Geli\u015fmekte olan \u00fclkelerde y\u0131lda 1.5 milyon \u00e7ocuk \u00f6l\u00fcm\u00fcn\u00fcn yakla\u015f\u0131k %88\u2019i ishalin de dahil oldu\u011fu hastal\u0131klardan kaynaklanmaktad\u0131r (2). Gastroenterit etkeni olan bakteriler aras\u0131nda <i>Shigella <\/i>spp<i>., Salmonella <\/i>spp<i>., Yersinia enterocolitica, Campylobacter jejuni, Aeromonas <\/i>spp<i>., Vibrio cholerae ve E. coli <\/i>su\u015flar\u0131 yer almaktad\u0131r (3).<\/span><\/p>\n<p class=\"p2\">Akut gastroenterit etkenleri, morbidite ve mortalitenin yan\u0131 s\u0131ra baz\u0131 karma\u015f\u0131k sendromik durumlara da neden olmaktad\u0131r. Shiga toksini \u00fcreten <i>Escherichia coli <\/i>infeksiyonunu takiben b\u00f6brek yetmezli\u011finin de dahil oldu\u011fu hemolitik \u00fcremik sendrom (H\u00dcS); <i>C. jejuni <\/i>infeksiyonunu takiben Guillain-Barr<span class=\"s2\">\u00e9<\/span> sendromu (GBS); ve Enteroaggregatif <i>E. coli <\/i>infeksiyonunu takiben ishal ile veya ishal olmaks\u0131z\u0131n malnutrisyon tablolar\u0131 kar\u015f\u0131m\u0131za \u00e7\u0131kabilmekte ve bu tablolar ciddi seyreden uzun s\u00fcreli hastal\u0131klar ile sonu\u00e7lanabilmektedir (4). 1989 y\u0131l\u0131ndan bu yana, bula\u015f\u0131c\u0131 ishalle ba\u011flant\u0131l\u0131 olgular\u0131n bildirilmesi zorunlu olmakla birlikte \u00e7o\u011fu olgu sadece klinik tan\u0131ya dayan\u0131larak rapor edilmektedir (5).<\/p>\n<p class=\"p2\"><span class=\"s3\">Geli\u015fmekte olan \u00fclkelerde, modern olmayan laboratuvar ko\u015fullar\u0131nda ishale neden olan bakterilerin \u00e7o\u011funun tan\u0131s\u0131 ve s\u00fcrveryans\u0131 tam olarak yap\u0131lamamaktad\u0131r. Bu nedenle etkenlerin belirlenmesi ve \u00f6zellikle bakteriyel olanlar\u0131n antibiyotik duyarl\u0131l\u0131klar\u0131n\u0131n bilinmesinde fayda bulunmaktad\u0131r (6).<\/span><\/p>\n<p class=\"p2\">Gastroenterite neden olan etkenler, b\u00f6lgelere g\u00f6re de\u011fi\u015fiklik g\u00f6stermekte olup b\u00f6lgesel olarak olas\u0131 gastroenterit etkenlerinin bilinmesi, tan\u0131 ve tedavinin y\u00f6nlendirilmesi a\u00e7\u0131s\u0131ndan \u00f6nem arz etmektedir. Ayr\u0131ca antimikrobiyal duyarl\u0131l\u0131klar\u0131n\u0131n bilinmesi uygulanacak tedavide do\u011fru antibiyoti\u011fin se\u00e7ilmesinde hekime yol g\u00f6sterici olacakt\u0131r (6).<\/p>\n<p class=\"p2\">Bu \u00e7al\u0131\u015fmada, hastanemize ba\u015fvuran ishalli \u00e7ocuk hastalarda bakteriyel gastroenterit etkenlerin, Halk Sa\u011fl\u0131\u011f\u0131 Genel M\u00fcd\u00fcrl\u00fc\u011f\u00fc (HSGM) Ulusal Mikrobiyoloji Standartlar\u0131 Bula\u015f\u0131c\u0131 Hastal\u0131klar Tan\u0131 Rehberi\u2019ne g\u00f6re tespit edilmesi ve baz\u0131 mikroorganizmalar\u0131n antibiyotik duyarl\u0131l\u0131klar\u0131n\u0131n g\u00f6sterilerek ampirik olarak kullan\u0131labilecek antibiyotiklerin belirlenmesi ama\u00e7lanm\u0131\u015ft\u0131r (7).<\/p>\n<h2 class=\"p1\">Y\u00d6NTEMLER<\/h2>\n<p class=\"p2\">\u00c7al\u0131\u015fma kapsam\u0131nda, Necmettin Erbakan \u00dcniversitesi Meram T\u0131p Fak\u00fcltesi \u00e7ocuk acil ve di\u011fer \u00e7ocuk kliniklerine, Nisan 2018 ve Kas\u0131m 2018 tarihleri aras\u0131nda, ishal \u015fikayeti ile ba\u015fvuran, akut gastroenterit d\u00fc\u015f\u00fcn\u00fclen ve etyolojik ajanlar\u0131n belirlenmesi amac\u0131yla mikrobiyoloji laboratuvar\u0131na kabul edilen 487 \u00e7ocuk hastan\u0131n d\u0131\u015fk\u0131 \u00f6rnekleri incelendi. \u00c7al\u0131\u015fmam\u0131zda, bakteriyel