T\u00fcm ya\u015f gruplar\u0131nda ve her iki cinste toplum k\u00f6kenli \u0130YE\u2019ye s\u0131kl\u0131kla <i>Escherichia coli <\/i>ve <i>Klebsiella<\/i> spp. gibi Gram-negatif basiller neden olmaktad\u0131r (2).<\/p>\n<p class=\"p2\">\u0130drar yolu infeksiyonlar\u0131 piy\u00fcri ve \u00e7e\u015fitli klinik semptomlar\u0131n e\u015flik etti\u011fi b\u00f6brek, toplay\u0131c\u0131 sistem ve mesane inflamasyonu olarak tan\u0131mlan\u0131r. Klinik bulgular; hastan\u0131n ya\u015f\u0131, cinsiyeti ve e\u015flik eden hastal\u0131klar nedeniyle de\u011fi\u015fkenlik g\u00f6sterebilir. Bulgular sepsisle sonu\u00e7lanabilen akut piyelonefritten, asemptomatik bakteri\u00fcriye kadar \u00e7ok \u00e7e\u015fitli yelpazede olabilir (3). Antibiyogram test sonucunun zaman almas\u0131 nedeniyle genellikle hastalara ampirik antibiyotik tedavisi uygulanmaktad\u0131r. Geli\u015fmi\u015f \u00fclkelerde \u0130YE tedavisinde kullan\u0131lan antibiyotikler toplamda kullan\u0131lan antibiyotiklerin yakla\u015f\u0131k %15\u2019ini olu\u015fturmakta olup trimetoprim-s\u00fclfametoksazol (TMP-SXT) ve siprofloksasin \u0130YE tedavisinde en s\u0131k kullan\u0131lan ajanlar aras\u0131nda yer almaktad\u0131r (3).<\/p>\n<p class=\"p2\">Toplum k\u00f6kenli \u0130YE\u2019lerde ampirik tedaviye yan\u0131t al\u0131namamas\u0131 durumunda ya da tekrarlayan olgularda idrar k\u00fclt\u00fcr\u00fc ve antibiyotik duyarl\u0131l\u0131k sonu\u00e7lar\u0131na ba\u015fvurulmaktad\u0131r. Tedavide ideal antibiyoti\u011fin belirlenebilmesi i\u00e7in yerel antibiyotik diren\u00e7 durumunun d\u00fczenli olarak takip edilmesi \u00f6nemlidir (4). Artan antimikrobiyal diren\u00e7, ampirik antibiyotik se\u00e7iminde \u00f6nemli bir sorun olu\u015fturmakta ve tedavi ba\u015far\u0131 oranlar\u0131n\u0131 d\u00fc\u015f\u00fcrmektedir (5). \u00c7al\u0131\u015fmam\u0131zda, toplum k\u00f6kenli \u0130YE \u015f\u00fcphesiyle hastanemiz laboratuvar\u0131na gelen idrar \u00f6rneklerinde etken olarak <i>E. coli<\/i> ve <i>Klebsiella<\/i> spp.\u2019\u0131n y\u0131llara g\u00f6re da\u011f\u0131l\u0131m\u0131n\u0131 ve TMP-SMT ve siprofloksasin antimikrobiyal diren\u00e7 oranlar\u0131n\u0131 incelemeyi ama\u00e7lad\u0131k.