Bu nedenle diren\u00e7li bakterilerle geli\u015fen infeksiyonlar\u0131n tedavisi i\u00e7in eski antibiyotiklerin yeniden kullan\u0131m\u0131na ba\u015flanm\u0131\u015ft\u0131r. Kolistin bu antibiyotiklerden biri olup \u00e7oklu antibiyotik direnci olan <i>Pseudomonas aeruginosa<\/i>, <i>Acinetobacter baumannii<\/i>, <i>Klebsiella pneumoniae <\/i>ve <i>Escherichia<\/i> <i>coli<\/i> bakterilerinin neden oldu\u011fu infeksiyonlar\u0131n tedavisinde son se\u00e7enek olarak kullan\u0131lmaktad\u0131r (3, 4).<\/p>\n<p class=\"p2\"><span class=\"s1\">Son y\u0131llarda hastane k\u00f6kenli infeksiyonlarda s\u0131k kullan\u0131lmas\u0131 nedeniyle tedaviler ba\u015far\u0131l\u0131 olsa bile diren\u00e7 geli\u015fme riski g\u00f6z \u00f6n\u00fcnde tutularak kolistin duyarl\u0131l\u0131\u011f\u0131 takip edilmelidir. Klinik mikrobiyoloji laboratuvarlar\u0131n\u0131n, karbapenem diren\u00e7li Gram-negatif bakterilerde kolistin direncini tan\u0131mlayabilmesi \u00f6nemlidir. Ancak bu \u015fekilde, kolistin direnci belirlenen bakteri ile infekte olan hastalarda ciddi yan etkisi olan bu antibiyoti\u011fin gereksiz yere kullan\u0131m\u0131n\u0131n \u00f6n\u00fcne ge\u00e7ilmi\u015f olur. Son y\u0131llarda, t\u00fcm d\u00fcnyada oldu\u011fu gibi \u00fclkemizde de kolistin diren\u00e7li bakteriler raporlanm\u0131\u015ft\u0131r (5). Bu nedenle, kolistin duyarl\u0131l\u0131k testinin \u201cClinical and Laboratory Standards Institute (CLSI)\u201d ve \u201cEuropean Committee on Antimicrobial Susceptibility Testing (EUCAST)\u201d taraf\u0131ndan \u00f6nerilen s\u0131v\u0131 mikrodil\u00fcsyon y\u00f6ntemi ile \u00e7al\u0131\u015f\u0131larak do\u011fru sonucun raporlanmas\u0131 \u00f6nemlidir (6). Ancak s\u0131v\u0131 mikrodil\u00fcsyon y\u00f6nteminin pratik olmamas\u0131ndan dolay\u0131 rutin bakteriyoloji laboratuvarlar\u0131nda kolistin direncini belirlemek her zaman problem olmu\u015ftur. Bu nedenle pratik, basit, h\u0131zl\u0131 ve etkin bir y\u00f6nteme ihtiya\u00e7 vard\u0131r.<\/span><span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p class=\"p2\"><span class=\"s1\">Bu \u00e7al\u0131\u015fmada, Gram-negatif bakterilerde kolistin duyarl\u0131l\u0131\u011f\u0131n\u0131n belirlenmesinde daha \u00f6nceden tan\u0131mlanan basit ve pratik bir y\u00f6ntem olan kolistin s\u0131v\u0131 disk el\u00fcsyon (KSDE) testinin de\u011ferlendirilmesi ama\u00e7land\u0131.