gastroenterit etkenlerinden olan <i>Salmonella <\/i>spp., <i>Shigella <\/i>spp., <span class=\"s1\"><i>E. coli <\/i>O157:H7 su\u015fu,<i> Camylobacter <\/i>spp.,<i> Vibrio cholerae<\/i>,<i> Yersinia entocolitica <\/i>ve<i> Aeromonas <\/i>spp. gibi bakteriyel etkenlerin ara\u015ft\u0131r\u0131lmas\u0131na uygun y\u00f6ntemler kullan\u0131lm\u0131\u015ft\u0131r. <i>C. difficile<\/i> izolasyonu hedeflendi\u011fi i\u00e7in \u00f6zellikle antibiyotik kullan\u0131m \u00f6yk\u00fcs\u00fc olan hastalar \u00e7al\u0131\u015fmaya dahil edilmemi\u015ftir.<\/span><\/p>\n<p class=\"p2\">D\u0131\u015fk\u0131 \u00f6rnekleri, steril, s\u0131zd\u0131rmaz kapakl\u0131 d\u0131\u015fk\u0131 kaplar\u0131nda yakla\u015f\u0131k 1 gram olacak \u015fekilde topland\u0131. \u00d6rnekler, d\u0131\u015fk\u0131n\u0131n varsa kanl\u0131 ve\/veya mukuslu b\u00f6lgelerinden al\u0131nd\u0131. D\u0131\u015fk\u0131 \u00f6rneklerinden; selenit F \u00e7o\u011falt\u0131c\u0131 besiyerine, Hektoen enterik (HE) (Liofilchem, \u0130talya) agara, <i>Salmonella Shigella <\/i>(SS) (Liofilchem, \u0130talya) agara, \u201cxylose lysine desoxycholate (XLD)\u201d (Liofilchem, \u0130talya) agara, sorbitol MacConkey (SMAC) (Liofilchem, \u0130talya) agara, MacConkey (Liofilchem, \u0130talya) agara, <i>Campylobacter \u201c<\/i>Oxoid\u2122 Skirrow Selective Supplement\u201d (Thermo Scientific, \u0130ngiltere) agara, \u201cChromatic <i>Vibrio<\/i>\u201d (Liofilchem, \u0130talya) agara ve % 5\u2019lik koyun kanl\u0131 agara klasik k\u00fclt\u00fcr y\u00f6ntemi ile ekimler yap\u0131ld\u0131.<\/p>\n<p class=\"p2\">D\u0131\u015fk\u0131 kab\u0131nda taze olarak verilmi\u015f d\u0131\u015fk\u0131 \u00f6rne\u011finden, lam-lamel aras\u0131 preparat haz\u0131rlanarak 10x ve 40x objektifle inceleme yap\u0131ld\u0131. \u00d6rneklerin l\u00f6kosit ve eritrosit i\u00e7erip i\u00e7ermedi\u011fi de\u011ferlendirildi. Ba\u011f\u0131rsak parazitleri ve yumurtalar\u0131 a\u00e7\u0131s\u0131ndan de\u011ferlendirme yap\u0131ld\u0131; pozitif de\u011ferlendirmeler \u00e7al\u0131\u015fmadan \u00e7\u0131kar\u0131ld\u0131. <i>Camylobacter <\/i>spp. \u00fcremesinden \u015f\u00fcphe edilen \u00f6rnekler Gram boyamas\u0131 da yap\u0131larak incelendi.<\/p>\n<p class=\"p2\">\u0130shalli \u00e7ocuk hastalar\u0131n d\u0131\u015fk\u0131 \u00f6rneklerinden bir \u00f6ze dolusu \u00f6rnek al\u0131narak, tam plak y\u00fczeyine tek koloni ekimi yap\u0131ld\u0131. <i>Salmonella<\/i> ve <i>Shigella<\/i> spp<i>. <\/i>izolasyon i\u00e7in zenginle\u015ftirme amac\u0131yla selenit F \u00e7o\u011falt\u0131c\u0131 besiyerine inok\u00fcle edildi ve 37\u00baC\u2019de 8 saat ink\u00fcbe edildi. Sekiz saatlik ink\u00fcbasyondan sonra selenit F \u00e7o\u011falt\u0131c\u0131 besiyerinden bir \u00f6ze dolusu al\u0131narak SS, HE ve XLD agar kat\u0131 besiyerlerine pasajlar yap\u0131ld\u0131 ve besiyerleri 37\u00baC\u2019de 18-24 saat ink\u00fcbasyona b\u0131rak\u0131ld\u0131. Sonras\u0131nda \u00e7e\u015fitli se\u00e7ici besiyerlerinde, <i>Salmonella <\/i>spp. izolasyon oranlar\u0131n\u0131n g\u00f6zlenmesi hedeflendi.<\/p>\n<p class=\"p2\"><i>Escherichia coli <\/i>O157:H7 su\u015funun izolasyonu i\u00e7in;<span class=\"s4\"> d\u0131\u015fk\u0131 <\/span>kab\u0131 i\u00e7erisinden al\u0131nan \u00f6rneklerden birer \u00f6ze al\u0131narak sorbitol MacConkey agar ve EMB agar kat\u0131 besiyerlerine tek koloni ekimi yap\u0131ld\u0131 ve besiyerleri 37\u00baC\u2019de 18-24 saat ink\u00fcbasyona b\u0131rak\u0131ld\u0131.