019ten k\u00fc\u00e7\u00fck olmas\u0131 durumu istatistiksel olarak anlaml\u0131 d\u00fczey olarak kabul edildi.<\/p>\n<h2 class=\"p1\">BULGULAR<span class=\"Apple-converted-space\">\u00a0 <\/span><\/h2>\n<p class=\"p3\">\u00c7al\u0131\u015fma kapsam\u0131nda; 144 hastadan ilk ba\u015fvurusunda, 19 hastadan ise daha sonraki d\u00f6nemlerde tekrarlayan d\u0131\u015f kulak yolu ak\u0131nt\u0131s\u0131 nedeniyle al\u0131nm\u0131\u015f olan toplam 171 d\u0131\u015f kulak yolu k\u00fclt\u00fcr sonucu incelendi; 90 \u00f6rnekte patojen mikroorganizma \u00fcremesi g\u00f6zlenirken, 58 \u00f6rnekte normal kulak floras\u0131 \u00fcredi ve 23 \u00f6rnekte mikroorganizma \u00fcremesi g\u00f6zlenmedi.<\/p>\n<p class=\"p3\"><span class=\"s3\">Patojen mikroorganizma \u00fcreyen 90 hastan\u0131n; 39 (%43)\u2019u erkek, 51 (%57)\u2019i kad\u0131n ve ya\u015f medyan ortalamas\u0131 30 [14-47] y\u0131l olarak tespit edildi. Patojen mikroorganizma \u00fcremesi olmayan 81 hastan\u0131n; 43 (%53)\u2019\u00fc erkek, 38 (%47)\u2019i kad\u0131n ve ya\u015f medyan ortalamas\u0131 19 [5-35] y\u0131l idi. Patojen mikroorganizma \u00fcremesi olan hastalar ile olmayan hastalar cinsiyet a\u00e7\u0131s\u0131ndan benzer bulunurken (<i>p<\/i>=0.202), ya\u015f medyan de\u011feri \u00fcreme olan grupta anlaml\u0131 olarak daha y\u00fcksekti (<i>p<\/i>=0.005). On hastada kolesteatom mevcuttu; bir hastada kolesteatom n\u00fcks\u00fc geli\u015fmi\u015f olup d\u0131\u015f merkeze sevk edilmi\u015fti. Maya \u00fcremesi olan hastalardan birinde <i>C. albicans<\/i>, be\u015finde non<i>-albicans Candida<\/i> \u00fcremesi olmu\u015ftu; non<i>-albicans Candida<\/i> \u00fcremesi olan hastalar\u0131n tamam\u0131 18 ya\u015f \u00fczeri gruptayd\u0131 (Tablo 3).<\/p>\n<p class=\"p3\">Tekrarlayan kulak ak\u0131nt\u0131s\u0131 nedeniyle 19 hastadan tekrar k\u00fclt\u00fcr al\u0131nm\u0131\u015f olup s\u00f6z konusu k\u00fclt\u00fcrlerde en fazla <i>Pseudomonas<\/i> spp. (n=3) \u00fcremesi oldu\u011fu tespit edildi. <i>Pseudomonas<\/i> spp. (%30.5) ve <i>S. aureus <\/i>(%31.5)\u2019un\u00a0kulak iltihab\u0131ndan sorumlu en yayg\u0131n bakteriyel patojenler oldu\u011fu g\u00f6r\u00fcld\u00fc (Tablo 3). \u0130zole edilen etkenlerin ya\u015fa g\u00f6re da\u011f\u0131l\u0131m\u0131na bak\u0131ld\u0131\u011f\u0131nda, 18 ya\u015f \u00fcst\u00fc hastalarda 18 ya\u015f alt\u0131 hastalara g\u00f6re anlaml\u0131 d\u00fczeyde daha fazla \u00fcreme oldu\u011fu tespit edildi (<i>p<\/i>&lt;0.001).