{"id":26908,"date":"2023-06-30T09:01:42","date_gmt":"2023-06-30T06:01:42","guid":{"rendered":"https:\/\/www.klimikdergisi.org\/?p=26908"},"modified":"2023-07-12T10:49:11","modified_gmt":"2023-07-12T07:49:11","slug":"saglik-hizmeti-iliskili-infeksiyonlar","status":"publish","type":"post","link":"https:\/\/www.klimikdergisi.org\/tr\/2023\/06\/30\/saglik-hizmeti-iliskili-infeksiyonlar\/","title":{"rendered":"Sa\u011fl\u0131k Hizmeti \u0130li\u015fkili \u0130nfeksiyonlar\u0131n Etken, T\u00fcr ve Diren\u00e7 Profillerinin De\u011ferlendirilmesi"},"content":{"rendered":"<h2 class=\"p1\">G\u0130R\u0130\u015e<\/h2>\n<p class=\"p2\">Sa\u011fl\u0131k hizmeti ili\u015fkili (SH\u0130) infeksiyonlar, hastalar\u0131n yat\u0131\u015flar\u0131 esnas\u0131nda ink\u00fcbasyon d\u00f6neminde olmay\u0131p hastaneye yatt\u0131ktan sonra ortaya \u00e7\u0131kan infeksiyonlard\u0131r (1); hastaneye yat\u0131\u015ftan 48 saat sonra ve hastaneden taburcu olduktan sonraki 3 g\u00fcnde ortaya \u00e7\u0131kan t\u00fcm infeksiyonlar\u0131 i\u00e7ermektedir (2). Ancak bu tan\u0131m infeksiyonun t\u00fcr\u00fcne g\u00f6re de\u011fi\u015febilmektedir. \u00d6rne\u011fin cerrahi alan infeksiyonlar\u0131nda, i\u015flem t\u00fcr\u00fcne g\u00f6re 30-90 g\u00fcn i\u00e7inde geli\u015fen infeksiyonlar da SH\u0130 olarak de\u011ferlendirilir (3). Nozokomiyal infeksiyon olarak da isimlendirilen SH\u0130 infeksiyonlar, yatan hastalarda klinik sonu\u00e7lar\u0131 etkilemekte ve d\u00fcnya \u00e7ap\u0131nda ciddi bir endi\u015feye yol a\u00e7maktad\u0131r (4). Bu infeksiyonlar, \u00f6zellikle yo\u011fun bak\u0131m \u00fcnitesi (YB\u00dc)\u2019nde yayg\u0131n bir \u015fekilde g\u00f6r\u00fclmekte, hastanede yat\u0131\u015f s\u00fcrelerini uzatmakta, \u00e7oklu organ yetmezli\u011fine ve mortalitede art\u0131\u015fa yol a\u00e7maktad\u0131r (5). Sa\u011fl\u0131k hizmeti ili\u015fkili infeksiyon oranlar\u0131 \u00fclkelere, b\u00f6lgelere ve hastanelere g\u00f6re farkl\u0131l\u0131k g\u00f6sterebilmektedir. Hastaneler taraf\u0131ndan olu\u015fturulan infeksiyon kontrol komiteleri, infeksiyon oranlar\u0131n\u0131 izlemekte ve analiz etmektedir. B\u00f6ylece elde edilen veriler do\u011frultusunda hastanelerin kendilerine \u00f6zg\u00fc infeksiyon oranlar\u0131 ve y\u00fcksek riskli servisler belirlenmekte, al\u0131nacak \u00f6nlemler ve e\u011fitim programlar\u0131yla hastanenin \u00f6zellikleri ve gereksinimlerine g\u00f6re yeniden d\u00fczenlenmeler yap\u0131lmaktad\u0131r (1). Sa\u011fl\u0131k hizmeti ili\u015fkili infeksiyonlarda izole edilen etkenlerin bir\u00e7ok antibiyoti\u011fe diren\u00e7li olmalar\u0131ndan dolay\u0131 tedavilerinde zorluklar ya\u015fanmaktad\u0131r (6).<\/p>\n<p class=\"p4\">Bu \u00e7al\u0131\u015fmada, 2016-2020 y\u0131llar\u0131 aras\u0131nda \u00e7e\u015fitli kliniklerde yat\u0131r\u0131larak takip edilen ve SH\u0130 infeksiyonu tan\u0131s\u0131 alan hastalardaki infeksiyon t\u00fcrlerinin ve etkenlerinin yan\u0131 s\u0131ra diren\u00e7 profillerinin irdelenmesi ama\u00e7land\u0131.