<\/p>\n<h2 class=\"p1\">Y\u00d6NTEMLER<\/h2>\n<p class=\"p2\">\u00c7al\u0131\u015fma kapsam\u0131nda, Ocak 2011 ile A\u011fustos 2021 tarihleri aras\u0131nda poliklinikler ve acil servislerden T\u0131bbi Mikrobiyoloji Laboratuvar\u0131\u2019na g\u00f6nderilen her ya\u015f grubuna ait t\u00fcm idrar k\u00fclt\u00fcr \u00f6rnekleri retrospektif olarak incelendi. \u0130drar \u00f6rne\u011finde 10<sup>4<\/sup> cfu\/ml ve \u00fczerinde koloni olu\u015fturarak \u00fcreyen bir \u00e7e\u015fit mikroorganizma idrar yolu infeksiyonunun etkeni olarak kabul edildi (6). \u00c7al\u0131\u015fmaya, idrar k\u00fclt\u00fcr\u00fcnde etken olarak <i>E. coli<\/i> ve <i>Klebsiella <\/i>spp. saptanan \u00f6rnekler ve bunlar\u0131n TMP-SXT, siprofloksasin duyarl\u0131l\u0131k sonu\u00e7lar\u0131 dahil edildi. Kontaminasyon olarak sonu\u00e7lanan veya mikolojik \u00fcremesi olan k\u00fclt\u00fcr \u00f6rnekleri \u00e7al\u0131\u015fma d\u0131\u015f\u0131 b\u0131rak\u0131ld\u0131. Her hastan\u0131n tek bir \u00f6rne\u011fi \u00e7al\u0131\u015fmaya dahil edildi. K\u00fclt\u00fcr sonu\u00e7lar\u0131, \u00fcreyen etken ve antibiyotik duyarl\u0131l\u0131klar\u0131 a\u00e7\u0131s\u0131ndan de\u011ferlendirildi.<\/p>\n<p class=\"p2\">\u00d6rnekler, standart 0.01 ml\u2019lik \u00f6ze ile %5 koyun kanl\u0131 agar ve \u201ceosin methylen blue\u201d (EMB) agara kantitatif y\u00f6ntemle ekildi. Aerop ko\u015fullarda 35-37 \u2070C\u2019lik et\u00fcvde, 24-48 saat ink\u00fcbe edildikten sonra \u00fcreme kontrol\u00fc yap\u0131ld\u0131. \u00dcreyen bakterilerin tan\u0131mlanmas\u0131 ve antibiyotik duyarl\u0131l\u0131k testleri konvansiyonel y\u00f6ntemler (Kirby Bauer disk dif\u00fczyon y\u00f6ntemi) ve otomatize sistemlerle (Phoenix BD ve Bruker MALDI-TOF MS) \u00e7al\u0131\u015f\u0131ld\u0131. Antibiyogram verileri; 2011-2015 y\u0131llar\u0131 aras\u0131nda Klinik ve Laboratuvar Standartlar\u0131 Enstit\u00fcs\u00fc (Clinical and Laboratory Standards Institute &#8211; CLSI) ve 2016-2021 y\u0131llar\u0131 aras\u0131nda Avrupa Antimikrobiyal Duyarl\u0131l\u0131k Komitesi (European Committee on Antimicrobial Susceptibility Testing &#8211; EUCAST) \u00f6nerileri do\u011frultusunda de\u011ferlendirildi (7).<\/p>\n<p class=\"p2\"><span class=\"s1\">Verilerin analizi, SPSS (Statistical Package for the Social Sciences) versiyon 22.0 program\u0131 (IBM Corp., Armonk, NY, ABD) ile yap\u0131ld\u0131. Kalitatif veriler g\u00f6zlem say\u0131s\u0131 ve s\u0131kl\u0131\u011f\u0131 (%), kantitatif veriler ortanca, \u00e7eyrekler aras\u0131 aral\u0131k, minimum (min), maksimum (mak) de\u011ferler ile ifade edildi. Kantitatif verilerin analizinde Mann-Whitney U testi ve kalitatif verilerin analizinde \u03c72 testi kullan\u0131ld\u0131. T\u00fcm analizler %95 g\u00fcven aral\u0131\u011f\u0131 (GA)\u2019nda yap\u0131ld\u0131 ve <i>p<\/i>&lt;0.05 de\u011ferleri istatistiksel olarak anlaml\u0131 d\u00fczey olarak kabul edildi.