<span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p class=\"p3\">On sekiz ya\u015f \u00fcst\u00fc hastalarda; <i>Pseudomonas<\/i> spp. (%24), <i>S. aureus<\/i> (%17) ve <i>Aspergillus niger<\/i> (%11) en s\u0131k g\u00f6r\u00fclen patojenlerdi. <i>Pseudomonas<\/i> spp.\u2019nin 18 ya\u015f \u00fcst\u00fc hastalarda, 0-5 ya\u015f aras\u0131 (<i>p<\/i>=0.024) ve 5-18 ya\u015f aras\u0131 (<i>p<\/i>=0.015) hastalara g\u00f6re anlaml\u0131 d\u00fczeyde daha fazla g\u00f6r\u00fcld\u00fc\u011f\u00fc tespit edildi (Tablo 3).<span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p class=\"p3\">Be\u015f ya\u015f alt\u0131 hastalarda en s\u0131k olarak <i>S. aureus<\/i> (%46.6) ve ikinci s\u0131kl\u0131kta <i>S. pneumoniae<\/i> (%33.3) \u00fcremesi olmu\u015ftu. <i>S. pneumoniae<\/i> \u00fcremesinin be\u015f ya\u015f alt\u0131 hastalarda, 18 ya\u015f \u00fcst\u00fc hastalara g\u00f6re anlaml\u0131 d\u00fczeyde daha y\u00fcksek oldu\u011fu g\u00f6r\u00fcld\u00fc (<i>p<\/i>&lt;0.001). Be\u015f ya\u015f alt\u0131 hastalarda k\u00fcf \u00fcremesi olmad\u0131 (Tablo 3).<span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p class=\"p3\"><span class=\"s3\"><i>P<\/i><\/span><i>seudomonas<\/i> spp. \u00fcremesi olan hastalar\u0131n ortalama ya\u015f\u0131 41.5 (\u00b1 23.04) y\u0131l ve <i>S. aureus<\/i> \u00fcremesi olan hastalar\u0131n ortalama ya\u015f\u0131 23.1 (\u00b118.73) y\u0131l olarak bulundu. Etkeni bakteriyel, viral veya fungal patojenler olabilen OM; \u00f6dem, hiperemi, a\u011fr\u0131ya neden olur ve orta kulakta s\u0131v\u0131 birikiminin yan\u0131 s\u0131ra inflamasyon ile karakterizedir. \u201cY\u00fcz\u00fcc\u00fc kula\u011f\u0131\u201d olarak da adland\u0131r\u0131lan OE ise zaman zaman cerahatli ak\u0131nt\u0131larla birlikte a\u011fr\u0131, k\u0131zar\u0131kl\u0131k ve hassasiyetin e\u015flik etti\u011fi d\u0131\u015f kulak yolu infeksiyonu ve iltihaplanmas\u0131 ile ortaya \u00e7\u0131kar (15).<\/p>\n<p class=\"p3\"><span class=\"s1\">Akut ve kronik kulak infeksiyonlar\u0131, hastane ba\u015fvurular\u0131n\u0131n ve bunun sonucunda antibiyotik tedavisinin en yayg\u0131n nedenleri olarak olduk\u00e7a \u00f6nemlidir (15). Yak\u0131n bir d\u00f6neme kadar otitin alt\u0131 ya\u015f\u0131n alt\u0131ndaki \u00e7ocuklarda daha yayg\u0131n oldu\u011fu d\u00fc\u015f\u00fcn\u00fcl\u00fcrken yap\u0131lan bir \u00e7al\u0131\u015fmada otit \u015fikayetleri i\u00e7in hastane ba\u015fvurular\u0131n\u0131n yakla\u015f\u0131k yar\u0131s\u0131n\u0131n yeti\u015fkinlerden olu\u015ftu\u011fu bildirilmi\u015ftir (16). Yap\u0131lan bir di\u011fer \u00e7al\u0131\u015fmada da ya\u015f ortalamas\u0131 32.8 y\u0131l olarak bulunmu\u015ftur (17). \u00c7al\u0131\u015fmam\u0131zda da yap\u0131lan \u00e7al\u0131\u015fmalar\u0131 destekler bir \u015fekilde ya\u015f medyan de\u011feri 30 [14-47] y\u0131l olarak bulundu.