<\/p>\n<h2 class=\"p1\">Y\u00d6NTEMLER<\/h2>\n<h3 class=\"p6\">\u00c7al\u0131\u015fma Protokol\u00fc<\/h3>\n<p class=\"p7\">\u00c7al\u0131\u015fma kapsam\u0131nda, Ocak 2016-Aral\u0131k 2020 tarihleri aras\u0131nda SB\u00dc Van E\u011fitim ve Ara\u015ft\u0131rma Hastanesi\u2019nde yat\u0131r\u0131larak takip edilen, \u0130nfeksiyon Hastal\u0131klar\u0131 ve Klinik Mikrobiyoloji B\u00f6l\u00fcm\u00fc taraf\u0131ndan ABD Hastal\u0131k Kontrol ve Korunma Merkezleri (Centers for Disease Control and Prevention-CDC) kriterlerine g\u00f6re SH\u0130 infeksiyon tan\u0131s\u0131 konan hastalar \u00e7al\u0131\u015fmaya dahil edildi. Hastane, 1161\u2019i eri\u015fkin hasta servislerine ve 339\u2019u \u00e7ocuk servislerine ait olmak \u00fczere 1515 yatakl\u0131 olup yo\u011fun bak\u0131m \u00fcniteleri, \u00e7ocuk, cerrahi ve dahili servislerden olu\u015fmaktad\u0131r. Yirmi dokuz yatakl\u0131 Anestezi ve Reanimasyon YB\u00dc, sekiz yatakl\u0131 kalp damar cerrahisi YB\u00dc, 20 yatakl\u0131 koroner YB\u00dc, sekiz yatakl\u0131 cerrahi YB\u00dc, sekiz yatakl\u0131 dahili YB\u00dc, 104 yatakl\u0131 \u00e7ocuk YB\u00dc olmak \u00fczere toplam alt\u0131 YB\u00dc bulunmaktad\u0131r. <span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p class=\"p2\">\u00c7ocuk servisleri \u00e7al\u0131\u015fmaya dahil edilmedi ve SH\u0130 infeksiyon geli\u015fti\u011fi d\u00fc\u015f\u00fcn\u00fclen t\u00fcm yeti\u015fkin hastalardan, kan k\u00fclt\u00fcrleri ve infeksiyon oda\u011f\u0131 oldu\u011fu d\u00fc\u015f\u00fcn\u00fclen idrar, balgam, beyin-omurilik s\u0131v\u0131s\u0131n\u0131n yan\u0131 s\u0131ra bo\u011faz, trakea, yara yeri ve kateterden \u00f6rnekler al\u0131nd\u0131. Sa\u011fl\u0131k Bilimleri \u00dcniversitesi Van E\u011fitim ve Ara\u015ft\u0131rma Hastanesi Mikrobiyoloji Laboratuvar\u0131\u2019nda yap\u0131lan k\u00fclt\u00fcr sonucunda \u00fcreme saptanan b\u00fct\u00fcn mikroorganizmalar SH\u0130 infeksiyonlar a\u00e7\u0131s\u0131ndan de\u011ferlendirildi. Hastanede SH\u0130 infeksiyonlar\u0131n s\u00fcrveyans\u0131, infeksiyon kontrol hekimleri ve hem\u015fireleri taraf\u0131ndan \u201cUlusal Sa\u011fl\u0131k Hizmeti \u0130li\u015fkili Enfeksiyonlar S\u00fcrveyans Standartlar\u0131\u201d kapsam\u0131nda y\u00fcr\u00fct\u00fclmektedir (3). Sa\u011fl\u0131k hizmeti ili\u015fkili infeksiyonlara neden olan mikroorganizmalar ve bunlar\u0131n diren\u00e7 oranlar\u0131 k\u00fclt\u00fcre dayal\u0131 s\u00fcrveyans y\u00f6ntemi ile retrospektif olarak de\u011ferlendirildi. Hastalara ait t\u00fcm veriler Ulusal Sa\u011fl\u0131k Hizmeti \u0130li\u015fkili Enfeksiyonlar S\u00fcrveyans A\u011f\u0131 (USH\u0130ESA) bilgi sisteminden elde edildi. Sa\u011fl\u0131k hizmeti