<\/span><\/p>\n<h2 class=\"p2\"><span class=\"s2\">Y\u00d6NTEMLER<\/span><\/h2>\n<h3 class=\"p3\">Bakteri \u0130zolatlar\u0131<\/h3>\n<p class=\"p2\">\u00c7al\u0131\u015fmam\u0131za 2018-2020 y\u0131llar\u0131 aras\u0131nda \u00e7e\u015fitli klinik \u00f6rneklerden izole etti\u011fimiz toplam 193 Gram-negatif bakteri dahil edildi. Bu izolatlar <i>K. pneumoniae<\/i> (59), <i>E. coli<\/i> (20), <i>E. cloacae<\/i> (18), <i>A. baumannii<\/i> (51) ve <i>P. aeruginosa<\/i> (45)\u2019dan olu\u015fmaktad\u0131r.<span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<h3 class=\"p3\">S\u0131v\u0131 Mikrodil\u00fcsyon Referans Y\u00f6ntemi<\/h3>\n<p class=\"p2\">Kolistin s\u00fclfat (Sigma-Aldrich, Almanya) ve 32 ile 0.064 mg\/lt konsantrasyonlar aras\u0131nda, katyon ayarl\u0131 Mueller-Hinton buyyon (MHB) kullan\u0131larak \u00e7ift kat dil\u00fcsyonlar \u015feklinde haz\u0131rlanan 96 kuyucuklu mikroplaklarda CLSI\/EUCAST \u00f6nerilerine g\u00f6re \u00e7al\u0131\u015f\u0131ld\u0131 (7, 8). Kalite kontrol\u00fc; negatif kontrol olarak <i>E. coli<\/i> 25922 ve pozitif kontrol olarak mcr-1 \u00fcreten <i>E. coli<\/i> su\u015fu (NCTC 13846) kullan\u0131larak yap\u0131ld\u0131.<\/p>\n<h3 class=\"p3\">Kolistin S\u0131v\u0131 Disk El\u00fcsyon (KSDE) Testi<\/h3>\n<p class=\"p2\">KSDE testi, Simner ve arkada\u015flar\u0131n\u0131n (9) tan\u0131mlad\u0131\u011f\u0131 metoda uygun olarak \u00e7al\u0131\u015f\u0131ld\u0131. Her bir bakteri izolat\u0131 i\u00e7in 10 ml katyon ayarl\u0131 MHB i\u00e7eren 5 t\u00fcp haz\u0131rland\u0131. Bu t\u00fcplere; 0, \u00bd, 1, 2, 4 adet kolistin diski (10 \u00b5g; Becton Dickinson, ABD) eklendi. Son konsantrasyonlar s\u0131ras\u0131yla; 0 (\u00fcreme kontrol\u00fc), 0.5, 1, 2 ve 4 \u00b5g\/ml dir. Daha sonra t\u00fcpler oda s\u0131cakl\u0131\u011f\u0131nda kolistinin s\u0131v\u0131ya ge\u00e7ebilmesi i\u00e7in inok\u00fclasyondan \u00f6nce 30 dakika bekletildi. Kanl\u0131 agarda \u00fcretilen taze k\u00fclt\u00fcr kolonilerinden 0.5 McFarland bakteri s\u00fcspansiyonlar\u0131 haz\u0131rland\u0131 ve her t\u00fcpe 50 \u00b5l bu s\u00fcspansiyonlardan eklenerek vortekslendi. 35 \u00b0C\u2019de 18-20 saat ink\u00fcbasyondan sonra kolistin minimum inhibit\u00f6r konsantrasyonu (M\u0130K) de\u011ferleri okundu ve EUCAST verilerine g\u00f6re de\u011ferlendirildi.