<\/span><\/p>\n<p class=\"p3\">Otitis median\u0131n erkeklerde daha yayg\u0131n olarak ortaya \u00e7\u0131kt\u0131\u011f\u0131n\u0131 bildiren \u00e7al\u0131\u015fmalar oldu\u011fu gibi kad\u0131nlarda daha yayg\u0131n oldu\u011funu bildiren \u00e7al\u0131\u015fmalar da mevcuttur (18-21). \u00c7al\u0131\u015fmam\u0131zda 171 d\u0131\u015f kulak yolu k\u00fclt\u00fcr\u00fc sonucu incelenmi\u015f ve etken patojen \u00fcreyen hastalar\u0131n cinsiyeti a\u00e7\u0131s\u0131ndan fark olmad\u0131\u011f\u0131 g\u00f6r\u00fclm\u00fc\u015ft\u00fcr (<i>p<\/i>=0.202). \u00c7eviker ve arkada\u015flar\u0131n\u0131n (22) yapt\u0131\u011f\u0131 \u00e7al\u0131\u015fmada da cinsiyetler a\u00e7\u0131s\u0131ndan fark olmad\u0131\u011f\u0131 bildirilmi\u015ftir.<\/p>\n<p class=\"p3\">Otitis eksternada en s\u0131k olarak tragus ve\/veya kulak kep\u00e7esi \u00fczerine bask\u0131 ile artan \u015fiddetli a\u011fr\u0131 g\u00f6r\u00fcl\u00fcr. \u0130\u015fitme azalmas\u0131, ka\u015f\u0131nt\u0131 ve kulak ak\u0131nt\u0131s\u0131 di\u011fer semptomlard\u0131r. \u015eiddetli bakteriyel otitis eksternada d\u0131\u015f kulak yolunun tamamen kapanmas\u0131 sonucunda kulak kep\u00e7esinde \u00f6dem g\u00f6r\u00fclebilir. Kronik s\u00fcp\u00fcratif otitis median\u0131n semptomlar\u0131 aras\u0131nda ise kulak ak\u0131nt\u0131s\u0131, a\u011fr\u0131, i\u015fitme azalmas\u0131, dengesizlik ve ba\u015f d\u00f6nmesi bulunmaktad\u0131r (23). \u00c7al\u0131\u015fmam\u0131zdaki hastalarda en s\u0131k g\u00f6r\u00fclen ba\u015fvuru \u015fikayetleri kulak ak\u0131nt\u0131s\u0131 (%60.2) ve kulak a\u011fr\u0131s\u0131 (%12.8) idi.<\/p>\n<p class=\"p3\"><span class=\"s1\">\u00c7al\u0131\u015fmam\u0131zda, d\u0131\u015f kulak yolu k\u00fclt\u00fcr\u00fc incelenen 90 (%53) hastada etken patojen mikroorganizma \u00fcredi\u011fi tespit edildi. Bakteri \u00fcremesi olan toplam 74 \u00f6rne\u011fin 38 (%51)\u2019inde Gram-negatif, 36 (%49)\u2019s\u0131nda Gram-pozitif bakteri \u00fcremesi g\u00f6r\u00fcld\u00fc. Toplam 152 \u00e7ocu\u011fun dahil edildi\u011fi bir \u00e7al\u0131\u015fmada, \u00e7ocuklar\u0131n 115 (%75.6)\u2019inde patojenik bakteri \u00fcremesi bildirilmi\u015f olup en s\u0131k izole edilen patojenler\u00a0<i>S. aureus<\/i> (%27)<i> ve\u00a0P. mirabilis<\/i> (%12.5)\u2019dir (24).