<\/span><\/p>\n<p class=\"p2\">\u00c7al\u0131\u015fma, Sa\u011fl\u0131k Bilimleri \u00dcniversitesi \u0130zmir Tepecik E\u011fitim ve Ara\u015ft\u0131rma Hastanesi Giri\u015fimsel Olmayan Hastalar\u0131n %74.3\u2019\u00fc kad\u0131n olup medyan ya\u015f 30 [\u201cinterquartile range\u201d (IQR)=52; min-maks=0-100] y\u0131l idi.<\/p>\n<p class=\"p2\"><span class=\"s2\">S<\/span>iprofloksasin ve TMP-SXT diren\u00e7 oranlar\u0131 <i>E. coli <\/i>i\u00e7in s\u0131ras\u0131yla en d\u00fc\u015f\u00fck ve en y\u00fcksek %25-40 ve %35-51 aral\u0131\u011f\u0131nda saptand\u0131 (\u015eekil 1 ve 2).<span class=\"Apple-converted-space\">\u00a0 <\/span><i>Klebsiella<\/i> spp. i\u00e7in ise diren\u00e7 oranlar\u0131 s\u0131ras\u0131yla en d\u00fc\u015f\u00fck ve en y\u00fcksek %17-43 ve %31-43 aral\u0131\u011f\u0131ndayd\u0131 (\u015eekil 3 ve 4).<\/p>\n<p class=\"p2\">Erkek cinsiyette siprofloksasin ve TMP-SXT diren\u00e7 oranlar\u0131 kad\u0131n cinsiyete g\u00f6re anlaml\u0131 d\u00fczeyde daha y\u00fcksek saptand\u0131 (<i>p<\/i>&lt;0.001) (\u015eekil 5 ve 6).<\/p>\n<h2 class=\"p1\">\u0130RDELEME<\/h2>\n<p class=\"p2\">\u0130drar yolu infeksiyonlar\u0131 toplum kaynakl\u0131 olarak en s\u0131k kar\u015f\u0131la\u015f\u0131lan bakteriyel infeksiyonlardan olup her ya\u015f grubunda g\u00f6r\u00fclebilir. Asemptomatik bakteri\u00fcri ve sistit gibi hafif klinik tablolardan piyelonefrit ve \u00fcrosepsise kadar de\u011fi\u015febilen geni\u015f klinik yelpazesi bulunan bu infeksiyonlar \u00f6nemli morbidite ve mortalite nedenidir (8). Y\u00fcksek n\u00fcks oranlar\u0131 ve izole edilen bakterilerdeki artan antibiyotik direnci nedeniyle s\u00f6z konusu infeksiyonlar\u0131n \u00f6nemi artmaktad\u0131r (8). Toplum k\u00f6kenli \u0130YE\u2019ye en s\u0131k neden olan mikroorganizmalar, Gram-negatif basiller olup bunlar aras\u0131nda ilk s\u0131ray\u0131 <i>E. coli <\/i>(%50-90) almaktad\u0131r (2); <i>E. coli<\/i>\u2019den sonra en s\u0131k izole edilen patojenler aras\u0131nda <i>Klebsiella<\/i> spp. bulunmaktad\u0131r (9).<\/p>\n<p class=\"p2\">\u0130drar yolu infeksiyonu tan\u0131s\u0131nda alt\u0131n standart y\u00f6ntem k\u00fclt\u00fcrd\u00fcr. K\u00fclt\u00fcr ve antibiyogram i\u015flemlerinin sonu\u00e7lanmas\u0131 zaman ald\u0131\u011f\u0131ndan, hekimler s\u0131kl\u0131kla ampirik tedaviye ba\u015flamay\u0131 tercih etmektedir. Ampirik tedavide kullan\u0131lacak ajanlar\u0131n do\u011fru se\u00e7imi i\u00e7in b\u00f6lgenin etken da\u011f\u0131l\u0131m\u0131 ve antibiyotik diren\u00e7 durumunun bilinmesi \u00f6nemlidir (10). Avrupa \u00dcroloji Derne\u011fi (European Association