<\/p>\n<h3 class=\"p3\">Veri Analizi<\/h3>\n<p class=\"p2\">\u201c<span class=\"s1\"><i>\u00c7ok b\u00fcy\u00fck hata<\/i>\u201d (\u00c7BH), KSDE y\u00f6ntemi ile duyarl\u0131 iken referans y\u00f6ntem ile diren\u00e7li; \u201c<i>b\u00fcy\u00fck hata<\/i>\u201d (BH), KSDE y\u00f6ntemi ile diren\u00e7li iken referans y\u00f6ntem ile duyarl\u0131 bulunmas\u0131 olarak tan\u0131mland\u0131. Referans y\u00f6ntem olan s\u0131v\u0131 mikrodil\u00fcsyon ile KSDE y\u00f6ntemi aras\u0131ndaki \u201c<i>temel uyum<\/i>\u201d (TU) ve \u201c<i>kategorik uyum<\/i>\u201d (KU) de\u011ferleri hesapland\u0131. KSDE testi i\u00e7in TU de\u011feri olarak bakterilerin y\u00fczdesi referans metoda g\u00f6re M\u0130K de\u011ferinin \u00b11 dil\u00fcsyon dahilinde kabul edilerek; KU de\u011feri ise bakterilerin duyarl\u0131 ya da diren\u00e7li olmalar\u0131na g\u00f6re ayn\u0131 kategoride bulunma y\u00fczdesine g\u00f6re hesapland\u0131.<\/span><\/p>\n<p class=\"p2\">\u00c7al\u0131\u015fma i\u00e7in Ahi Evran \u00dcniversitesi Giri\u015fimsel Olmayan Klinik Ara\u015ft\u0131rmalar Etik Kurulu\u2019ndan 29 May\u0131s 2020 tarih ve 2020-07\/48 karar numaras\u0131yla onay Referans s\u0131v\u0131 mikrodil\u00fcsyon y\u00f6ntemine g\u00f6re M\u0130K aral\u0131klar\u0131 &lt;0.064 ile \u2265<b>\u00a0<\/b>32 \u00b5g\/ml aras\u0131ndayd\u0131 (Tablo 1). KSDE testinin performans de\u011ferlendirilmesinde, KU %94 ve TU %89 olarak tespit edildi; \u00c7BH oran\u0131 ise s\u0131f\u0131rd\u0131. KSDE testi ile \u00fc\u00e7 <i>K. pneumoniae<\/i> izolat\u0131 diren\u00e7liyken referans y\u00f6ntem ile duyarl\u0131yd\u0131lar. BH oran\u0131 %5 olarak saptand\u0131.<\/p>\n<p class=\"p2\"><span class=\"s3\">Test edilen 96 non-fermantatif bakteri grubunda, referans y\u00f6nteme g\u00f6re 93 izolat kolistin duyarl\u0131 \u00fc\u00e7\u00fc ise kolistin diren\u00e7liydi. Diren\u00e7li izolatlardan biri <i>A. baumannii<\/i> di\u011fer ikisi ise <i>P. aeruginosa<\/i> olarak tespit edildi. Referans s\u0131v\u0131 mikrodil\u00fcsyon y\u00f6ntemine g\u00f6re M\u0130K aral\u0131klar\u0131 &lt;0.064 ile 32 \u00b5g\/ml aras\u0131ndayd\u0131 (Tablo 2). KSDE testinin performans de\u011ferlendirmesinde KU %94 ve TU %92 olarak tespit edildi. <i>P. aeruginosa<\/i> izolat\u0131nda referans y\u00f6nteme g\u00f6re M\u0130K de\u011feri 32 olup KSDE testi 0.5\u2019ti; \u00c7BH oran\u0131 ise %33\u2019t\u00fc. Referans y\u00f6nteme g\u00f6re M\u0130K de\u011feri 0.064 olan <i>A. baumannii<\/i> izolat\u0131nda ise KSDE ile M\u0130K de\u011feri &gt;4 bulundu; BH oran\u0131 ise %1\u2019di (Tablo 3). \u00c7al\u0131\u015f\u0131lan izolatlar\u0131n referans y\u00f6nteme g\u00f6re kolistin M\u0130K da\u011f\u0131l\u0131m\u0131 Tablo 4\u2019te g\u00f6sterilmi\u015ftir.