<span class=\"Apple-converted-space\">\u00a0<\/span><\/span><\/p>\n<p class=\"p3\"><span class=\"s3\">\u00c7al\u0131\u015fmam\u0131zda, patojen mikroorganizma \u00fcremesi olan hastalarda en s\u0131k kar\u015f\u0131la\u015f\u0131lan etken olarak <i>S. aureus <\/i>(%31.5) ve <i>Pseudomonas <\/i>spp. (%30) izole edildi. Otitis externa ve OM\u2019daki bask\u0131n patojenlere dair bulgular\u0131m\u0131z \u00f6nceki \u00e7al\u0131\u015fmalar\u0131 desteklemektedir. Yap\u0131lan di\u011fer \u00e7al\u0131\u015fmalarda da en s\u0131k kar\u015f\u0131la\u015f\u0131lan etken olarak <i>P. aeruginosa<\/i> izole edilmi\u015ftir (22,25). Samsun\u2019da 62 hastan\u0131n kulak k\u00fclt\u00fcr\u00fcn\u00fcn incelendi\u011fi ba\u015fka bir \u00e7al\u0131\u015fmada, en s\u0131k kar\u015f\u0131la\u015f\u0131lan etkenler olarak <i>S. aureus<\/i> (%24.2) ve <i>P. aeruginosa<\/i> (%19.4) saptanm\u0131\u015ft\u0131r (26). Sa\u011fl\u0131kl\u0131 ve hasta ki\u015filerdeki mikrobiyomun ara\u015ft\u0131r\u0131ld\u0131\u011f\u0131 bir di\u011fer \u00e7al\u0131\u015fmada hem OM hem de OE\u2019nin bakteriyel analizlerinde <i>P. aeruginosa<\/i> ve <i>S. aureus<\/i> bask\u0131n patojenler olarak bulunmu\u015ftur (15). \u00dclkemizde yap\u0131lan bir \u00e7al\u0131\u015fmada 695 adet d\u0131\u015f kulak yolu k\u00fclt\u00fcr sonucu incelenmi\u015f olup izole edilen mikroorganizmalar aras\u0131nda <i>S. aureus<\/i> (%17.8) ve <i>P. aeruginosa<\/i> (%17.1) en s\u0131k izole edilen iki patojen olarak tespit edilmi\u015ftir (27). \u00c7al\u0131\u015fmam\u0131z\u0131n bulgular\u0131 \u00f6nceki \u00e7al\u0131\u015fmalarla benzerlik g\u00f6stermektedir.<span class=\"Apple-converted-space\">\u00a0<\/span><\/span><\/p>\n<p class=\"p3\">Klinik <i>Aspergillus<\/i> izolatlar\u0131n\u0131n incelendi\u011fi bir \u00e7al\u0131\u015fmada, kulak k\u00fclt\u00fcr \u00f6rneklerinde en s\u0131k olarak <i>A. niger<\/i>, ikinci s\u0131rada <i>A. fumigatus<\/i> \u00fcremesi saptanm\u0131\u015ft\u0131r (28). Bir di\u011fer \u00e7al\u0131\u015fmada \u00fcreme saptanan kulak k\u00fclt\u00fcrlerinin 9 (%13.6)\u2019unda mantar ve 57 (%86.4)\u2019sinde bakteri \u00fcremesi bildirilmi\u015ftir; k\u00fclt\u00fcrlerin 7 (%10.6)\u2019sinde k\u00fcf mantar\u0131 \u00fcrerken, 2 (%3.1)\u2019sinde <i>Candida <\/i>\u00fcremesi olmu\u015ftur (22). \u00c7al\u0131\u015fmam\u0131zda 21 (%12) \u00f6rnekte mantar \u00fcremesi g\u00f6r\u00fcld\u00fc. En s\u0131k mantar etkeni olarak <i>A. niger<\/i> (n=14) \u00fcremesi saptanm\u0131\u015f olup daha \u00e7ok ileri ya\u015fta olan hastalarda g\u00f6r\u00fcld\u00fc ve ortalama ya\u015f 41.2 (\u00b125.11) y\u0131l olarak bulundu. Klinik otomikoz etkenlerinin ara\u015ft\u0131r\u0131ld\u0131\u011f\u0131 bir \u00e7al\u0131\u015fmada en s\u0131k kar\u015f\u0131la\u015f\u0131lan etken olarak <i>A. niger<\/i> bildirilmi\u015f ve <i>A. niger<\/i>\u2019in neden oldu\u011fu otomikozlar 21-40 ya\u015f aras\u0131 erkeklerde daha fazla tespit edilmi\u015ftir (29).