<\/p>\n<p class=\"p2\"><span class=\"s1\"><i>Campylobacter <\/i>spp. izolasyonu i\u00e7in; d\u0131\u015fk\u0131 kab\u0131 i\u00e7erisine al\u0131nan \u00f6rneklerden bir ek\u00fcvyon yard\u0131m\u0131 ile al\u0131nan \u00f6rnekler <i>Campylobacter<span class=\"Apple-converted-space\">\u00a0 <\/span><\/i>\u201cOxoid\u2122 Skirrow Selective Supplement\u201d agar besiyerine ekildi. Ekilen plaklar, mikroaerofilik atmosfer ko\u015fullar\u0131nda 42\u00baC\u2019de 72 saate kadar ink\u00fcbe edildi.<\/span><\/p>\n<p class=\"p2\"><i>Yersinia enterocolitica <\/i>enterik patojenlerinin izolasyonu i\u00e7in; d\u0131\u015fk\u0131 kab\u0131 i\u00e7erisine al\u0131nan \u00f6rneklerden birer \u00f6ze al\u0131narak MacConkey agar besiyerine tek koloni ekim y\u00f6ntemi ile ekim yap\u0131ld\u0131. Ekim yap\u0131lan MacConkey besiyeri 37\u00baC\u2019de 24 saat aerop \u015fartlarda ink\u00fcbasyona b\u0131rak\u0131ld\u0131.<\/p>\n<p class=\"p2\"><span class=\"s1\"><i>Vibrio cholerae <\/i>enterik patojenlerinin izolasyonu i\u00e7in; d\u0131\u015fk\u0131 kab\u0131 i\u00e7erisine al\u0131nan \u00f6rneklerden birer \u00f6ze al\u0131narak \u201cChromatic Vibrio\u201d agar besiyerine tek koloni ekim y\u00f6ntemi ile ekim yap\u0131ld\u0131. Ekim yap\u0131lan \u201cChromatic Vibrio\u201d agar besiyeri 37\u00baC\u2019de 18-24 saat aerop \u015fartlarda ink\u00fcbasyona b\u0131rak\u0131ld\u0131.<\/span><\/p>\n<p class=\"p2\"><i>Aeromonas <\/i>spp. enterik patojenlerin izolasyonu i\u00e7in; d\u0131\u015fk\u0131 kab\u0131 i\u00e7erisine al\u0131nan \u00f6rneklerden birer \u00f6ze al\u0131narak %5\u2019lik koyun kanl\u0131 agara tek koloni ekim y\u00f6ntemi ile ekim yap\u0131ld\u0131. Ekim yap\u0131lan %5\u2019lik koyun kanl\u0131 agar 37\u00baC\u2019de 24 saat aerop \u015fartlarda ink\u00fcbasyona b\u0131rak\u0131ld\u0131.<\/p>\n<p class=\"p2\">K\u00fclt\u00fcr sonu\u00e7lar\u0131n\u0131n de\u011ferlendirilmesi ve tan\u0131mlanmas\u0131, konvansiyonel y\u00f6ntemlerle ve otomatize sistem VITEK\u00ae 2 (bioM\u00e9rieux, Marcy l\u2019Etoile,\u00a0Fransa) ile yap\u0131ld\u0131. VITEK\u00ae 2 cihaz\u0131yla, <i>Salmonella<\/i> spp. veya <i>Shigella <\/i>spp<i>. <\/i>olarak tan\u0131mlanan izolatlar serolojik do\u011frulama yap\u0131larak antibiyogram testlerine al\u0131nd\u0131.<\/p>\n<p class=\"p2\">EUCAST kriterleri\u2019ne g\u00f6re ger\u00e7ekle\u015ftirilen antibiyotik duyarl\u0131l\u0131k testleri ve 37\u00b0C\u2019de 18-24 saat ink\u00fcbasyon sonunda duyarl\u0131l\u0131klar belirlendi (8). Serogrupland\u0131rma, polivalan \u00f6zellikteki antiserumlar (Plasmatec, \u0130ngiltere ) kullan\u0131larak agl\u00fctinasyon deneyi ile yap\u0131ld\u0131.<\/p>\n<p class=\"p2\"><span class=\"s1\">\u0130nk\u00fcbasyon sonras\u0131nda b\u00fct\u00fcn plaklar \u015f\u00fcpheli enterik patojen kolonileri bak\u0131m\u0131ndan incelendi. <i>Salmonella <\/i>spp<i>. <\/i>ve <i>Shigella <\/i>spp. i\u00e7in; SS, HE ve XLD agar besiyerinde 18-24 saat sonra iyi geli\u015fmi\u015f olan koloniler incelendi. XLD agarda, k\u0131rm\u0131z\u0131 siyah merkezi olan koloniler ve k\u0131rm\u0131z\u0131 koloniler; HE agarda, mavi-ye\u015fil siyah merkezi olan koloniler ve ye\u015fil nemli koloniler; SS agarda, siyah merkezi olan koloniler ve renksiz, \u015feffaf koloniler se\u00e7ildi. Daha sonra do\u011frulamak i\u00e7in biyokimyasal testlere tabi tutuldu.