ili\u015fkili dansite ve infeksiyon h\u0131zlar\u0131 a\u015fa\u011f\u0131da verilen form\u00fcllerle hesapland\u0131: <span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p class=\"p2\" style=\"padding-left: 40px;\">SH\u0130 infeksiyon dansitesi = (SH\u0130 infeksiyon say\u0131s\u0131\/ Hasta g\u00fcn\u00fc) x1000<span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p class=\"p2\" style=\"padding-left: 40px;\">SH\u0130 infeksiyon h\u0131z\u0131 = (SH\u0130 infeksiyon say\u0131s\u0131 \/ Hasta say\u0131s\u0131) x 100<\/p>\n<h3 class=\"p6\">Klinik \u0130zolatlar\u0131n Mikrobiyolojik Tan\u0131mlamas\u0131<\/h3>\n<p class=\"p7\">K\u00fclt\u00fcr\u00fc yap\u0131lmak \u00fczere laboratuvara g\u00f6nderilen klinik \u00f6rneklerde \u00fcreyen mikroorganizmalar\u0131n tan\u0131mlanmas\u0131, konvansiyonel mikrobiyolojik y\u00f6ntemler ve VITEK<span class=\"s1\">\u00ae<\/span> 2 (bioM\u00e9rieux, Marcy l\u2019Etoile, Fransa) otomatize bakteri tan\u0131mlama sistemi kullan\u0131larak yap\u0131ld\u0131. \u00dcretici firma \u00f6nerilerine uygun olarak tan\u0131mlanan su\u015flar\u0131n antibiyotik duyarl\u0131l\u0131klar\u0131 ve fenotipik diren\u00e7 profilleri mikrodil\u00fcsyon y\u00f6ntemi ile belirlendi. Sa\u011fl\u0131k hizmeti ili\u015fkili infeksiyon olarak tan\u0131mlanan hastalardan <i>Enterococcus <\/i>t\u00fcrlerinde vankomisin direnci y\u0131llara g\u00f6re azal\u0131rken, <i>S. aureus <\/i>su\u015flar\u0131nda metisilin direncinde art\u0131\u015f saptand\u0131 (Tablo 8).<b><span class=\"Apple-converted-space\">\u00a0<\/span><\/b><\/p>\n<p class=\"p2\">Sa\u011fl\u0131k hizmeti ili\u015fkili infeksiyon geli\u015fen hastalar\u0131n yakla\u015f\u0131k %98.8\u2019inde infeksiyon olu\u015fumunu kolayla\u015ft\u0131r\u0131c\u0131 bir veya daha fazla say\u0131da invazif giri\u015fim (\u00fcriner kateter, mekanik ventilasyon, ven\u00f6z katater, santral katater vs.) yap\u0131ld\u0131\u011f\u0131 saptand\u0131<span class=\"s2\">.<span class=\"Apple-converted-space\">\u00a0<\/span><\/span><\/p>\n<h2 class=\"p1\">\u0130RDELEME<\/h2>\n<p class=\"p2\">Hastalar\u0131n modern tedavi imkanlar\u0131na kavu\u015fmas\u0131, invazif giri\u015fimlerin artmas\u0131 ve ya\u015fam s\u00fcresinin uzamas\u0131yla birlikte SH\u0130 infeksiyonlarda art\u0131\u015f g\u00f6r\u00fclmesi bu infeksiyonlar\u0131 kontrol ve \u00f6nleme y\u00f6ntemlerini daha da \u00f6nemli k\u0131lmaktad\u0131r (1,7). S\u00f6z konusu infeksiyonlar morbidite, mortalite, hastanede kal\u0131\u015f s\u00fcresi ve sa\u011fl\u0131k bak\u0131m maliyetlerinde art\u0131\u015f ile ili\u015fkilidir (8). Yo\u011fun bak\u0131m \u00fcnitelerindeki SH\u0130 infeksiyon etkenlerinin antibiyotik duyarl\u0131l\u0131klar\u0131n\u0131 belirlemek ampirik tedavi yakla\u015f\u0131m\u0131 a\u00e7\u0131s\u0131ndan son derece \u00f6nemlidir (9).