of Urology &#8211; EAU) rehberlerinde belli oranlar\u0131n \u00fczerinde diren\u00e7 saptanan antibiyotiklerin, ampirik tedavide kullan\u0131mlar\u0131 \u00f6nerilmektedir (11). \u00dclkemiz genelinde yerel s\u00fcrveyans verilerinin, s\u0131k kar\u015f\u0131la\u015f\u0131lan etkenlerin ve diren\u00e7 oranlar\u0131n\u0131n belirli aral\u0131klarla g\u00fcncellenmesi, \u00f6zellikle ampirik antimikrobiyal ajan se\u00e7iminde yol g\u00f6sterici olacakt\u0131r. \u00c7al\u0131\u015fmam\u0131zda, toplum k\u00f6kenli \u0130YE\u2019lerde en s\u0131k kar\u015f\u0131la\u015f\u0131lan etkenler olan <i>E. coli<\/i> ve <i>Klebsiella <\/i>spp.\u2019de son 10 y\u0131l i\u00e7erisindeki siprofloksasin ve TMP-SXT diren\u00e7 oranlar\u0131n\u0131 de\u011ferlendirdik.<\/p>\n<p class=\"p2\">\u0130drar yolu infeksiyonlar\u0131n\u0131n tedavisinde antibiyotiklere s\u0131kl\u0131kla ampirik olarak ba\u015flanmaktad\u0131r. En s\u0131k kullan\u0131lan ajanlar TMP-SXT ve siprofloksasindir (1). Bu antibiyotiklerin s\u0131k kullan\u0131m\u0131n\u0131n nedeni olarak diren\u00e7 oranlar\u0131n\u0131n y\u00fcksek olmas\u0131 g\u00f6sterilmektedir. G\u00fcney Kore\u2019de yap\u0131lan bir \u00e7al\u0131\u015fmada, <i>E. coli<\/i> i\u00e7in siprofloksasin direncinin %23.4 oldu\u011fu tespit edilmi\u015ftir (12). \u00dclkemizde yap\u0131lan \u00e7al\u0131\u015fmalarda ise <i>E. coli<\/i> i\u00e7in siprofloksasin diren\u00e7 oran\u0131 %38-55 aras\u0131nda bildirilmi\u015ftir (3,13,14). Duran ve arkada\u015flar\u0131n\u0131n (5) \u00e7al\u0131\u015fmas\u0131nda, TMP-SXT\u2019nin diren\u00e7 oran\u0131n\u0131n %42.6 oldu\u011fu tespit edilmi\u015ftir. \u00c7al\u0131\u015fmam\u0131zda, 10 y\u0131ll\u0131k d\u00f6nemde <i>E. coli<\/i> i\u00e7in siprofloksasin diren\u00e7 oranlar\u0131n\u0131n %25-40, TMP-SXT diren\u00e7 oranlar\u0131n\u0131n ise %35-51 aras\u0131nda oldu\u011funu tespit ettik. <i>E. coli<\/i> i\u00e7in siprofloksasin diren\u00e7 oranlar\u0131n\u0131n y\u0131llar i\u00e7erisinde (2011-2017 y\u0131llar\u0131 aras\u0131nda) \u00f6nce azalma, sonras\u0131nda (2018-2021 y\u0131llar\u0131 aras\u0131nda) ise tekrar yava\u015f bir \u015fekilde artma e\u011filiminde oldu\u011funu saptad\u0131k. <i>E. coli<\/i> i\u00e7in TMP-SXT diren\u00e7 oranlar\u0131n\u0131n ise y\u0131llar i\u00e7erisinde azalma e\u011filiminde oldu\u011funu saptad\u0131k. Siprofloksasin diren\u00e7 oranlar\u0131n\u0131n y\u0131llar i\u00e7erisinde \u00f6nce azalma, sonra artma e\u011filiminde olmas\u0131, hekimler aras\u0131nda bu ajana kar\u015f\u0131 diren\u00e7 oranlar\u0131n\u0131n fazla oldu\u011fu kan\u0131s\u0131 nedeni ile re\u00e7ete edilmesindeki \u00e7ekinceler ile a\u00e7\u0131klanabilir.