<span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p class=\"p3\">\u00c7al\u0131\u015fmam\u0131zdaki be\u015f hastada polimikrobiyal \u00fcreme vard\u0131. En s\u0131k g\u00f6r\u00fclen etken <i>E. coli<\/i> ve <i>Candida<\/i> spp. idi. Yap\u0131lan bir \u00e7al\u0131\u015fmada, polimikrobiyal \u00fcremede en s\u0131k g\u00f6r\u00fclen etkenler olarak <i>P. mirabilis<\/i> ve <i>K. pneumoniae<\/i> bulunmu\u015ftur. S\u00f6z konusu \u00e7al\u0131\u015fmada, \u0131slak ve k\u00f6t\u00fc hijyenik ortam\u0131n <i>E. coli<\/i> gibi koliform bakterilerin ve <i>Candida<\/i> gibi mayalar\u0131n \u00e7o\u011falmas\u0131n\u0131 kolayla\u015ft\u0131rd\u0131\u011f\u0131; bunun da kulak zar\u0131 perforasyonu ile d\u0131\u015f kulaktan orta kula\u011fa g\u00f6\u00e7 yoluyla polimikrobiyal \u00fcremeye neden oldu\u011fu bildirilmi\u015ftir (30).<\/p>\n<p class=\"p3\">\u00c7al\u0131\u015fmam\u0131z kapsam\u0131nda en fazla antibiyotik direnci ampisiline (%77.7) kar\u015f\u0131 g\u00f6r\u00fcld\u00fc; bunu s\u0131ras\u0131yla seftazidim (%73.6), penisilin (%50) ve amoksisilin-klavunat (%44.4) izledi. \u0130zole edilen <i>Klebsiella<\/i> spp., <i>Proteus<\/i> spp. ve <i>Acinetobacter<\/i> spp. ampisiline kar\u015f\u0131 %100 diren\u00e7 g\u00f6sterdi. Otit tedavisinde s\u0131k olarak kullan\u0131lan siprofloksasine kar\u015f\u0131 diren\u00e7 6 (%8) hastada g\u00f6r\u00fcld\u00fc. \u0130zole edilen <i>S. pneumoniae<\/i> izolatlar\u0131n\u0131n hi\u00e7biri penisiline kar\u015f\u0131 diren\u00e7 g\u00f6stermedi.<b> <\/b><i>S. aureus<\/i> k\u00f6kenlerinin %60\u2019\u0131 penisiline diren\u00e7liydi. Tadesse ve arkada\u015flar\u0131n\u0131n (24) \u00e7al\u0131\u015fmas\u0131nda, de\u011ferlendirilen izolatlar\u0131n %11.2\u2019si gentamisin ve %7.3\u2019\u00fc siprofloksasine diren\u00e7li olarak tespit edilmi\u015ftir.\u00a0Ayr\u0131ca, s\u00f6z konusu \u00e7al\u0131\u015fmada; izolatlar\u0131n %85.2\u2019si ampisiline,\u00a0%71.4\u2019\u00fc penisiline ve %72\u2019si trimetoprim-s\u00fclfametoksazole diren\u00e7li bulunmu\u015ftur (24).<\/p>\n<p class=\"p3\">Sonu\u00e7 olarak; otitin birinci basamak tedavisinde \u00f6ncelikli olarak kullan\u0131lan ampisilin, amoksisilin ve amoksisilin-klavulanat gibi ila\u00e7lar en s\u0131k direncin g\u00f6r\u00fcld\u00fc\u011f\u00fc ila\u00e7lar olmu\u015ftur.