<span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p class=\"p2\">Yo\u011fun bak\u0131m \u00fcnitelerinde yatan hastalarda, SH\u0130 infeksiyon riskinin hastanenin di\u011fer b\u00f6l\u00fcmlerinde yatan hastalara g\u00f6re daha fazla oldu\u011fu ve SH\u0130 infeksiyonlar\u0131n bu \u00fcnitelerde 5-10 kat daha fazla g\u00f6r\u00fcld\u00fc\u011f\u00fc belirtilmektedir. Van ilinde yap\u0131lan bir \u00e7al\u0131\u015fmada, genel SH\u0130 infeksiyon oran\u0131 %3.5 oran\u0131nda bulurken Anesteziyoloji ve Reanimasyon YB\u00dc\u2019de bu oran %18.3 olarak saptanm\u0131\u015ft\u0131r; s\u00f6z konusu oran hastanedeki di\u011fer yo\u011fun bak\u0131m birimlerinden ve di\u011fer b\u00fct\u00fcn servislerden olduk\u00e7a y\u00fcksektir (1). \u00c7in\u2019de yap\u0131lan bir \u00e7al\u0131\u015fmada, SH\u0130 infeksiyon tan\u0131s\u0131 alan hastalar\u0131n %54\u2019\u00fcn\u00fcn yo\u011fun bak\u0131m \u00fcnitesinde takip edildi\u011fi g\u00f6r\u00fclm\u00fc\u015ft\u00fcr (4). \u00c7al\u0131\u015fmam\u0131zda da literat\u00fcre benzer \u015fekilde SH\u0130 infeksiyon tan\u0131s\u0131 alan hastalar\u0131n %53.3\u2019\u00fcn\u00fcn Anesteziyoloji ve Reanimasyon YB\u00dc\u2019de takip edildi\u011fi g\u00f6r\u00fcld\u00fc. Yo\u011fun bak\u0131m \u00fcnitelerinde invazif i\u015flemlerin yo\u011fun bir \u015fekilde uygulanmas\u0131n\u0131n yan\u0131 s\u0131ra geni\u015f spektrumlu ve uzun s\u00fcreli antibiyoterapinin kullan\u0131lmas\u0131 nedeniyle SH\u0130 infeksiyonlar \u00f6nemli bir sa\u011fl\u0131k sorunu olmaya devam etmektedir. Etken spesifik antibiyoterapinin uygun s\u00fcrede verilmesi, infeksiyon kontrol komitesince yo\u011fun bak\u0131m s\u00fcrveyans\u0131n\u0131n yap\u0131lmas\u0131 ve endikasyonun ortadan kalkmas\u0131yla birlikte invazif ara\u00e7 kullan\u0131m\u0131n\u0131n sonland\u0131r\u0131lmas\u0131 SH\u0130 infeksiyonlar\u0131 azaltmaya y\u00f6nelik \u00f6nemli \u00f6nlemler olarak \u00f6ne \u00e7\u0131kmaktad\u0131r.<\/p>\n<p class=\"p2\">\u00dclkemizde yap\u0131lan \u00e7al\u0131\u015fmalarda, SH\u0130 infeksiyon h\u0131z\u0131 b\u00f6l\u00fcmlere g\u00f6re de\u011fi\u015fiklik g\u00f6stermekle birlikte YB\u00dc\u2019lerde %3.8-27.6 aras\u0131nda de\u011fi\u015fmektedir (10-13). Avrupa\u2019da yap\u0131lan EPIC (The European Prevalence of Infections in Intensive Care) \u00e7al\u0131\u015fmas\u0131nda, SH\u0130 infeksiyon h\u0131z\u0131 %20.6 olarak bildirilmi\u015ftir (14). \u0130ran\u2019da yap\u0131lan bir meta-analizde, SH\u0130 infeksiyon prevalans\u0131 %4.5 olarak tespit edilmi\u015ftir (15). \u00c7al\u0131\u015fmam\u0131zda, SH\u0130 infeksiyon h\u0131zlar\u0131 2016 y\u0131l\u0131nda %0.60, 2017 y\u0131l\u0131nda %0.27 ve 2018 y\u0131l\u0131nda %0.20, 2019 y\u0131l\u0131nda %0.89, 2020 y\u0131l\u0131nda %3.77 olmak \u00fczere ortalama %0.42 oran\u0131nda saptand\u0131; bulunan bu sonu\u00e7 literat\u00fcre g\u00f6re