<\/span><\/p>\n<p class=\"p2\"><i>Salmonella<\/i> spp. i\u00e7in \u201ctriple sugar iron\u201d (TSI) besiyerinde asit\/asit veya asit\/alkali, genelde gaz ve H<sub>2<\/sub>S olu\u015fturan, \u00fcreaz ve indol negatif, hareket pozitif olan izolatlar; <i>Shigella<\/i> spp. i\u00e7inse TSI besiyerinde asit\/alkali, gaz ve H<sub>2<\/sub>S olu\u015fturmayan, sitrat, \u00fcreaz ve hareket negatif izolatlar, <i>Salmonella <\/i>spp. veya <i>Shigella <\/i>spp. oldu\u011fu d\u00fc\u015f\u00fcn\u00fclerek sonraki tan\u0131mlama basamaklar\u0131 i\u00e7in se\u00e7ildi.<\/p>\n<p class=\"p2\">Tan\u0131mlama basamaklar\u0131n\u0131n sonunda, <i>Salmonella <\/i>spp. veya <i>Shigella <\/i>spp. oldu\u011fu d\u00fc\u015f\u00fcn\u00fclen bakterilerin tan\u0131mlamas\u0131 otomatize sistem VITEK\u00ae 2 ile ger\u00e7ekle\u015ftirildi ve izolatlar serolojik do\u011frulama yap\u0131larak antibiyogram testlerine al\u0131nd\u0131.<\/p>\n<p class=\"p2\">Sorbitol MacConkey agarda bir gecelik ink\u00fcbasyon sonucunda sorbitol\u00fc fermente etmeyen renksiz koloniler ve EMB agarda parlak r\u00f6fle \u015feklinde \u00fcreyen koloniler, <i>E. coli <\/i>O157:H7 olmas\u0131 s\u00fcphesiyle se\u00e7ildi. Daha sonra do\u011frulamak i\u00e7in biyokimyasal testler (IMVIC, TSI, hareket testi, \u00fcreaz) yap\u0131ld\u0131 ve <i>E. coli <\/i>O157:H7 agl\u00fctinasyon kiti (Prolex, ABD) ile O157:H7 antijenlerinin varl\u0131\u011f\u0131 test edildi.<\/p>\n<p class=\"p2\"><i>Campylobacter <\/i>spp. tespiti i\u00e7in, <i>Campylobacter <\/i>\u201cOxoid\u2122 Skirrow Selective Supplement\u201d<span class=\"Apple-converted-space\">\u00a0 <\/span>besiyerinin y\u00fczeyinde su damlas\u0131na benzer \u015fekilde \u00fcreyen d\u00fczensiz kolonilerden Gram boyamas\u0131 yap\u0131ld\u0131. S veya virg\u00fcl \u015feklinde mart\u0131 kanad\u0131na benzeyen Gram negatif basiller g\u00f6r\u00fclm\u00fc\u015f ise oksidaz ve katalaz aktiviteleri test edildi ve oksidaz ve katalaz aktivitesi pozitif olanlar <i>Campylobacter <\/i>spp. olarak kabul edildi.<\/p>\n<p class=\"p2\"><i>Y. enterocolitica <\/i>tespiti i\u00e7in, MacConkey besiyerinde \u00fcreyen 1 mm\u2019den k\u00fc\u00e7\u00fck veya toplu i\u011fne ba\u015f\u0131 \u015feklinde, hafif pembe laktoz negatif, oksidaz negatif katalaz pozitif koloniler, <i>Y. enterocolitica<\/i> \u015f\u00fcphesiyle biyokimyasal testlere tabi tutuldu. TSI\u2019da asit\/asit ve gaz olu\u015fturmayan, 25 \u00baC\u2019de indol pozitifli\u011fi olan izolatlar\u0131n <i>Y. enterocolitica<\/i> olaca\u011f\u0131 d\u00fc\u015f\u00fcn\u00fcld\u00fc.<\/p>\n<p class=\"p2\"><i>Vibrio cholerae <\/i>tespiti i\u00e7in <i>\u201c<\/i>Chromatic Vibrio\u201d agar besiyerinde \u00fcreyen, turkuaz mavisi ve ye\u015fil mavi renkte olan koloniler, %5\u2019lik koyun kanl\u0131 agarda hemoliz olu\u015fturan koloniler, MacConkey ve SS agar\u2019da laktoz negatif koloniler ve oksidaz pozitif izolatlar se\u00e7ildi. Kolonilerden yap\u0131lan Gram boyal\u0131 yaymada; gram negatif, virg\u00fcl \u015feklinde k\u0131vr\u0131k basiller olarak g\u00f6zlenen koloniler biyokimyasal testlere tabi tutuldu. Biyokimyasal testlerde TSI\u2019de asit\/asit g\u00f6r\u00fcn\u00fcm\u00fc olan sitrat ve indol testi pozitif, \u00fcreaz testi negatif olan izolatlar\u0131n (O129 diskine duyarl\u0131 olanlar) <i>Vibrio cholerae<\/i> olaca\u011f\u0131 d\u00fc\u015f\u00fcn\u00fcld\u00fc.