<\/span><\/p>\n<h2 class=\"p1\">\u0130RDELEME<\/h2>\n<p class=\"p2\">Kolistin, karbapenemaz \u00fcreten Gram-negatif basillerin etken oldu\u011fu infeksiyonlar\u0131n tedavisinde halen son se\u00e7enek oldu\u011fu i\u00e7in laboratuvarlar\u0131m\u0131zda kolistin duyarl\u0131l\u0131\u011f\u0131n\u0131 do\u011fru saptaman\u0131n \u00f6nemi b\u00fcy\u00fckt\u00fcr. \u00c7o\u011fu mikrobiyoloji laboratuvar\u0131nda antibiyotik duyarl\u0131l\u0131k testleri disk dif\u00fczyon, gradyan dif\u00fczyon gibi y\u00f6ntemler ve otomatize sistemler (Vitek, Phonex) kullan\u0131larak ger\u00e7ekle\u015ftirilmektedir. Ancak hem CLSI hem de EUCAST kolistin i\u00e7in s\u00f6z konusu y\u00f6ntemleri \u00f6nermemektedir. Kolistinin b\u00fcy\u00fck katyonik bir molek\u00fcl olmas\u0131, plastik y\u00fczeylere tutunmas\u0131, otomatize sistem sonu\u00e7lar\u0131ndaki \u00c7BH oranlar\u0131nda %35-53 gibi y\u00fcksek de\u011ferlerle kar\u015f\u0131la\u015f\u0131lmas\u0131 nedeniyle referans y\u00f6ntem olarak s\u0131v\u0131 mikrodil\u00fcsyon y\u00f6ntemi tek se\u00e7enektir (10). Bununla birlikte, kaynaklar\u0131 yetersiz olan \u00fclkelerdeki laboratuvarlar i\u00e7in kolistin tozunu sa\u011flamak zordur. Ayn\u0131 zamanda t\u00fcm d\u00fcnyada, kolistinin \u00e7ok kullan\u0131m\u0131na ba\u011fl\u0131 olarak diren\u00e7 de artmaya devam etmektedir (11). Bu nedenle rutin laboratuvarlarda kullanabilecek basit, pratik ve ucuz bir kolistin test metodunun acilen geli\u015ftirilmesi gerekmektedir.<span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p class=\"p2\">\u0130lk olarak 2018 y\u0131l\u0131nda Simner ve arkada\u015flar\u0131 (9) CLSI\u2019n\u0131n bir alt komitesinde KSDE testini tan\u0131mlam\u0131\u015ft\u0131r. 2021 CLSI-M100-S31\u2019de <i>Enterobacterales<\/i> ve <i>Pseudomanas <\/i>su\u015flar\u0131nda kolistin i\u00e7in; s\u0131v\u0131 mikrodil\u00fcsyon, KSDE ve KA (kolistin agar) test metotlar\u0131n\u0131n kullan\u0131m\u0131 \u00f6nerilmi\u015ftir. <i>Acinetobacter <\/i>spp. i\u00e7in ise sadece kabul edilen M\u0130K y\u00f6ntemi s\u0131v\u0131 mikrodil\u00fcsyon olarak belirtilmi\u015ftir (7).<span class=\"Apple-converted-space\">\u00a0<\/span><\/p>\n<p class=\"p2\">2018 y\u0131l\u0131ndan g\u00fcn\u00fcm\u00fcze bu konuda yap\u0131lan \u00e7al\u0131\u015fma say\u0131s\u0131 olduk\u00e7a azd\u0131r. \u00d6zyurt ve arkada\u015flar\u0131n\u0131n (12) yapt\u0131klar\u0131 \u00e7al\u0131\u015fmada, Gram-negatif bakterilerde KSDE y\u00f6ntemi, otomatize sistem sonu\u00e7lar\u0131 ve s\u0131v\u0131 mikrodil\u00fcsyon y\u00f6ntemi birlikte de\u011ferlendirilmi\u015ftir. S\u00f6z konusu \u00e7al\u0131\u015fmada, KSDE ile s\u0131v\u0131 mikrodil\u00fcsyon y\u00f6ntemi aras\u0131ndaki