d\u00fc\u015f\u00fckt\u00fc (10-13,15). Bunun sebebi daha \u00f6nce yap\u0131lan \u00e7al\u0131\u015fmalarda saptanan h\u0131zlar\u0131n \u00f6zellikle yo\u011fun bak\u0131m \u00fcnitelerindeki SH\u0130 infeksiyon h\u0131zlar\u0131 olmas\u0131d\u0131r; \u00e7al\u0131\u015fmam\u0131zda ise hastanede yatan t\u00fcm hastalar \u00e7al\u0131\u015fmaya dahil edildi. Y\u0131llara g\u00f6re yo\u011fun bak\u0131m \u00fcnitesindeki infeksiyon h\u0131zlar\u0131 de\u011ferlendirildi\u011finde, \u00e7al\u0131\u015fmam\u0131z\u0131n di\u011fer \u00e7al\u0131\u015fmalarla uyumlu oldu\u011fu g\u00f6r\u00fcld\u00fc. Sa\u011fl\u0131k hizmeti ili\u015fkili infeksiyon h\u0131zlar\u0131na bak\u0131ld\u0131\u011f\u0131nda 2016-2018 y\u0131llar\u0131 aras\u0131nda azalma g\u00f6r\u00fcl\u00fcrken, 2018 y\u0131l\u0131ndan itibaren tekrar art\u0131\u015f tespit edildi.<span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p class=\"p2\">\u0130nfeksiyon etkenlerinin t\u00fcr da\u011f\u0131l\u0131m\u0131, infeksiyonun geli\u015fti\u011fi sisteme g\u00f6re farkl\u0131l\u0131k g\u00f6stermektedir. Kan dola\u015f\u0131m\u0131 infeksiyonlar\u0131nda genel olarak Gram-pozitif bakteriler izole edilirken, \u00fcriner sistem infeksiyonu ve pn\u00f6monide etken olarak Gram-negatif bakteriler daha s\u0131k kar\u015f\u0131m\u0131za \u00e7\u0131kmaktad\u0131r (16). \u00dclkemizde yap\u0131lan bir \u00e7al\u0131\u015fmada Gram-negatif bakteriler %59, Gram-pozitif bakteriler %32 ve mayalar %8.66 oran\u0131nda saptanm\u0131\u015ft\u0131r (17). B\u00fct\u00fcn izole edilen mikroorganizmalar aras\u0131nda en s\u0131k olarak <i>Acinetobacter<\/i> t\u00fcrleri (%29.9) saptanm\u0131\u015f olup bunu <i>P. aeruginosa<\/i> (%19.69) ve mayalar (%8.66) izlemi\u015ftir (18). EPIC I, II ve III, k\u00fcresel \u00f6l\u00e7ekte SH\u0130 infeksiyonlar\u0131n yo\u011fun bak\u0131mdaki seyrini anlamak i\u00e7in \u00f6nemli bir bak\u0131\u015f a\u00e7\u0131s\u0131 kazand\u0131ran ilk \u00e7al\u0131\u015fmalard\u0131r. S\u00f6z konusu \u00e7al\u0131\u015fmalardan EPIC III, 2017 y\u0131l\u0131nda ger\u00e7ekle\u015ftirilmi\u015f olup izolatlar\u0131n %67\u2019si Gram-negatif bakteri, %37\u2019si Gram-pozitif bakteri ve %16\u2019s\u0131 mantar olarak tespit edilmi\u015ftir. En s\u0131k izole edilen Gram negatif bakteriler; <i>Enterobacteriaceae<\/i> ailesi bakterileri (%26), <i>Pseudomonas <\/i>t\u00fcrleri (%16) ve <i>Acinetobacter<\/i> t\u00fcrleri (%11) olmu\u015ftur (14,19,20). \u00c7al\u0131\u015fmam\u0131zda, SH\u0130 infeksiyon etkenleri olarak Gram-pozitif, Gram-negatif ve<i> Candida <\/i>t\u00fcrlerinin y\u0131llara g\u00f6re da\u011f\u0131l\u0131m\u0131na bak\u0131ld\u0131\u011f\u0131nda, t\u00fcm y\u0131llarda en s\u0131k g\u00f6r\u00fclen etiyolojik grup Gram-negatif bakteriler olurken (%72.8-91.7), Gram-pozitif bakteriler %4.8-17.5 aral\u0131\u011f\u0131nda