<\/p>\n<p class=\"p2\"><span class=\"s1\">Bir\u00e7ok \u00e7al\u0131\u015fma, \u0130YE\u2019lerde <i>Klebsiella <\/i>spp.\u2019nin <i>E. coli<\/i>\u2019den sonra ikinci s\u0131kl\u0131kta izole edildi\u011fini ve genel olarak antibiyotik diren\u00e7 oranlar\u0131n\u0131n <i>E. coli<\/i>\u2019ye g\u00f6re daha y\u00fcksek oldu\u011funu g\u00f6stermektedir (15). \u00dclkemizde yap\u0131lan \u00e7al\u0131\u015fmalarda, <i>Klebsiella <\/i>spp. izolatlar\u0131nda diren\u00e7 oranlar\u0131 de\u011fi\u015fkenlik g\u00f6stermekle birlikte TMP-SXT i\u00e7in %34-46 ve siprofloksasin i\u00e7in %23-40 olarak bildirilmi\u015ftir (2,10). \u00c7al\u0131\u015fmam\u0131zda son 10 y\u0131l i\u00e7erisinde <i>Klebsiella <\/i>spp. i\u00e7in siprofloksasin diren\u00e7 oranlar\u0131n\u0131n %17-43, TMP-SXT diren\u00e7 oranlar\u0131n\u0131n ise %31-43 aras\u0131nda oldu\u011funu tespit ettik. <i>Klebsiella <\/i>spp. i\u00e7in siprofloksasin diren\u00e7 oranlar\u0131n\u0131n da y\u0131llar i\u00e7erisinde (2011-2014 y\u0131llar\u0131 aras\u0131nda) \u00f6nce azalma, sonras\u0131nda (2015-2021 y\u0131llar\u0131 aras\u0131nda) ise artma e\u011filiminde oldu\u011funu; TMP-SXT diren\u00e7 oranlar\u0131n\u0131n ise y\u0131llar i\u00e7erisinde yava\u015f bir art\u0131\u015f e\u011filiminde oldu\u011funu saptad\u0131k.<\/span><\/p>\n<p class=\"p2\">\u00c7al\u0131\u015fmam\u0131zda, 10 y\u0131ll\u0131k d\u00f6nemde hem TMP-SXT hem de siprofloksasine kar\u015f\u0131 diren\u00e7 oranlar\u0131n\u0131n her iki cinsiyette de azalma e\u011filiminde oldu\u011funu saptad\u0131k. Bununla birlikte, her iki ajana kar\u015f\u0131 diren\u00e7 oranlar\u0131n\u0131n erkeklerde kad\u0131nlara g\u00f6re daha fazla oldu\u011funu tespit ettik.<\/p>\n<p class=\"p2\">Retrospektif ve tek merkezli olmas\u0131, k\u00fclt\u00fcrde etken olarak saptanan t\u00fcm mikroorganizmalar\u0131n \u00e7al\u0131\u015fmaya dahil edilememesi, dahil edilen etken mikroorganizmalarda sadece iki ajana ait diren\u00e7 oranlar\u0131n\u0131n analiz edilmesi, hastalar\u0131n klinik bulgular\u0131 ve sonlan\u0131mlar\u0131 ile ilgili herhangi bir analiz yap\u0131lmamas\u0131 \u00e7al\u0131\u015fmam\u0131z\u0131n k\u0131s\u0131tl\u0131klar\u0131yd\u0131. \u00d6rneklerin poliklinikten ve acil servisten gelen hasta \u00f6rnekleri olmas\u0131 nedeniyle \u00fcreyen bakterilerin sa\u011fl\u0131k bak\u0131m\u0131 ile ili\u015fkili infeksiyon etkenleri olup olmad\u0131\u011f\u0131 ay\u0131rt edilemedi.