KU oran\u0131 %99.3, \u00c7BH oran\u0131 %0.2 ve BH oran\u0131 %0.5 olarak saptanm\u0131\u015ft\u0131r. Otomatize sistem ile referans y\u00f6ntemin KU oran\u0131 ise %95, \u00c7BH oran\u0131 ise %5 olarak tespit edilmi\u015ftir. Ancak nonfermentatif grupta, \u00c7BH oran\u0131 %33, BH oran\u0131 ise %1 tespit edilmi\u015ftir. Literat\u00fcrle uyumlu olarak nonfermentatif grupta bu testin kullan\u0131m\u0131n\u0131n uygun olmad\u0131\u011f\u0131 g\u00f6r\u00fclmektedir.<\/p>\n<p class=\"p2\">KSDE\u2019nin en b\u00fcy\u00fck avantajlar\u0131ndan biri kolistin tozuna ihtiya\u00e7 duyulmamas\u0131d\u0131r. Basit ekipmanlarla \u00e7al\u0131\u015f\u0131labiliyor olmas\u0131n\u0131n yan\u0131 s\u0131ra de\u011ferlendirilmesinin kolay olmas\u0131 da bu y\u00f6ntemin di\u011fer \u00f6nemli avantajlar\u0131d\u0131r. Ancak s\u0131n\u0131rda M\u0130K de\u011ferlerine sahip mcr-1 pozitif izolatlar i\u00e7in mutlaka s\u0131v\u0131 mikrodil\u00fcsyon y\u00f6ntem ile tekrar \u00e7al\u0131\u015f\u0131lmas\u0131 gerekti\u011fi vurgulanm\u0131\u015ft\u0131r (9). Plasmid kaynakl\u0131 kolistin direncinin g\u00f6sterilebilmesi i\u00e7in EDTA\u2019l\u0131 KSDE testi geli\u015ftirilerek \u00e7al\u0131\u015fmas\u0131 yap\u0131lm\u0131\u015ft\u0131r (14). \u015eu an i\u00e7in yayg\u0131n olan kromozomal kaynakl\u0131 kolistin direncidir; plazmid kaynakl\u0131 diren\u00e7 t\u00fcm d\u00fcnyada kolistin diren\u00e7li izolatlar\u0131n %3\u2019\u00fcn\u00fc olu\u015fturur (10).<span class=\"Apple-converted-space\">\u00a0 <\/span>\u00dclkemizde mcr-1 pozitif izolatlara nadir olarak rastlanmaktad\u0131r (5); s\u00f6z konusu izolatlarda kolistin M\u0130K de\u011ferleri 2-4 aras\u0131nda \u00e7\u0131kabilmektedir. CLSI, M\u0130K de\u011feri 2 olan izolatlar\u0131n kolistin sonu\u00e7lar\u0131n\u0131n s\u0131v\u0131 mikrodil\u00fcsyonla do\u011frulanmas\u0131n\u0131 \u00f6nermektedir.<\/p>\n<p class=\"p2\">\u00c7al\u0131\u015fmam\u0131zda k\u0131s\u0131tl\u0131l\u0131klar mevcuttur. S\u0131v\u0131ya ge\u00e7en kolistin konsantrasyonu ba\u015fka bir analitik metot ile do\u011frulanmal\u0131d\u0131r. Farkl\u0131 ticari markalar\u0131n kolistin diskleri kullan\u0131lmam\u0131\u015f; \u00e7al\u0131\u015f\u0131lan izolatlar\u0131n mcr-1 genlerine bak\u0131lmam\u0131\u015ft\u0131r. \u00c7al\u0131\u015fmaya dahil edilen nonfermentatiflerin say\u0131s\u0131 ise azd\u0131r.<\/p>\n<p class=\"p2\">Sonu\u00e7 olarak; rutin mikrobiyoloji laboratuvarlar\u0131nda basit ama g\u00fcvenilir y\u00f6ntemlere ihtiyac\u0131m\u0131z vard\u0131r.