ve <i>Candida <\/i>t\u00fcrleri %0-9.6 aral\u0131\u011f\u0131nda saptand\u0131. Gram-negatif bakteriler ve \u00f6zellikle bu grupta <i>Acinetobacter <\/i>t\u00fcrlerinin s\u0131kl\u0131kla izole edilmesi endi\u015fe vericidir. Tedavi se\u00e7eneklerinin k\u0131s\u0131tl\u0131 olmas\u0131, bakteri-ila\u00e7 denkleminde diren\u00e7 sorununun s\u00fcrekli kar\u015f\u0131m\u0131za \u00e7\u0131kmas\u0131 ve yeni ila\u00e7 geli\u015ftirmenin kolay olmamas\u0131 infeksiyon kontrol politikalar\u0131n\u0131n \u00f6nemini daha da art\u0131rmaktad\u0131r.<\/p>\n<p class=\"p2\">Yo\u011fun bak\u0131m \u00fcnitesi, pediatri servisleri veya yo\u011fun bak\u0131m \u00fcnitesi d\u0131\u015f\u0131ndaki servislerde s\u0131k kar\u015f\u0131la\u015f\u0131lan SH\u0130 infeksiyon t\u00fcrleri aras\u0131nda farkl\u0131l\u0131klar g\u00f6r\u00fclmektedir (13). Vincent ve arkada\u015flar\u0131 (14) taraf\u0131ndan yay\u0131mlanan EPIC I \u00e7al\u0131\u015fmas\u0131nda, YB\u00dc\u2019lerde en s\u0131k g\u00f6r\u00fclen infeksiyon pn\u00f6moni (%46.9) olup bunu \u00fcriner sistem infeksiyonu (%17.6), kan dola\u015f\u0131m\u0131 infeksiyonu (%12) ve cerrahi alan infeksiyonu (%6.9) takip etmektedir. \u0130ran\u2019da, 1995-2020 y\u0131llar\u0131 aras\u0131nda hastane k\u00f6kenli infeksiyonlara y\u00f6nelik yap\u0131lan bir meta-analiz \u00e7al\u0131\u015fmas\u0131nda, en s\u0131k <i>E. coli<\/i> ve <i>Klebsiella <\/i>t\u00fcrleri izole edilmi\u015f olup solunum yolu ve \u00fcriner sistem infeksiyonlar\u0131 en s\u0131k kar\u015f\u0131la\u015f\u0131lan SH\u0130 infeksiyonlar olarak saptanm\u0131\u015ft\u0131r (21). Anesteziyoloji ve Reanimasyon YB\u00dc&#8217;lerinde, 10 y\u0131ll\u0131k periyotta geli\u015fen SH\u0130 infeksiyonlara y\u00f6nelik yap\u0131lan bir \u00e7al\u0131\u015fmada; %18.4 oran\u0131nda SH\u0130 infeksiyon geli\u015fti\u011fi, en s\u0131k kan dola\u015f\u0131m\u0131 infeksiyonunun (%38.5) g\u00f6r\u00fcld\u00fc\u011f\u00fc ve bunu s\u0131ras\u0131yla pn\u00f6moninin (%24), ventilat\u00f6r ili\u015fkili pn\u00f6moninin (%14.7), \u00fcriner sistem infeksiyonunun (%13.8), cerrahi alan infeksiyonunun (%6) ve kateter infeksiyonunun (%3.3) izledi\u011fi bildirilmi\u015ftir (11). \u00c7al\u0131\u015fmam\u0131zda, geli\u015fen SH\u0130 infeksiyon t\u00fcrlerine bak\u0131ld\u0131\u011f\u0131nda di\u011fer infeksiyon t\u00fcrleri y\u0131llara g\u00f6re de\u011fi\u015fkenlik g\u00f6stermekle birlikte kan dola\u015f\u0131m\u0131 infeksiyonlar\u0131 literat\u00fcrden farkl\u0131 olarak en s\u0131k saptanan SH\u0130 infeksiyon t\u00fcr\u00fc oldu. Yap\u0131lan her t\u00fcrl\u00fc invazif giri\u015fim ba\u015fta kan dola\u015f\u0131m\u0131 infeksiyonlar\u0131 olmak \u00fczere geli\u015febilecek di\u011fer infeksiyonlar a\u00e7\u0131s\u0131ndan risk olu\u015fturmaktad\u0131r. Bu nedenle uzun s\u00fcreli ve gereksiz kateterizasyondan ka\u00e7\u0131n\u0131lmas\u0131, kateterizasyon s\u0131ras\u0131nda maksimum bariyer \u00f6nlemlerine uyulmas\u0131, kateter endikasyonlar\u0131n\u0131n g\u00fcnl\u00fck olarak de\u011ferlendirilmesi ve ilgili birim \u00e7al\u0131\u015fanlar\u0131na aral\u0131klarla e\u011fitim verilmesi hastane infeksiyonlar\u0131n\u0131 azaltmaya yard\u0131mc\u0131 olacakt\u0131r.