<\/p>\n<p class=\"p2\"><i>Aeromonas <\/i>spp. tespiti i\u00e7in, %5\u2019lik koyun kanl\u0131 agarda hemoliz olu\u015fturan ve EMB agarda laktoz negatif kolonilerden oksidaz testi pozitif olan \u00f6rnekler se\u00e7ildi. Biyokimyasal teste tabi tutuldu. Ayr\u0131ca, tespit edilen bu bakteriler otomatize sistem VITEK\u00ae 2 ile tan\u0131mlanm\u0131\u015f ve sonras\u0131nda <i>Salmonella<\/i> spp<i>.<\/i>\u2019nin serotiplendirmesi ve antibiyogram\u0131 yap\u0131lm\u0131\u015ft\u0131r.<\/p>\n<p class=\"p2\"><i>Salmonella<\/i> spp. izolasyonu amac\u0131yla ekim yap\u0131lan se\u00e7ici besiyerlerinde, HE agar besiyerinin 35\u2019inde \u00fcreyen kolonilerin <i>Salmonella <\/i>spp. olabilece\u011fi d\u00fc\u015f\u00fcn\u00fclm\u00fc\u015f ve biyokimyal olarak incelenmesi sonucunda en y\u00fcksek <i>Salmonella <\/i>spp. izolasyonu HE agarda (17 izolat) g\u00f6zlemlenmi\u015ftir. XLD agarda 29 \u00fcremeden 15\u2019i ve SS agarda ise 25 \u00fcremeden 12\u2019si <i>Salmonella <\/i>spp. olarak tan\u0131mlanm\u0131\u015ft\u0131r.<\/p>\n<p class=\"p2\">\u0130zole edilen 19 <i>Salmonella<\/i> t\u00fcr\u00fcn\u00fcn 16\u2019s\u0131 <i>Salmonella enteritidis<\/i> ve 3\u2019\u00fc<i> Salmonella <\/i>spp. olarak tan\u0131mlanm\u0131\u015ft\u0131r. %20 oran\u0131nda ampisilin, %16 oran\u0131nda trimetoprim-sulfametoksazol direnci, %17 oran\u0131nda sefotaksim direnci, %12 oran\u0131nda siprofloksasin direnci, %6 oran\u0131nda tetrasiklin direnci belirlenmi\u015f; pefloksasine diren\u00e7 g\u00f6zlenmemi\u015ftir.<\/p>\n<h2 class=\"p1\">TARTI\u015eMA<\/h2>\n<p class=\"p2\">Akut ishal ve akut gastroenterit, d\u00fc\u015f\u00fck, orta gelirli ve y\u00fcksek gelirli \u00fclkelerde yayg\u0131n g\u00f6r\u00fclen rahats\u0131zl\u0131klard\u0131r. \u0130shal ile seyreden hastal\u0131klar, 5 ya\u015f\u0131ndan k\u00fc\u00e7\u00fck \u00e7ocuklarda \u00f6l\u00fcm nedenleri aras\u0131nda \u00fc\u00e7\u00fcnc\u00fc s\u0131radaki yerini korumaktad\u0131r (9).<\/p>\n<p class=\"p2\">Geli\u015fmekte olan \u00fclkelerde morbidite ve mortaliteye sebep olan \u00f6nemli bakteriyel gastroenterit etkenlerinin, <i>Campylobacter <\/i>spp. (%12.5), enterotoksijen <i>E. coli<\/i> (ETEC) (%15), <i>Shigella <\/i>spp. (%10),<i> Salmonella <\/i>spp. (%3 ile10 aras\u0131nda), <i>Vibrio cholerae <\/i>(%7.5) ve enteropatojen <i>E. coli<\/i> (EPEC) (%2.5) oldu\u011fu bildirilmi\u015ftir. Bakteriler aras\u0131nda en s\u0131k <i>Salmonella, Shigella<\/i> ve <i>Campylobacter<\/i> t\u00fcrlerinin etken oldu\u011fu belirtilirken, etkenlerin b\u00f6lgesel da\u011f\u0131l\u0131m\u0131 farkl\u0131l\u0131klar g\u00f6stermektedir (10).<\/p>\n<p class=\"p2\"><span class=\"s1\">Son y\u0131llarda \u00fclkemizde <i>Salmonella<\/i> ve <i>Shigella<\/i> t\u00fcrlerinin izolasyonu i\u00e7in yap\u0131lan \u00e7al\u0131\u015fmalarda izolasyon s\u0131kl\u0131\u011f\u0131 de\u011fi\u015fmektedir. Bak\u0131c\u0131 ve arkada\u015flar\u0131n\u0131n (11), 787\u2019si \u00e7ocuk ve 414\u2019\u00fc eri\u015fkin olmak \u00fczere 1201 hastan\u0131n d\u0131\u015fk\u0131 k\u00fclt\u00fcrleri ile yapt\u0131\u011f\u0131 \u00e7al\u0131\u015fmada, %2.74 <i>Salmonella <\/i>t\u00fcr\u00fc ve %0.91<i> Shigella <\/i>t\u00fcr\u00fc tespit edilmi\u015ftir. \u0130nce\u2019nin (12), 1335 ishalli \u00e7ocuk d\u0131\u015fk\u0131 k\u00fclt\u00fcr\u00fcnde yapt\u0131\u011f\u0131 \u00e7al\u0131\u015fmada, %3.3 oran\u0131nda non tifoidal <i>Salmonella<\/i> t\u00fcr\u00fc ve %9.8 oran\u0131nda <i>Shigella <\/i>t\u00fcr\u00fc belirlenmi\u015ftir. Bu da <i>Salmonella<\/i> t\u00fcr\u00fcn\u00fcn gastroenterit bakteriler aras\u0131nda \u00f6nemli bir yerinin oldu\u011funu