Su\u015flar\u0131n metisilin direnci, sefoksitin (10 \u03bcg) diski kullan\u0131larak disk dif\u00fczyon y\u00f6ntemiyle belirlenmi\u015ftir. European Committee on Antimicrobial Susceptibility Testing (EUCAST) Staphylococcus spp. i\u00e7in vankomisin zon \u00e7ap\u0131 s\u0131n\u0131r de\u011ferleri i\u00e7ermedi\u011fi i\u00e7in, su\u015flar\u0131n vankomisine duyarl\u0131l\u0131\u011f\u0131n\u0131n belirlenmesinde minimal inhibit\u00f6r konsantrasyon (M\u0130K) de\u011ferleri Etest\u00ae y\u00f6ntemiyle ara\u015ft\u0131r\u0131lm\u0131\u015ft\u0131r. Su\u015flar\u0131n di\u011fer \u03b2-laktam d\u0131\u015f\u0131 antibiyotiklere in vitro duyarl\u0131l\u0131\u011f\u0131n\u0131 ara\u015ft\u0131rmak i\u00e7in fusidik asid (10 \u03bcg), linezolid (10 \u03bcg), kinupristin\/dalfopristin (15 \u03bcg), kloramfenikol (30 \u03bcg), rifampisin (5 \u03bcg), siprofloksasin\u00a0 (5 \u03bcg) ve levofloksasin (5 \u03bcg) diskleriyle Kirby-Bauer disk dif\u00fczyon y\u00f6ntemi kullan\u0131lm\u0131\u015ft\u0131r. Etest\u00ae y\u00f6ntemiyle elde edilen vankomisin M\u0130K de\u011ferleri ve Kirby-Bauer disk dif\u00fczyon y\u00f6ntemiyle test edilen antibiyotiklerin zon \u00e7aplar\u0131, EUCAST \u00f6nerileri do\u011frultusunda yorumlanm\u0131\u015ft\u0131r.<\/p>\n<p> <strong>Bulgular<\/strong>: 84 MRSA su\u015funun %56\u2019s\u0131 yara, %32.1\u2019i kan ve %11.9\u2019u idrar \u00f6rneklerinden izole edilen su\u015flard\u0131. En fazla MRSA su\u015fu izole edilen servisler s\u0131ras\u0131yla Dahiliye (%17.9), Anestezi ve Reanimasyon Yo\u011fun Bak\u0131m (%16.6), Beyin Cerrahisi (%14.3), Ortopedi ve Travmatoloji (%11.9) ve Pediyatri Servisi (%11.9) \u015feklinde idi. Vankomisin i\u00e7in elde edilen M\u0130Kmin-max, M\u0130K50 ve M\u0130K90 de\u011ferleri s\u0131ras\u0131yla 0.75 \u00b5g\/ml-1.5 \u00b5g\/ml, 1 \u00b5g\/ml ve 1.5 \u00b5g\/ml olarak tespit edildi. Su\u015flar\u0131n tamam\u0131 vankomisin, linezolid ve kinupristin\/dalfopristine duyarl\u0131 olarak saptand\u0131. Antimikrobiyal duyarl\u0131l\u0131k oranlar\u0131 kloramfenikol i\u00e7in %96.4 ve fusidik asid i\u00e7in %76.2 olarak tespit edilmi\u015fken bu oran rifampisin, levofloksasin ve siprofloksasin i\u00e7in s\u0131ras\u0131yla %20.2, %19 ve %16.6 olarak tespit edildi.<\/p>\n<p> <strong>Sonu\u00e7lar<\/strong>: MRSA infeksiyonlar\u0131n\u0131n tedavisinde her ne kadar glikopeptidler ilk se\u00e7enek olarak kullan\u0131lmaktaysa da diren\u00e7 geli\u015fiminin \u00f6nlenmesi amac\u0131yla uygun indikasyonlarda fusidik asidin de antibiyotik duyarl\u0131l\u0131\u011f\u0131 test edilerek tedavide alternatif olarak kullan\u0131labilece\u011fi g\u00f6z \u00f6n\u00fcnde bulundurulmal\u0131d\u0131r.\u00a0Klimik Dergisi 2019; 32(1): 52-6.<\/p>\n<p> <strong>Cite this article as<\/strong>: Co\u015fkun MV, Alper Y, Uyan\u0131k MH, Yazg\u0131 H.