<\/p>\n<p class=\"p2\">Son 10 y\u0131lda, antimikrobiyal diren\u00e7 s\u0131kl\u0131\u011f\u0131 k\u00fcresel olarak artm\u0131\u015ft\u0131r. G\u00fcn\u00fcm\u00fczde \u00e7oklu ila\u00e7 diren\u00e7li (multidrug resistance- MDR) bakteriler SH\u0130 infeksiyonlar\u0131n \u00f6nemli bir nedeni olarak kabul edilmekte ve hem sa\u011fl\u0131k \u00e7al\u0131\u015fanlar\u0131 hem de hastalar i\u00e7in ciddi bir endi\u015feye yol a\u00e7maktad\u0131r (4). Hindistan\u2019da yap\u0131lan bir \u00e7al\u0131\u015fmada, SH\u0130 infeksiyonu etkeni olarak en s\u0131k <i>Acinetobacter <\/i>t\u00fcrleri saptanm\u0131\u015ft\u0131r. <i>Acinetobacter <\/i>spp. su\u015flar\u0131n\u0131n %83.5\u2019i meropeneme diren\u00e7li iken seftazidim direnci %90.6 ve sefoperazon\/sulbaktam direnci %25.5 oran\u0131nda bildirilmi\u015ftir. Hatta aminoglikozidlere ve kinolonlara kar\u015f\u0131 diren\u00e7 \u00e7ok daha y\u00fcksek saptanm\u0131\u015f (%84.5 amikasine ve %86.5 siprofloksasine kar\u015f\u0131) olup MDR-<i>Acinetobacter <\/i>spp. %82.1, MDR-<i>Pseudomonas<\/i> spp. %56.9, MDR-<i>E. coli<\/i> %26.3 ve MDR-<i>Klebsiella <\/i>spp. %52.4 oranlar\u0131nda tespit edilmi\u015ftir. Gram-pozitif grupta, <i>S. aureus<\/i>\u2019da penisiline kar\u015f\u0131 y\u00fcksek derecede diren\u00e7 (%86.5) g\u00f6r\u00fcl\u00fcrken bunu siprofloksasin direnci (%69) izlemi\u015ftir. S\u00f6z konusu \u00e7al\u0131\u015fmada, enterokok t\u00fcrlerinin %11.9\u2019unda (34\/286) vankomisin diren\u00e7li enterokoklar (VRE) saptanm\u0131\u015ft\u0131r (22). \u00c7in\u2019de yap\u0131lan bir \u00e7al\u0131\u015fmada, izole edilen su\u015flar\u0131n diren\u00e7 bak\u0131m\u0131ndan de\u011ferlendirmesinde; metisiline diren\u00e7li <i>S. aureus <\/i>(MRSA) pozitifli\u011fi %28.6, MDR-<i>P. aeruginosa<\/i> %37.6, MDR-<i>Acinetobacter<\/i> spp. %50.7, GSBL-<i>E. coli <\/i>pozitifli\u011fi %55.3 ve karbapenem direnci %0.1, GSBL-<i>K. pneumoniae<\/i> pozitifli\u011fi %38.9 ve karbapenem direnci %3, <i>Enterococcus faecium<\/i> ve <i>Enterococcus<span class=\"Apple-converted-space\">\u00a0 <\/span>faecalis<\/i>\u2019te vankomisin direnci s\u0131ras\u0131yla %1.8 ve %1.3 olup total MDR pozitifli\u011fi %42.5 olarak saptanm\u0131\u015ft\u0131r (4). \u00dclkemizde yap\u0131lan bir \u00e7al\u0131\u015fmada, metisilin direncinin KNS\u2019de %47 oldu\u011fu tespit edilmi\u015ftir. S\u00f6z konusu \u00e7al\u0131\u015fmada, <i>E. coli <\/i>su\u015flar\u0131n\u0131n %40\u2019\u0131nda, <i>K. pneumoniae<\/i>\u2019nin %60\u2019\u0131nda GSBL pozitifli\u011fi bulunmu\u015ftur. <i>A. baumannii <\/i>su\u015flar\u0131n\u0131n %48.65\u2019inde <i>P. aeruginosa\u2019<\/i>n\u0131n %40\u2019\u0131nda ve <i>E. coli <\/i>su\u015flar\u0131n\u0131n %2\u2019sinde karbapenem direnci saptanm\u0131\u015ft\u0131r (12). \u00c7al\u0131\u015fmam\u0131zda, Gram-negatif bakterilerden <i>Acinetobacter <\/i>t\u00fcrlerinde GSBL g\u00f6r\u00fclme oran\u0131 (%0-66.7 aral\u0131\u011f\u0131nda) y\u0131llar i\u00e7inde de\u011fi\u015fkenlik g\u00f6sterirken karbapenem direncinde (%88.5-93.8 aral\u0131\u011f\u0131nda) art\u0131\u015f g\u00f6r\u00fcld\u00fc. <i>Acinetobacter <\/i>t\u00fcrlerinde kolistin duyarl\u0131l\u0131\u011f\u0131nda belirgin farkl\u0131l\u0131k g\u00f6zlenmedi. <i>E. coli<\/i>\u2019 de GSBL oranlar\u0131nda %68-69.5 aral\u0131\u011f\u0131 ile y\u0131llara g\u00f6re farkl\u0131l\u0131k saptanmad\u0131. <i>K. pneumoniae<\/i>\u2019de karbapenem direncinde %31.2-62.5 aral\u0131\u011f\u0131ndaki oranlarla art\u0131\u015f saptand\u0131. <i>Pseudomanas <\/i>t\u00fcrlerinde GSBL oranlar\u0131nda azalma (%0-100) saptan\u0131rken karbapenem direncinde (%18.7-60 aral\u0131\u011f\u0131nda) 2017 ve 2018\u2019de art\u0131\u015f olurken di\u011fer y\u0131llarda ayn\u0131 seviyede kald\u0131\u011f\u0131 g\u00f6r\u00fcld\u00fc. Gram-pozitif bakterilere antibiyotik direnci a\u00e7\u0131s\u0131ndan bak\u0131ld\u0131\u011f\u0131nda <i>Enterococcus <\/i>t\u00fcrlerinde vankomisin direnci %0-29 aral\u0131\u011f\u0131yla y\u0131llara g\u00f6re azal\u0131rken, <i>S. aureus <\/i>su\u015flar\u0131nda metisilin direncinde %10-66.6 aral\u0131\u011f\u0131yla art\u0131\u015f saptand\u0131.<b> <\/b>Koag\u00fclaz negatif stafilokoklarda metisilin direnci saptanmad\u0131.<\/p>\n<p class=\"p2\">Sa\u011fl\u0131k hizmeti ili\u015fkili infeksiyonlar etiyolojik etkenlerde g\u00f6r\u00fclen antibiyotik direnci nedeniyle hem d\u00fcnyada hem de \u00fclkemizde \u00f6nemli bir sa\u011fl\u0131k sorunu olmaya devam etmektedir. Sa\u011fl\u0131k hizmeti ili\u015fkili infeksiyon t\u00fcrlerinin, etken mikroorganizmalar\u0131n ve diren\u00e7 paternlerinin infeksiyon kontrol komiteleri taraf\u0131ndan s\u00fcrveyans ile belirlenmesi hastane genelinde ve \u00f6zellikle yo\u011fun bak\u0131m \u00fcnitelerinde klinisyenlerin yakla\u015f\u0131m\u0131n\u0131 son derece etkilemektedir. \u00c7al\u0131\u015fmam\u0131zda, SH\u0130 infeksiyon h\u0131z\u0131n\u0131n literat\u00fcre g\u00f6re d\u00fc\u015f\u00fck oldu\u011fu g\u00f6r\u00fclmekle beraber ba\u015fta karbapenem direnci olmak \u00fczere ila\u00e7 direncinin hemen hemen t\u00fcm ajanlarda art\u0131\u015f g\u00f6sterdi\u011fi,<span class=\"Apple-converted-space\">\u00a0 <\/span><i>Acinetobacter<\/i> t\u00fcrlerinin de \u00f6nemli ve sorun yaratan ajanlardan biri olmaya devam etti\u011fi g\u00f6r\u00fclmektedir.