g\u00f6stermektedir. <i>Salmonella<\/i> t\u00fcr\u00fcn\u00fcn s\u0131kl\u0131\u011f\u0131, tiplendirilmesi ve antibiyogram\u0131 bu bakteriye kar\u015f\u0131 tedavide \u00f6nemli yer tutmaktad\u0131r. Ayr\u0131ca \u00e7al\u0131\u015fmam\u0131zda, di\u011ferlerinden farkl\u0131 olarak, <i>Salmonella <\/i>t\u00fcrlerinin izolasyonu i\u00e7in hangi besiyerinin daha ideal olaca\u011f\u0131 \u00fc\u00e7 farkl\u0131 besiyeri kullan\u0131larak ara\u015ft\u0131r\u0131lm\u0131\u015f olup en y\u00fcksek izolasyon oran\u0131 HE agarda (17 izolat) g\u00f6zlemlenmi\u015f; bunu XLD agar ve SS agar izlemi\u015ftir. Hastanelerde s\u0131kl\u0131kla SS agar kullan\u0131ld\u0131\u011f\u0131 g\u00f6zlenmektedir. Bu konuya dikkat \u00e7ekilmesinin faydal\u0131 olaca\u011f\u0131n\u0131 d\u00fc\u015f\u00fcn\u00fcyoruz.<\/span><\/p>\n<p class=\"p2\">\u0130nce (12)\u2019nin yapt\u0131\u011f\u0131 \u00e7al\u0131\u015fmada, 1335 ishalli \u00e7ocuk d\u0131\u015fk\u0131 k\u00fclt\u00fcr\u00fcnde 41 <i>Salmonella<\/i> t\u00fcr\u00fc bakteri belirlemi\u015f ve d\u0131\u015fk\u0131 k\u00fclt\u00fcr\u00fcnde \u00fcremi\u015f olan bu mikroorganizmalar\u0131n serotiplendirmesi yap\u0131ld\u0131\u011f\u0131nda; 41 <i>Salmonella<\/i> su\u015funun 28 (%68.2)\u2019inin S. <i>enteritidis, <\/i>7 (%17)\u2019sinin<i> S. typhimurium,<\/i> 3 (%7.3)\u2019\u00fcn\u00fcn<i> S. irumu, <\/i>3 (%7.3)\u2019\u00fcn\u00fcn <i>S. paratyphi B <\/i>oldu\u011fu saptanm\u0131\u015ft\u0131r.<\/p>\n<p class=\"p2\"><span class=\"s6\">G\u00fclmez ve arkada\u015flar\u0131n\u0131n (16), 4162 d\u0131\u015fk\u0131 \u00f6rne\u011fi ile yapt\u0131\u011f\u0131 \u00e7al\u0131\u015fmada,143 <i>Salmonella<\/i> t\u00fcr\u00fc belirlemi\u015f ve en s\u0131k rastlanan <i>Salmonella<\/i> serovar\u0131 <i>Salmonella enteritidis<\/i> olup yaln\u0131zca bir <i>Salmonella typhi <\/i>bulmu\u015ftur. Tural-Kara ve arkada\u015flar\u0131n\u0131n (17), 2425 \u00e7ocuk hastada yapt\u0131\u011f\u0131 \u00e7al\u0131\u015fmada, <i>Salmonella<\/i> t\u00fcr\u00fc belirlemi\u015f olup 29\u2019u <i>S. enteritidis <\/i>ve 3\u2019\u00fc <i>S. typhimurium\u2019<\/i>dur; 4 su\u015f ise tiplendirilememi\u015ftir. Bizim \u00e7al\u0131\u015fmam\u0131zda da di\u011fer \u00e7al\u0131\u015fmalara benzer \u015fekilde en s\u0131k olarak <i>Salmonella enteritidis <\/i>izole edilmi\u015ftir.<\/span><\/p>\n<p class=\"p2\">T\u00fcrkiye\u2019de akut gastroenterit olgular\u0131 \u00fczerine yap\u0131lan \u00e7al\u0131\u015fmalarda <i>Campylobacter <\/i>spp. izolasyon oranlar\u0131n\u0131n %1.4 ile %14.6 aras\u0131nda oldu\u011fu rapor edilmi\u015ftir (18). \u00d6zen ve arkada\u015flar\u0131n\u0131n (19), 412 hasta d\u0131\u015fk\u0131 \u00f6rne\u011fi ile yapt\u0131\u011f\u0131 \u00e7al\u0131\u015fmada, 6 (%1.5) \u00f6rnekte <i>Campylobacter <\/i>spp. tespit edilmi\u015ftir. Kayman ve arkada\u015flar\u0131n\u0131n (22) 3287 hastaya ait d\u0131\u015fk\u0131 \u00f6rne\u011fi ile yapt\u0131\u011f\u0131 \u00e7al\u0131\u015fmada, gastroenteritli olgular\u0131n %5.4 (179\/3287)\u2019\u00fcnden <i>Campylobacter <\/i>spp. izole edilmi\u015ftir. <i>Campylobacter <\/i>spp<i>. <\/i>izole edilen olgular\u0131n %71 (127\/179)\u2019inin \u00e7ocuk ve %58 (104\/179)\u2019inin erkek olgular oldu\u011fu izlenmi\u015ftir.<\/p>\n<p class=\"p3\">Farkl\u0131 \u00fclkede yap\u0131lan \u00e7al\u0131\u015fmalarda ise gastroenterit etyolojisinde <i>Campylobacter jejuni<\/i> ba\u015fta olmak \u00fczere <i>Campylobacter<\/i>\u2019lerin en s\u0131k rastlanan etken oldu\u011fu belirtilmektedir. <span class=\"s7\">Lee ve arkada\u015flar\u0131n\u0131n (23) 18-44 ayl\u0131k olan 442 \u00e7ocuk d\u0131\u015fk\u0131 \u00f6rne\u011fi ile yapt\u0131\u011f\u0131 \u00e7al\u0131\u015fmada, %8.3 oran\u0131nda <i>Campylobacter<\/i> spp. tespit edilmi\u015ftir.<\/span><\/p>\n<p class=\"p2\">Bu \u00e7al\u0131\u015fmada, %1.4 <i>Campylobacter<\/i> spp. tespit edilmi\u015f olup sonu\u00e7lar\u0131m\u0131z baz\u0131 \u00e7al\u0131\u015fmalarla uyumluyken baz\u0131lar\u0131ndan daha d\u00fc\u015f\u00fck oranda izolasyon g\u00f6r\u00fclm\u00fc\u015ft\u00fcr. Mevsim ve beslenme al\u0131\u015fkanl\u0131klar\u0131 ve hijyen k\u00fclt\u00fcr\u00fcn\u00fcn geli\u015fmesi bu bakterilerin izolasyon oranlar\u0131n\u0131 etkileyebilmektedir.<\/p>\n<p class=\"p2\">Enterohemorajik <i>E. coli<\/i> (EHEC) su\u015flar\u0131 \u00f6nemli bir ba\u011f\u0131rsak patojeni olarak son y\u0131llarda \u00f6nem kazanmaktad\u0131r. EHEC su\u015flar\u0131n\u0131n en \u00e7ok izole edileni ve en iyi bilineni, O157 serogrubudur. <i>E. coli<\/i> 0157:H7\u2019nin yayg\u0131nl\u0131\u011f\u0131 ile ilgili \u00e7al\u0131\u015fmalar\u0131n sonu\u00e7lar\u0131 farkl\u0131d\u0131r. \u00dclkemizde yap\u0131lan \u00e7al\u0131\u015fmalarda, <i>E. coli<\/i> O157 ve <i>E. coli<\/i> O157 H7 varl\u0131\u011f\u0131n\u0131n tespiti \u015f\u00f6yledir: Zarakolu ve arkada\u015flar\u0131n\u0131n<span class=\"Apple-converted-space\">\u00a0 <\/span>(24) 1200 \u00f6rnek ile yapt\u0131\u011f\u0131 \u00e7al\u0131\u015fmada, \u00f6rneklerin 5\u2019inde <i>E. coli<\/i> O157:H7 izole edilmi\u015ftir. SMAC agarda sorbitolu fermente etmeyen 5 <i>E. coli <\/i>izolat\u0131 ile yap\u0131lan <i>E. coli <\/i>O157:H7 lateks test reaktif \u00e7al\u0131\u015fmas\u0131nda 3 (%0.6)<i> E. coli <\/i>O157:H7 su\u015fu tespit edilmi\u015ftir. Sonu\u00e7lar\u0131m\u0131za bak\u0131ld\u0131\u011f\u0131nda \u00fclkemizdeki di\u011fer \u00e7al\u0131\u015fmalar\u0131n sonu\u00e7lar\u0131 ile benzer oldu\u011fu g\u00f6r\u00fclmektedir. \u00dclkemizde yap\u0131lan \u00e7al\u0131\u015fmalar\u0131n baz\u0131lar\u0131nda <i>E. coli<\/i> O157:H7 ve<i> E. coli<\/i> O157 varl\u0131\u011f\u0131 tespit edilmemi\u015f olmas\u0131na ra\u011fmen farkl\u0131 \u00e7al\u0131\u015fmalarda tespit edilmi\u015f olmas\u0131, <i>E. coli<\/i> O157:H7 ve<i> E. coli<\/i> O157 varl\u0131\u011f\u0131n\u0131n \u00f6nemini g\u00f6stermektedir. Il\u0131man iklime sahip b\u00f6lgelerde <i>Y. enterocolitica <\/i>oran\u0131n\u0131n daha d\u00fc\u015f\u00fck oldu\u011fu bildirilmektedir. Kuzey Avrupa \u00fclkeleri, Kanada ve G\u00fcney Amerika\u2019da infeksiyon etkeni olarak <i>Y. enterocolitica <\/i>s\u0131kl\u0131kla bildirilmekte olup d\u00fc\u015f\u00fck s\u0131cakl\u0131kta \u00fcreyebilmesi sebebiyle daha \u00e7ok buzdolab\u0131nda saklanan \u00fcr\u00fcnlerden bula\u015fmaktad\u0131r (23).<\/span><\/p>\n<p class=\"p2\">\u00dclkemizde <i>Y. enterocolitica<\/i>\u2019ya ba\u011fl\u0131 olarak geli\u015fen gastroenterit s\u0131kl\u0131\u011f\u0131n\u0131 ara\u015ft\u0131rmaya y\u00f6nelik bulunabilen s\u0131n\u0131rl\u0131 say\u0131daki \u00e7al\u0131\u015fman\u0131n sonu\u00e7lar\u0131na g\u00f6re, bu bakterinin d\u0131\u015fk\u0131 k\u00fclt\u00fcrlerinden izolasyonu %0-4.9 aras\u0131nda; yurt d\u0131\u015f\u0131nda yap\u0131lan \u00e7al\u0131\u015fmalarda ise % 0-2.2 aras\u0131nda tespit edilmi\u015ftir (14). \u00d6zkan ve G\u00fcnhan\u2019\u0131n (30), 191 ishalli hastada yapt\u0131\u011f\u0131 \u00e7al\u0131\u015fmada, <i>Y. enterocolitica<\/i> tespit edilmemi\u015ftir. Kaya ve arkada\u015flar\u0131n\u0131n (31), 128 gastroenteritli hasta d\u0131\u015fk\u0131 \u00f6rnekleri ile yapt\u0131\u011f\u0131 \u00e7al\u0131\u015fmada, 2 (%1.56) hasta \u00f6rne\u011finden <i>Y. enterocolitica<\/i> izole edilmi\u015ftir. Zarakolu ve arkada\u015flar\u0131n\u0131n (24), 1995-1997 y\u0131llar\u0131 aras\u0131nda 1200 ishalli ve 100 sa\u011fl\u0131kl\u0131 \u00e7ocu\u011fun d\u0131\u015fk\u0131 \u00f6rne\u011fi ile yapt\u0131\u011f\u0131 \u00e7al\u0131\u015fmada, <i>Y. enterocolitica<\/i> izole edilmemi\u015ftir. Yaz\u0131c\u0131 ve arkada\u015flar\u0131 (14), 200 olgunun etkenlerini ara\u015ft\u0131rd\u0131klar\u0131 \u00e7al\u0131\u015fmada, <i>Y. enterocolitica<\/i> izole etmemi\u015ftir. \u00dclkemizin nispeten \u0131l\u0131man bir iklime sahip olmas\u0131 ve <i>Y. enterocolitica<\/i> infeksiyonlar\u0131n\u0131n bula\u015fmas\u0131nda olduk\u00e7a etkili bir hayvan olan domuzun yiyecek olarak t\u00fcketilmemesi nedeniyle bu bakteri a\u00e7\u0131s\u0131ndan \u00f6nemli bir risk bulunmad\u0131\u011f\u0131 belirtilmektedir (14). Bu \u00e7al\u0131\u015fmada da <i>Y. enterocolitica<\/i> izole edilememi\u015f olup \u00fclkemizde yap\u0131lan di\u011fer \u00e7al\u0131\u015fmalar\u0131n sonu\u00e7lar\u0131 ile uyumludur.<\/p>\n<p class=\"p2\">Fekal oral yol ile bula\u015fan <i>Vibrio<\/i> infeksiyonlar\u0131 di\u011fer hastal\u0131klar gibi sosyoekonomik y\u00f6nden geli\u015fmekte olan \u00fclkelerde b\u00fcy\u00fck salg\u0131nlara sebep olmaktad\u0131r. Geli\u015fmi\u015f \u00fclkelerde ise sporadik olgular \u015feklinde meydana gelmektedir. Epidemik koleran\u0131n k\u00f6keni Hindistan ve G\u00fcney Asya \u00fclkeleridir. Bu \u00fclkelerde her y\u0131l salg\u0131nlar g\u00f6r\u00fclmekte ve bu b\u00f6lgelerden d\u00fcnyaya yay\u0131lmaktad\u0131r. Zarakolu ve arkada\u015flar\u0131n\u0131n (24), 1995-1997 y\u0131llar\u0131 aras\u0131nda 1200 ishalli ve 100 sa\u011fl\u0131kl\u0131 \u00e7ocu\u011fun d\u0131\u015fk\u0131 \u00f6rne\u011fi ile ilgili yapt\u0131\u011f\u0131 \u00e7al\u0131\u015fman\u0131n yan\u0131 s\u0131ra Erdo\u011fan ve arkada\u015flar\u0131n\u0131n (13), 1999-2002 y\u0131llar\u0131 aras\u0131nda \u00e7ocuk ve yeti\u015fkin hastalara ait 4674 d\u0131\u015fk\u0131 \u00f6rne\u011fi ile yapt\u0131\u011f\u0131 \u00e7al\u0131\u015fmada da <i>V. cholerae<\/i> tespit edilmemi\u015ftir. Yaz\u0131c\u0131 ve arkada\u015flar\u0131n\u0131n (14), 2007-2008 y\u0131llar\u0131 aras\u0131nda akut gastroenterit semptomlar\u0131 bulunan 200 olgunun etkenlerini ara\u015ft\u0131rd\u0131klar\u0131 \u00e7al\u0131\u015fmada <i>V. cholerae<\/i> \u00fcretilememi\u015ftir. Bizim \u00e7al\u0131\u015fmam\u0131zda da <i>V. cholerae<\/i> izole edilememi\u015f olup \u00fclkemizde yap\u0131lan \u00e7al\u0131\u015fmalarla uyumludur.<\/p>\n<p class=\"p2\"><i>Aeromonas <\/i>t\u00fcrleri, son y\u0131llarda gastroenterit etkenleri aras\u0131nda say\u0131lmakta olan bakterilerdir. \u00d6zellikle yaz aylar\u0131nda \u00e7evre sular\u0131ndaki konsantrasyonlar\u0131 h\u0131zla artarak ishale neden olduklar\u0131 belirtilmektedir. \u00dclkemizde, <i>Aeromonas<\/i> t\u00fcr\u00fc ile ilgili yap\u0131lan \u00e7al\u0131\u015fmalarda; Zarakolu ve arkada\u015flar\u0131n\u0131n (24) yapt\u0131\u011f\u0131 \u00e7al\u0131\u015fmada, <i>Aeromonas <\/i>t\u00fcrleri kapsam\u0131nda bakteri izole edilmemi\u015ftir.
